In this position your primary duties will involve spending multiple hours with patients while administering a variety of cognitive and psychological tests, as well as scoring the assessments after the evaluation. These tests measure things like intelligence, memory, personality, and mental health conditions. Psychometrists working in the role of community support workers are under the supervision of neuropsychologists and do not diagnose patients themselves, but they play a key role in gathering important data with the assessments and their behavioral observations during the evaluation. Psychometrists also communicate with the attending provider during the evaluation to ensure the evaluation is completed in a manner that ensures the best patient care is provided. You will also help make sure the proper resources have been set up prior to the appointment, for example that interpreter services have been requested when necessary, and also make sure that patients have adequate understanding of what a neuropsychological evaluation involves prior to their appointment.

Community Support Worker
REPORTS TO: Clinical Manager
PURPOSE
Under the direction of a clinical manager, the Community Support Worker provides support
to consumers and their families or caregivers with mental health and substance abuse
related issues. The Community Support Worker will facilitate individual mental health
intervention and activities that are strength‐based and focused on promoting recovery,
symptom reduction, while increasing coping skills and help to achieve the highest level of
functioning in the community.
EDUCATION& EXPERIENCE
• Bachelor's degree or at least 2 years of related work experience
GENERAL RESPONSIBILITIES
• Ensure all necessary licenses and/or certifications remain current
• Provide medically necessary services to the consumer in accordance with the
treatment plan that is designed to meet the mental health, educational, vocational,
residential, financial, social and other needs of the consumer.
• Follow Mental Health Rehabilitation Services (MHRS) guidelines
• Ensure that treatment plan authorizations and reauthorizations, CALOCUS/LOCUS
and updated diagnostic assessments and all other assessments are current and DO
NOT expire
• Serve as an intricate member of the SLS team to ensure continuity of care and
supports the services 24 hours a day and seven days per week, as necessary
• Provides structured face‐o‐ e s eduled sessions to consumers at least once per
month, or as needed based on the treatment plans
• Provide services in the home, school, office and community setting
• Ensure that the majority of mental health services are primarily rendered in the
community and limited services remain clinic-based.
• Complete documentation within 24 hours of service or report to supervisor via phone

• Life Skills Adaptation: Help clients adapt skills across various areas of life, including home, work, education, leisure, and social settings.
• Crisis Intervention: Provide immediate support during crises to ensure client safety and well-being.
• Training & Development: Complete required training sessions, both in-person and online, to stay current with best practices.
• Documentation: Maintain accurate and detailed documentation of client interactions and progress on a daily basis.
• Productivity Standards: Meet established productivity standards for the role.
• Minimum Age Requirement: Must be at least 18 years old to meet licensing standards.
• HIPAA Compliance: Adhere to HIPAA regulations and maintain the confidentiality of client information.
• Meeting Attendance: Schedule and attend necessary meetings to coordinate care.
• Attendance: Maintain regular and reliable attendance to ensure continuity of care for clients.

• Education: Must have a High School diploma or GED.
