The treatment programs are accredited by the Commission on Accreditation for Rehabilitative Facilities (CARF).

WORK DESCRIPTION:

To develop and maintain cordial relationships with officers and departments of the court, including private attorneys who represent clients of the court who are charged with offenses involving substance abuse. Provides individual services to adult, substance-using clients by following evidence-based practices; assists clients in building recovery resources; and develops individual Recovery Plans that help clients enhance self-efficacy, maintain engagement in ongoing HIV medical care, reduce HIV risk and transmission behaviors, and continue recovery post-treatment.

ESSENTIAL FUNCTIONS:

  • To serve as a liaison between Haymarket Center and assigned Circuit Court(s) of Venue.
  • Provide recovery support services at time of discharge from substance abuse treatment services.
  • Engage clients in motivational interviewing and motivational enhancement techniques
  • Work with clients to develop a Recovery Plan that addresses their comprehensive needs
  • Develop a clear understanding along with implementation of the Recovery Coach Model implement Contingency Management in accordance with project protocols.
  • Meet with clients at least once a week to ensure implementation of the Recovery Plan
  • Assist clients in accessing community resources to meet their additional needs
  • Link clients to recovery support services
  • Support clients to engage family members and significant others in recovery planning
  • Monitor client engagement in medical care
  • Collaborate with other service providers to ensure linkage to needed services
  • Provide medication adherence counseling
  • Maintain contact with clients post-treatment for at least one year
  • Document all contact with client and collateral contacts in a timely manner
  • Participate in Health & Wellness events
  • Assist in completion of program evaluation and reporting
  • Attend programmatic meetings and collaborative events
  • Other duties as assigned
Requirements
  • Able to work with diverse populations, cultural backgrounds, and sexual orientations
  • Knowledge about HIV/AIDS a must

  • Bachelor's degree in human services or related field preferred

+ Firefighters will be responsible for the response to all hazardous material incidents as well as rescue situations involving but not limited to confined space incidents, aerial or high angle incidents, water related incidents, vehicle entrapment, and equipment entrapment.
+ Firefighters will be responsible for the response to all fire prevention and suppression activities of whatever kind or nature relating to structures, buildings, and other assets located on the plant property.
+ Firefighters will be expected to acquire a working knowledge of the fire detection, suppression, and transmission systems on the plant property including system operation, system deactivation, system support activities, and system restoration.
+ Firefighters will be responsible for the completion of various fire maintenance duties. These duties will include but not be limited to:
+ the pickup, refilling, servicing, delivery and required testing of plant fire extinguishers; low pressure hydrostatic testing;
+ refilling SCBA
+ inspection and recharging of C02 cartridges used in Ansul fire extinguish
+ fire alarm system testing;
+ hydrant testing;
+ fire department connection inspections;
+ Hose reel inspections and any other reasonable fire maintenance related tasks.
+ Firefighters will be responsible for completing all fire, fire maintenance and fire system related reports associated with their work duties.
+ **Qualified applicants for the Firefighter position will meet the minimum requirements, as described below:**
+ High school diploma or equivalent required.
+ Licensing requirements are subject to state and/or local laws and regulations and may be required prior to employment
+ Must have a clean criminal record.
+ Ability to successfully pass a drug screen/background check.
+ Individual must be able to pass a State level fit test and yearly NFPA Physical
+ Must have Fire I & II, Hazmat Awareness, Hazmat Operations, a valid EMT or EMR certification
**Responsibilities:**
+ Provide customer service to our clients by carrying out safety and security procedures, site-specific policies and when appropriate, emergency response activities
+ Respond to incidents and critical situations in a calm, problem solving manner
+ Conduct regular and random patrols around the business and perimeter. Working environments and conditions may vary by client site.
**Minimum Requirements:**
+ Be at least 18 years of age for unarmed roles; 21+ years of age for armed roles
+ Possess a high school diploma or equivalent, or 5 years of verifiable experience

As a Cleveland Clinic Home Care RN on the East team, you are responsible for delivering high quality support and medical services in a comfortable home setting within eastern Cuyahoga, Lake, Geauga and Ashtabula Counties. In this role, you will provide assessments, various treatments and education to help avoid readmissions. After seeing patients, you will complete documentation, make calls, attend meetings, coordinate additional care and arrange visit schedules. **A caregiver in this position works days from 8:00 a.m. -- 5:00 p.m. with on-call, every fourth weekend and one winter/summer holiday requirements.**
A caregiver who excels in this role will:
+ Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
+ Participate in a team-based care model led by the Care Coordinator in each region.
+ Implement and monitor patient care plans established by, and report patient needs after each visit to, the Care Coordinator for further case management.
+ Perform accurate and complete starts of care, resumptions, recertification's and routine visits as requested and within specified time frames.
+ Perform medication reconciliation at each visit.
+ Assess patients for safety risks, abuse and neglect.
+ Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
+ Report abnormal findings and/or changes in patient condition to the Physician and update Care Coordinators with those changes and any follow-up needed.
+ Communicate any patient's fall out/missed visits to the Care Coordinator immediately.
+ Participate in and support quality improvement projects and participate in department committees as assigned.
Minimum qualifications for the ideal future caregiver include:
+ Graduate from an accredited School of Professional Nursing
+ Current state licensure as a Registered Nurse (RN)
+ One year of current clinical experience as an RN
+ One year of experience in an acute medical/surgical or homecare setting
+ Valid Ohio Driver's License
+ Proof of automobile insurance coverage ($100,000/$300,000) upon hire and per policy 8-175 thereafter
