The Patient Care Coordinator is an on-site clinical team member that assists in coordinating care for the assigned team. The Patient Care Coordinator provides care navigation through daily triage and follow-up with referral sources, patients, clinicians, and clinical leaders through the management of incoming communication to the Clinical Manager. Family Hospice of UPMC is seeking caring, skilled, and compassionate PCC to join our team.
As a PCC for the dedicated team, you will work Monday - Friday daylight shifts 8:00 am-4:30 pm for the surrounding area. There will be rotations for on-call, holiday, and weekend coverage.

**Responsibilities:**
+ Participates in on-call coverage rotation for the agency.
+ Oversees the ordering and approval of medical supplies to ensure that all supplies are included on the physician orders and ordered in the appropriate quantity.
+ Monitors lab results, following up with physicians and team members.
+ Covers from other Patient Care Coordinators when needed.
+ Participate in interdisciplinary team conferences as needed.
+ Performs medication reconciliation on all therapy only cases as needed
+ Monitors admissions, discharges and re-hospitalizations and communicates with the appropriate clinical liaison(s) or facilities to facilitate continuity of care between settings.
+ Aids clinical staff in follow up communication with MD offices and referral sources as applicable.
+ Coordinates all incoming orders associated with census to ensure appropriateness and accuracy and timeliness of care, enters into Medical Record, and coordinates with scheduling as appropriate.
+ Assists in document control processes, including but not limited to, review of Face-to-Face documentation and physician orders for home health. Reports problems immediately to the Clinical Manager.
+ Triages and directors all incoming communications during workday hours for the current census assigned and follows up to facilitate appropriate service delivery.
+ Current Registered Nurse license in Pennsylvania. One-year recent home health experience preferred.
+ Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations; must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify appropriate resource for hand off.
+ Must have requisite knowledge of current Home Health Medicare, Medicaid, State and Federal regulations and Joint Commission accreditation standards. Must be able to demonstrate problem-solving abilities and have excellent telephone, interpersonal and written communication skills. Strong commitment to teamwork and customer service, both internally and externally, is required. Must have strong/proven data entry and general computer skills. Provides own transportation and auto insurance and abides by all laws, rules, and recommendations for safe driving while on duty.

What you'll be doing
Patient care. You will work directly with patients to provide safe, comfortable, and high-quality dialysis treatment. Responsibilities include gathering patient stats before and after treatment, initiating treatment, monitoring patients during treatment, terminating treatment, and collecting patient blood samples per physician orders.
Technician Duties. You will ensure quality comes first by preparing machines for hemodialysis treatments, cleaning and disinfecting machines after treatment, and conducting machine safety checks and logging results for quality control, ensuring all protocols and regulations are followed.
Safety and Quality. You will use appropriate safety measures, including personal protective equipment. Working under the supervision of a Charge Nurse, you will adhere to all company policies, procedures, and state/federal laws and regulations. You will participate in all required staff meetings and continuing education offerings.
PATIENT CARE TECHNICIAN
STATE SPECIFIC BOARD OF NURSING REQUIREMENTS
California
- Must possess current Certified Hemodialysis Technician (CHT) certificate from California Department of Public Health (CDPH) at time of hire OR
- Hired at USRC location recognized by state of California as an approved dialysis technician training program and obtain California state certification (CHT) within six (6) weeks of successful completion of training program.
Maryland
- Must possess current Certified Nursing Assistants -- Dialysis Technicians (CNA-DT) certificate from Maryland Board of Nursing OR
- Hired at USRC location recognized by state of Maryland as an approved dialysis technician training program and obtain Maryland CNA-DT certificate within three (3) months from date of hire OR
- Out-of-state applicant must have active BONENT certification and provide proof of initial application for CNA-DT certification.
New Mexico
- Must have New Mexico dialysis technician certificate at time of hire OR
- Hired at USRC location recognized by state of New Mexico as an approved dialysis technician training program and obtain New Mexico state certification within six (6) months of successful completion of training program OR
- New Mexico certification of hemodialysis technician is required for out of state applicant with active state or national hemodialysis certification; must obtain New Mexico state certification prior to working as a Certified Hemodialysis Technician.
- After January 1 st , 2024, all initial applications for the certified hemodialysis technician will require a national hemodialysis technician certificate.
- The applicant must submit proof of a valid national hemodialysis technician certificate from a recognized organization.
- Hemodialysis technician students must have a current basic life support credential prior to and while the student has contact with patients
OHIO
- Must have Ohio dialysis technician intern certificate at time of hire OR
- Hired at USRC location recognized by state of Ohio as an approved dialysis technician training program and submit application for Ohio dialysis technician intern certificate to board no later than four (4) weeks prior to completion of the approved training program.
- Must obtain Ohio Board of Nursing Certificate (OCDT) not less than twelve (12) months and not later than eighteen (18) months of enrolling in the USRC OCDT program.
OREGON
- Must possess current Certified Hemodialysis Technician (CHDT) certificate from Oregon Public Health Division at time of hire OR
- Must obtain Oregon dialysis technician provisional certificate within three (3) weeks of successful completion of training program and obtain Certified Hemodialysis Technician (CHDT) certificate from Oregon Public Health Division within eighteen (18) months. An Oregon Provisional Certification is valid for six months and can be renewed for one additional six month period.

We now offer a Night Program - if qualified, you could receive an additional $2 per hour on top of our current off-shift differential ($1.10). Must have 6 months of experience to qualify and must commit to work all full-time shifts as night shifts.
As a Patient Care Technician with UPMC, you'll have a meaningful impact in the care of our patients every day. We offer an eight-day training course for qualified entry level candidates to learn and demonstrate the skills needed to be a successful Patient Care Technician.
Responsibilities:

- Requests assistance in planning and prioritizing activities as needed. Participate in self-review as requested by the unit director.

- Provides direct patient care as directed by the RN including all activities of daily living such as bathing, feeding, transporting, toileting and ambulating patients.
- Acquires and distribute supplies and equipment to facilitate patient care delivery including, but not limited to, performing messenger functions, ordering, charging/crediting supplies, setting up patient care equipment within the scope of the nursing assistant position and stocking patient rooms and unit supply areas.
- Performs routine tests such as blood glucose monitoring, patient weights, vital signs and I&Os.
- Constructively receives feedback and direction.
- Provides companionship/supervision one in one with patients as required.
- Delivers patient nourishments, fill water pitchers and distribute patient mail.
- May apply, monitor, and remove patient restraints as per physician or hospital protocol. Provide diversion activities to reduce or prevent use of restraints.
- Documents in the medical record according to established procedures.
Experience Required:
- None (you will be provided with the appropriate training to prepare you for the role)
- High school diploma or equivalent.
- If no previous hospital or pre-hospital training, successful completion of UPMC patient care technician class OR successful completion of the first year of clinical work in a nursing program.
- Successful completion of basic information system training.
- Microcomputer experience preferred.
- Ability to effectively communicate both orally and in writing.
- Emergency Medical Technician (EMT) or Paramedic or currently enrolled in an EMT or Paramedic program preferred.
- Access to medications is limited to the distribution of the medication to the nurse.
- CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
