Job Description
Incumbent is responsible for tracking and auditing services to ensure we are meeting quality of care and grant expectations. This individual is the lead on working with any individual with a disability to ensure proper access. Responsible for taking the lead on schedule of shelter coverage.
Essential Job Functions:
- Assist Director in coordination of direct services between all sites.
- Facilitate weekly team meeting and communicate to director or designee any concerns
- Manage referrals for service, census list and Master Lists for all sites.
- Facilitate various psycho-educational groups with residential and outreach clients
- Chart all pertinent information including individual sessions and document progress toward achieving goals.
- Provide transportation to residents for goal-related activities.
- Answer 24-hour hotline and provide crisis support and advocacy as needed. Record calls in Phone Log book.
- Conduct telephone intakes for women seeking shelter or resources and willing to enter our program.
- Complete necessary documentation for clients including assessments, consent forms, etc.
- Provide crisis intervention support to clients.
- Create and maintain client files.
- Ensure team members has what they need to provide appropriate service to clients (training, supplies etc.)
- Provide ongoing support to community members and organizations regarding Domestic and Sexual Violence.
- Provide advocacy including, legal/court advocacy to residential or outreach clients.
- Track statistics including bed nights, hotline logs and groups.
- Assist Director with outcomes and reporting.
- Facilitate ongoing meetings with clients regarding satisfaction with services and coordinate follow up services.
- Attend meetings in the community to coordinate services for at risk or underserved populations including individuals with disabilities.
- Provide lead case management services to individuals with disabilities
- Provide ongoing information during team membersing regarding individual issues and concerns, as well as clients' progress towards goals.
- Maintain a supportive, professional relationships with all team members, volunteers, and students.
- Participate in QI activities.
- Complete other duties as assigned by supervisor.
Knowledge, Skills, and Abilities:
- Basic knowledge of domestic Violence and Sexual Assault.
- Basic knowledge of trauma
- Demonstrates good judgment and discernment in dealing with people.
- Capable of communicating ideas clearly and effectively
- Demonstrate the ability to operate a computer and operate Google applications.
Experience and Education Qualifications:
- Bachelor's Degree in Social Work, Counseling, Psychology, Sociology, or related field and one-year experience in human service organization;
- OR
- Associate's Degree and two years' experience in human services field;
- OR
- Three years' experience providing case management and advocacy pertaining to resource acquisition in a human service organization;
- OR
- A survivor of domestic violence who has demonstrated knowledge and/or experience working in the field of domestic violence and two years' experience providing case management and advocacy pertaining to resource acquisition.
- Demonstrated ability to operate a computer and use Microsoft Office applications.
Supervisory Requirements:
- Communicate the strategic direction of the organization and encourage participation by all team members.
- Provide leadership and guidance to all aspects of the department.
- Take an active role in monitoring the identification, development, and execution of strategic objectives.
- Involve, as appropriate, all team members to achieve goals.
- Effectively communicate to team members any changes and newsworthy events within the department or company.
- Handle difficult team member situations directly, using appropriate discretion and Human Resource advice to show respect for the individual
- Champion change and effectively manage the implementation of new ideas.

- Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.

BASIC FUNCTION:
Service provider performs duties for the care, treatment, and safety of the individuals served with developmental/physical disabilities. This includes but is not limited to: personal care, program completion and documentation, and/or occupational and physical therapy programs.
DESCRIPTION OF ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Reliable, prompt attendance demonstrating flexibility and cooperation with team as needed to provide adequate facility coverage and follows team (self)-scheduling guidelines.
- Monitors all individuals served at all times and communicates all information in communication log and if applicable, verbally reports to next staff scheduled.
- Completes all daily assignments as directed by Team Leader and/or Residential Services Coordinator (RSC)/Home coordinator (HC);
- This includes but is not limited to:
- Individual Service Plans/Personal Plan Implementation Strategies
- Behavior Development Programs
- Charting and documentation
- Initiates recreational and social activities in the home and community as well as ensures all scheduled outings are completed, including Special Olympics practice(s).
- Must pass all medication training to administer medication and provide active, ongoing interaction to the individuals served.
- Maintains the home in a clean, safe, healthful condition, logging repair requests correctly.
- Assists individuals in care of clothing and personal items.
- Assists in all emergency situations and is knowledgeable of fire extinguishers, alarms, and disaster.
- Uses all mechanical lifts, belts, chairs, and other mobility equipment in facilities where necessary, appropriately, and cleans them as necessary. This includes all wheelchairs. Utilizes good body mechanics at all times.
- Provides safe transportation of individuals served; uses safe, defensive driving skills, adhering to all policies and procedures regarding the use and care of Milestone vehicles.
- Responsible for knowledge of basic cooking skills or willingness to learn. Follows the established menu plan, adhering to prescribed diets. Develops shopping lists or items need to prepare planned meals, purchases items as needed (staying within designated budget), and submits receipts to Team Leader/RSC/HC.
- Maintains cordial and cooperative relationship with individuals served, their families and guardians and other staff members, utilizing and promoting positive problem solving skills and ensuring a positive, pleasant home environment.
- Monitors individuals' health needs and documents such; accompanies individuals on medical/dental appointments as assigned by Team Leader/HC/RSC.
- Complies with all Milestone, Inc. policies and procedures.
- In compliance with Milestone, Incorporated's philosophy and policies toward Human Rights; immediately intervenes on behalf of the individual(s) served and reports to the appropriate supervisor and/or agency (OIG, IDPH, DCFS.)
- Orders necessary forms and supplies from the appropriate personnel as needed.
- Other duties as needed or assigned.
QUALIFICATIONS:
Minimum age of 18.
Completion of Habilitation Aide Training within the first 30 days of employment.
High School diploma or GED and passing of TABE testing.
Strong written and verbal communication skills
Must be able to intervene physically in behavioral or health related crisis.
Safe driving record/approved driver.
