Job Summary :
The Manager of Community Care is responsible for the development, implementation, relationship management, oversight, and evaluation of key partnerships and community benefits targeted to improve health outcomes and member experience in alignment with the model of care and strategic initiatives. This position directs and supervises a team of Community Health Workers (CHWs) delivering services to the pediatric population.
Duties and Responsibilities:
- Plans and implements programs to meet the social and emotional needs of members and members' families.
- Develop and maintain a resource library of services available to children and families through their benefits and other community resources.
- Partner with healthcare and social services providers across the delivery system to collaboratively address gaps related to access for pediatric members and families; these partners include: hospitals, pediatricians, Community Based Organizations (CBOs), Social Determinants of Health (SDOH) and Behavioral Health (BH) providers, and others.
- Develop, implement and maintain workflows, job aids, policies and procedures for CHW staff to successfully execute their job duties.
- Conduct analyses on members' utilization of services and identify opportunities for improving member experience and health outcomes.
- Communicate the findings of analyses with various stakeholders, solicit input to generate well-informed recommendations, and leverage findings to develop and implement interventions or revisions to existing programs.
- Accomplish staff results by communicating job expectations; planning, monitoring, and appraising job results; coaching, counseling, and implementing corrective action steps when necessary; developing, coordinating, and enforcing systems, policies, procedures, and productivity standards. Complete performance evaluations in a timely manner.
- Support direct staff through individual supervision, group supervision, or in vivo observation.
- In collaboration with the Chief Health Officer and other departments, monitor and regularly evaluate program status, and develop and implement internal and external communications to keep key stakeholders informed (e.g., the Leadership Team, San Mateo Health Commission, HPSM network providers, and other partners).
Requirements and Qualifications:
- Master's degree in social work or psychology.
- Current and unrestricted CA Licensure: LCSW, LMFT, or Psychologist (required).
- Minimum three (3) years of care delivery experience, preferably with Medicare/Medi-Cal experience or with pediatric populations.
- Two (2) years' experience in a managerial role.
- Direct operational management experience or health care project management experience in a matrixed reporting structure.
- Bilingual skills preferred.
- Change management, process improvement, and redesign tools.
- Microsoft Office Suite applications, including Outlook, Word, Excel, Access, Visio, and PowerPoint.
- Managed healthcare regulatory requirements.
- Supervisory principles and practices, as well as techniques and methods to organize and manage direct reports.
- Social determinants of health and principles of community health work.
- Digest and synthesize regulatory requirements for diverse audiences.

Program Manager - Community Care
Oversee daily operations and staff at an Affordable Housing site, coordinate resident services and partnerships, develop service plans, manage events and communications, ensure compliance with documentation standards, and foster a collaborative, mission-driven team culture aligned with Catholic Charities' values.
Job Responsibilities
- Manage day-to-day operations of Care Coordination office and activities at assigned Affordable Housing site.
- Provides referrals to service agencies and service providers to the residents and monitor service delivery. Coordinate bi-annual building assessments to identify residents who are at risk.
- Develop annual service plans.
- Maintain updated service directory of local resources, create, and maintain partnerships with external partners.
- Assist in coordinating resident experiences (monthly building meetings, monthly resident newsletters, clubs, events, outreach activities.)
- Directly supervise assigned Care Coordinators at other designated Affordable Housing sites.
- Responsible for assigning work to staff and ensuring that work is performed in an efficient, prompt, professional and accurate manner and provides guidance when necessary.
- Completes all required documentation in accordance with agency/programmatic policies and procedures.
- Assist in handling resident complaints.

Relevant Education:
Minimum: Bachelors Degree
Preferred: Masters degree
Relevant Experience:

Intermediate computer and data entry skills.

Excellent written and verbal communication skills.
Strong interpersonal skills.
Strong organizational and multi-tasking skills, with the ability to manage multiple projects simultaneously.
Familiarity with community resources and services.

Minimum Education: Bachelor's degree social work, counseling or related field

**The role you'll contribute:**
Works collaboratively across the region with community care nurses, physicians, staff and other health care professionals within the Central Division North Region and PHSO network, to provide care coordination across the health care continuum for patients within the Community Care/Ambulatory Care Coordination program. Serves as an integral member of the health care team who works to ensure members with complex issues or high utilization, navigate through the health care continuum while improving member/family experience and eliminate care gaps through addressing the social determinants of health. Coordinates a wide range of community-based and healthcare support services for members.

+ Provides Coordination of Care across the care continuum including a manner consistent with safe, efficient and cost-effective resource utilization.
+ Assists in the identification of patient populations needing care coordination
+ Works with families and patients on needs that may or will affect the patients health, including all dimensions of the social determinants of health such as, transportation issues, financial concerns, end of life planning, etc. in all community settings such as: members home, primary medical home (PCP/FQHC) etc, to eliminate fragmentation, duplication or gaps in health care.
+ Identifies potential barriers related to patients home setting and self-care/management needs.
+ Maintains an active case load of at risk patients.
+ Optimizes member independence through providing education and links to community resources
+ Strong knowledge regarding Social Services and Care Management processes in preadmission, emergency department, outpatient/ambulatory services, and post-acute care services.
**The expertise and experiences you'll need to succeed:**

+ Master's Degree
+ Clinical Social Worker License (LCSW)
+ Driver's License (DL)
+ Computer skills and experience with Microsoft Office programs, including Word, Excel, Outlook, and PowerPoint are
+ Verbal and written skills necessary to effectively communicate with various members of the health care team, other health facilities, community health related organizations, various external parties and regulatory agencies.
+ Ability to use office equipment such as telephone, personal computer, copier, fax machine, etc.
