We are seeking a Community Program Coordinator to play a key role in overseeing the integration of various teams at the Levi Watkins Outpatient Center (including support of the After Care Clinic) in order to address the social determinants of health as a routine part of health care.
Specific Duties & Responsibilities
Manage and develop the Hopkins Community Connection (HCC) Levi Watkins Outpatient Center Advocates and Student Leaders
- Oversee, develop, and motivate a team of 10 - 20 undergraduate and graduate student volunteer advocates.
- Be a daily presence at the clinic and various evening meetings to guide Advocates' professional and leadership development.
- Develop and manage a team of student leaders who oversee and support peers working as Advocates through biweekly meetings, individual coaching sessions, and trainings.
- Lead sessions in weekend-long trainings for Advocates and student leaders each semester, using collaboratively developed curricula.
- Actively collaborate with the other HCC program leaders to implement effective Advocate training design and delivery.
- Serve as local report for full-time Community Health Workers as needed.
Execute Program Operations
- Collaborate on design of and implement standards for quality case management; recommend program improvements to program team.
- Effectively manage a partial caseload of patients assessing and supporting client navigation to community resources.
- Find the best possible service(s) for the clients using resource tools, and researching new resources as needed.
- Work with individuals and families to create effective action plans to access resources, follow up regularly with clients on their action plans until the case is closed, and communicate case work with clinic teams.
- Communicate program highlights, challenges, and outcomes to Program Manager and clinic teams.
Build and Maintain Integrated Solution and Partner Relationships
- Create strong and healthy relationships with physicians, nurses, social workers, referring providers, and other clinic staff to build an integrated solution within the clinical setting and remain knowledgeable about the internal resource landscape.
- Support integration into the clinical setting by introducing case management best practices and providing training for clinic staff on operational practices for meeting patients' essential needs.
Ensure and Track High Quality Service Delivery
- Maintain records by documenting client case work according to clinic protocol and utilization of the clinic's Electronic Medical Record system.
- Ensure the accuracy of programmatic outcomes as reported by the client management database.
- Use program data to manage towards program milestones, related metrics and improve operations.
- Engage in group planning, evaluation and problem-solving and contributing to highly collaborative.
Potential Supervisory responsibility
- 1-2 Community Health Workers (as local report).
- 10 - 20 Undergraduate student volunteers.
Special Knowledge, Skills, and Abilities
- Proficiency with the Microsoft Office suite and comfort in technology-rich environments.

The Medical College of Georgia (MCG) is one of the nation's largest medical schools by class size, with 304 students per class. The MCG educational experience is anchored by the main campus in Augusta, with regional clinical campuses for third- and fourth-year students across the state, and two four-year campuses located in Athens (in partnership with the University of Georgia) and in Savannah (in partnership with Georgia Southern University). MCG's expanding partnerships with physicians and hospitals across Georgia currently provide more than 350 sites where students can experience the full spectrum of medicine, from complex care hospitals to small-town solo practices. MCG and its teaching hospitals also provide postgraduate education to approximately 575 residents and fellows in 50 different Accreditation Council for Graduate Medical Education-approved programs. Job Summary
This Community Program Coordinator position is designed to coordinate administrative and technical work in support of suicide prevention programming and intervention initiatives. This person will coordinate the planning, implementation, data collection, and data entry, with careful, exact documentation within the contract/grant and institutional guidelines. The position will include work with a state contract/grant to prevent suicide.
Responsibilities
Suicide Prevention Programming
- Attend state required meetings and webinars
- Implement Strategic Prevention Framework model for suicide prevention to include a needs assessment, capacity building, planning, implementation of selected strategies, and evaluation of programs
- Preparing state required reports monthly
- Data entry into state database of monthly activities
- Attend meetings with community partners
- Attending staff training and meetings.
- Participating in project planning and development of implementation plans, while insuring adherence to accepted scientific research principals.
Required Qualifications
Bachelor's degree from an accredited college or university in health-related field and one year of experience in health education, public health or community related activities.

Master's degree from an accredited college or university and experience in community prevention programming.

Experience in involving public or community health and/or community outreach.
Working knowledge or principles of child/ adult learning theory desirable.
