Under the general supervision of the Site Director, the Clinical Specialist – Crisis Services will provide therapeutic support, crisis intervention, clinical assessments, and evidence-based therapeutic interventions. This Clinician also delivers a person centered and goal-oriented approach to individuals experiencing acute mental health crises.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Conducts electronic case reviews.
  • Authorizes treatment plans.
  • Provides consultations utilizing medical necessity criteria.
  • Evaluates clinical appropriateness.
  • Monitors provider treatment plans to ensure quality and effectiveness of service.
  • Performs data gathering, documentation and analysis for desired outcomes.
  • Advocates for clients to ensure treatment needs are met.
  • Follows guidelines of the integrated care case management program with respect to identifying and assessing clients, developing and managing the treatment plans, and facilitating complex care arrangements.
  • Performs initial and ongoing review of client’s clinical status and functioning (behavioral and physical).
  • Communicates with medical and behavioral providers regarding treatment planning.
  • Communicates with medical and behavioral providers regarding clinical and psychosocial needs.
  • Engages the client and providers in identifying short-term and long-term goals consistent with the clinical situation and client’s strengths.
  • Manages client care through the MHWIN system.
  • Enters data and reports into written formats and electronic databases.
  • Monitors client’s compliance with services.
  • Participates in schedule shift reporting, multidisciplinary meetings, supervision, unit staff meetings and case consultation.
  • Assists clients by performing triage services and responding to urgent matters in an appropriate timeframe.
  • Communicates with outside partnerships such as Hospitals, Inpatient Psychiatric facilities, Outpatient Mental Health Clinics, and Partial Hospitalization Programs regarding Crisis Residential Services and bed availability.
  • Conducts comprehensive Behavioral Health Assessments for each individual and/or Pre-admission review as needed.
  • Completes all documentation including Behavioral Health Assessments, group and individual therapy progress notes and non-billable notes within a 24-hour timeframe.
  • Performs related duties as assigned.

Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line. What you can expect:

In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.PCTs will work in conjunction with dialysis RNs to provide dialysis care.

Patient Care: Spending the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment.

Complexity: DSD Dialysis care is a complex but fulfilling service line. Meeting and/or exceeding clinical outcomes are the goals and multitasking is an expectation to meet the needs of the service, the patients and our SNF partners.

Autonomy and Critical Thinking: Provide comprehensive patient care in an independent setting. Plan to be on your toes at all times. You use your mind constantly. You apply your "core" skills to monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup. **PURPOSE AND SCOPE:**

Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies procedures and training and in compliance with regulations set forth by the corporation state and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the FMCNA commitment to the Quality Enhancement Program (QEP) and CQI Activities including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FMC Quality Enhancement Goals (QEP).
**DUTIES / ACTIVITIES:**
CUSTOMER SERVICE:
+ Responsible for driving the FMS culture through values and customer service standards.

**PRINCIPAL RESPONSIBILITIES AND DUTIES**
PATIENT CARE:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC) Adequacy Monitoring Program (AMP) Urea Kinetic Modeling (UKM) and regarding disease process/access.
+ Welcome assigned patients and inquire as to their well-being since their last treatment. Report any complaints or observations to the nurse supervisor.
+ Evaluate vascular access for patency and report any unusual findings to nurse supervisor.
+ Obtain necessary pre and post treatment vital signs and weight. Report unusual findings to nurse supervisor.

+ Perform vascular access cannulation and report any difficulties in cannulation or unusual findings to nurse supervisor.
+ Perform administration of Heparin as delegated or as allowed by state law.
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy and report any unusual findings to nurse supervisor.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures
+ Evaluate patient prior to termination of venous access - standing & sitting blood pressure.
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient for prior to discharge and report any unusual findings to nurse supervisor.
+ Perform and record Pre and Post dialysis evaluation weight and vital signs with initial identification.
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled including Narrow Venous Limits (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and if applicable progress notes.
+ Report any significant information and/or change in patient condition directly to the nurse supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the nurse supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures provide appropriate intervention as needed document and report any unusual findings to the nurse supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
MAINTENANCE/TECHNICAL:
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface chair equipment and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate.
+ Perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
+ Initiate Solution Delivery System (SDS) system.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare organize and efficiently utilize supplies and equipment to prevent wastage.
CLERICAL & ADMINISTRATIVE:
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
+ Enters all treatment data into the designated clinical application accurately and in a timely manner.
+ Review treatment sheets for completeness ensure nursing signatures are documented and ensure omitted entries are completed or corrected where appropriate.
+ Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
+ Prepare lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
+ Collect label appropriately prepare and store lab samples according required laboratory specifications.

OTHER:

WHAT WE ARE LOOKING FOR:

Role/Position Definition: Responsible for the provision of direct care to patients under the direction of a Registered Nurse and/or Licensed Practical Nurse. Assists in the assessment and monitoring of patients’ conditions and therapeutic needs. Responsible for the provision of direct care to patients to assist with all Activities of Daily Living (ADL) including elimination, nutritional needs, mobility and emotional support. In addition, accurate completion of vital sign flow sheets, charges for supplies and other duties deemed within their scope of practice.

Qualifications/Position Requirements:

  1. Education/Experience

  1. Completion of a Nurse Aide or Patient Care Technician training program preferred.
  2. 2+ years of experience as a Nurse Aide strongly preferred.

  1. 2+ years of inpatient experience providing care in a hospital setting preferred.

  1. Licensure/Certification

  1. Current unrestricted Nurse Aide License from the State of Michigan preferred.
  2. Current Basic Life Support Certification for Healthcare Providers, from the American Heart Association, required.

  1. Knowledge, Skills, and Abilities

  1. Knowledge of BLS protocols in code situations, pain management, wound care, and performance of patient assessments.
  2. Knowledge of medical terminology.

Duties & Responsibilities:

  1. Transfers patients using appropriate assistive devices and help from others. Assists patient with safe transfer techniques using proper body mechanics.
  2. Answers call lights promptly and in person and places the call light in reach of the patient taking into account their disability, making alternative arrangements that meet the patient’s limitations.
  3. Assists in meeting patient’s dietary needs by aiding with feeding when necessary.
  4. Keeps patient dry at all times. Meets patient’s elimination needs promptly with attention to maintaining skin integrity. Changes and/or empties ostomy, Foley bag, rectal bag and others as per policy and records output.
  5. Assists in keeping nursing area and hospital free of hazards including electrical, physical, and mechanical. Responsible for helping maintaining patients’ rooms cleanliness/neatness and keeps patients’; rooms free of hazards.
  6. Provides input regarding the development, monitoring and updating of individualized care plans.
  7. Walking rounds done at least every hour on assigned patients.
  8. Creates and maintains a positive atmosphere of customer service, including continuous quality improvement, team work, mutual respect, professionalism and accountability.
  9. Attends and participates in all appropriate departmental/SCMC meetings and training and development opportunities.
  1. Follows established policies and procedures, including but not limited to:
    1. SCMC employment policies and procedures
    2. Infection control/safety policies and procedures
    3. Federal, state, and local regulations
  1. Demonstrate the knowledge and skill necessary to provide the care for patients appropriate to the age of the patient served including but not limited to, principles of growth, development and psychological needs.
  2. Performs/assists patients with activities of daily living skills including bathing, dressing, oral hygiene, peri-care, application of prosthesis/orthotic, and/or assistive devices, with the goal of patient independence. Accurately documents all care provided and patient’s response in EMR prior to the end of the shift.
  3. Accurately measures and records temperature, pulse, respirations and blood pressure, pulse oximetry, oral intake and outputs. Accurately notes and records bowel movements and assists with the tracking of bowel and bladder training programs. Notifies nurses of any abnormal findings. Documents any findings in EMR.
  4. Provides for the patient’s mobility needs completely as prescribed by written orders and/or care plan. Transfers patients from bed to and from wheelchair, stretcher, toilets/commodes using appropriate assistive devices and help from others. Assists patient with safe transfer techniques.
  5. Conducts daily environmental rounds and reports findings to the RN/LPN.
  6. Monitors inventory and maintains appropriate levels of supplies and equipment. Makes ordering recommendations.
  7. Assists with wound care at the direction of the RN/LPN on duty
  8. Performs all other duties as assigned.
