$78,773.63 - $122,099.12 The Perioperative Clinical Service Line Coordinator – Control Desk reports to the Nurse Manager and holds a key leadership position within the division. In collaboration with the Nurse Manager, this leader assumes the responsibility of coordinating and articulating the philosophy, objectives, policies, and procedures of the Medical Center, the Nursing Service, and Perioperative Nursing into effective action. This nurse must have strong leadership and communication skills in order to maintain a safe and caring environment that supports excellence in clinical practice and customer service. This individual serves as a unit-based leader and acts as a liaison to clinical staff, physicians, support staff, and others involved in promoting positive patient outcomes across the continuum.
  • In depth knowledge of and ability to implement the AORN Standards and Recommended Practices for Peri-operative nursing.
  • Analytical skills to resolve problems that require the use of basic scientific, mathematical, or technical principles.
  • Communicates cooperatively and effectively with patients, family members, employees and others through verbal and communication.  Engages in interactive dialogues with the multidisciplinary team. Seeks out the input of others to achieve consensus. Reads and interprets documents such as Patient Care Standards and Quality Reports.  Problem-solves, performs computations, and interprets data. Interfaces with other Perioperative leadership staff to coordinate daily needs of the unit. Researches, distributes, and communicates information on new equipment, supplies and procedures. Provides input for staff meetings in collaboration with Nurse Manager. Meets with staff to problem solve supply management and equipment issues. Liaisons with all departments and disciplines that interface with the Perioperative area to facilitate positive patient outcomes.
  • Ability to exchange information on factual matters, schedule appointments, greet visitors, explain medical center polices, and/or relay messages to appropriate personnel. This type of interaction requires courtesy and tact when dealing with patients, visitors and/or medical center employees. More sensitive situation may be referred to others as appropriate
  • Ability to be honest, to be punctual, and to perform the job in adherence to the highest standards of ethical conduct as defined by the AMC Code of Conduct. Ability to be accurate in such matters as record-keeping, and to use good judgment when performing the functions of the job or when interacting with others. Ability to accept direction, carry out orders, to work cooperatively with others and to avoid the creation of unnecessary conflict.  Ability to adhere to AMC’s policies, procedures and practices, and to utilize AMC’s problem-solving processes for resolving grievances or disagreements.
  • Ability to independently organize and prioritize work to complete assignments in a timely manner and meet established guidelines. Ability to multitask and gather data from appropriate resources.
  • Accepts responsibility for self-development to remain cognizant of current trends and new methods within the nursing profession and health care environment
  • Computer skills such as those found in the Microsoft Suite as well as EPIC.
  • Bachelor of Science in Nursing (BSN) preferred (current and continuous enrollment in a BSN program with completion from an accredited school of nursing within 2 years is also acceptable). 
  • Current New York State licensure as Registered Nurse required.
  • CNOR is preferred (current pursuit of CNOR with completion within 2 years is also acceptable)
  • Valid CPR required.
  • Three years previous Operating Room experience.
  • Demonstrates clinical competency as an OR nurse in both the scrub and circulating roles.
  • Demonstrated leadership experience charge or resource role.
  • Previous management/supervisory experience preferred.

POSITION SUMMARY : The Patient Care Coordinator (Nursing) is accountable for the administrative and clinical coordination and management of multidisciplinary care teams. The coordinator ensures that patient care's administrative and clinical aspects are consistently maintained at the highest home health standards, continuity of care, and service delivery are most efficient. 

ESSENTIAL DUTIES & RESPONSIBILITIES:

Care Coordination and Scheduling

Acting as the administrative liaison between the agency,  patients, caregivers, payers, and providers. 
  • Assist in managing patient care and ensuring quality service and satisfaction. 

  • Works in partnership with administrative and clinical personnel throughout the agency. 

  • Monitor the work of schedulers, field clinicians, and other administrative office personnel and assist in working more effectively.

  • Manage intake and referral processes, including insurance verification and authorization.

  • Coordinates schedules for in-home visits by nurses, therapists, and other care providers.

  • Maintain accurate and up-to-date records in the EHR.

Patient and Family Communication

  • Act as the primary point of contact for patients and families regarding care plans and service updates.

  • Communicate with empathy and clarity, keeping family members informed about care plans, schedule changes, or new physician orders.

  • Address the concerns/complaints of the patient, caregiver, client, provider, and other stakeholders, resolving the issues and escalating as needed.

Interdisciplinary Team Collaboration

  • Facilitate and participate in multidisciplinary case conference meetings with administrative and clinical personnel to coordinate the delivery of patient care.

Care Plan Management

  • Administratively partner with admitting clinician and other administrative personnel to facilitate the completion of the plan of care (485) and other administrative paperwork for compliance.
  • Maintain and update individualized care plans for each patient.

  • Monitors patients’ progress and ensures timely reassessment and care plan updates.

  • Coordinate with the clinical team to adjust care plans or visit frequency as necessary.

  • Ensure services are delivered in accordance with the physician's orders and agency policy.

Patient Follow-Up

  • Conduct regular follow-ups with field clinicians through supervisory home visits to check patients' well-being and confirm that the care instructions are being followed.
  • Conduct regular follow-up through phone calls to identify any new needs and service satisfaction.

Documentation and Records

  • Performs data entry, record keeping, correspondence, computer composition, technical drafting, and office work.
  • Answers the phone, answers routine (non-clinical) administrative and clinical questions from patients, physicians, and providers reading information from an electronic health record (EHR).

  • Maintains confidentiality and safety of patient clinical records.

  • Maintains administrative compliance with the submission of all clinical orders, authorizations, visits, and other documents required by federal and state regulations.

Regulatory Compliance

  • Assist the management team in administering, planning, and facilitating in-services. 
  • Assist the management in compliance with the required documents required by the state and federal regulations.

  • Assist the management team in compliance with the Medicare/Medicaid requirements and HIPAA privacy rules.

  • Comply with accepted professional standards and principles.

  • Verify that the care delivery matches the physician's orders and authorized frequencies.

  • Assist in managing compliance with the OASIS assessment transmittal to the federal government.

  • Maintain compliance with Medicare, Medicaid, and other payor requirements.

Quality Assurance

  • Participate in quality improvements and compliance activities.

  • Conduct audits and review patient charts for completeness.

  • Work with the management team to develop better processes for care coordination.

  • Assist in identifying problems with performance and developing solutions to those problems.

  • Provide input on improving patient satisfaction and outcomes based on feedback and observation.

  • Initiate informal measures to correct performance issues for formal disciplinary actions to the management team.

  • Recommend training needs to improve performance of the administrative and clinical team.

  • Perform other administrative & clinical duties and activities as delegated.

EDUCATION/ LICENSURE REQUIREMENTS:

This job works collaboratively to maintain patient safety and provide for the delivery of basic patient care under the direct supervision of a licensed nurse. May provide constant observation of the patient and notifies nursing personnel of changes in behavior or status and when leaving the room, and intervenes as needed to maintain patient safety. **Education**
Preferred - High School diploma or equivalent.
**Work Experience**

**Certifications**
Required - Current state of practice Certification as a Nurses' Assistant (CNA); Basic Life Support (BLS) from the American Heart Association to be obtained within 30 days from hire; Crisis Prevention and Intervention (CPI) to be obtained within 30 days from hire.
**Knowledge Skills and Abilities (KSAs)**
+ Proficiency in using computers, software, and web-based applications.
+ Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
+ Excellent organizational skills.
+ Excellent time management skills.
+ Ability to work a flexible work schedule (e.g. 24/7, weekend, holiday, on call availability).
**Job Duties**
+ Supports the plan of care as delegated by staff nurse within scope of skills verification.
+ Maintains a clean and safe environment in the patient care area.
+ Provides care based on physical, psychosocial, educational, safety and related criteria, appropriate to the ages of patients served in assigned areas.
+ Communicates patient care needs effectively with patients, families, and other staff members, including multi-disciplinary teams.
+ Completes documentation of patient related tasks.
+ Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
+ Performs other related duties as assigned.
