**About this Unit:**
The Newborn Intensive Care Unit (NICU) is a 14 bed level 3 that cares for infants that are 28 weeks gestational age and older. St.John's NICU offers nasal CPAP and ventilators.Our NICU is newly remodeled and have expanded our care to higher risk patients (28 weeks gestational age).
The Patient Care Supervisor:
+ Partners with manager to supervise/lead day-to-day or shift-to-shift operations to ensure support of the mission, vision, values and goals of the organization.
+ Assists manager to plan, lead, coordinate and evaluate patient care activities including staffing, resource management, quality and safety, regulatory compliance, and financial management.
+ Prioritizes assignments to assure timely, productive, and efficient use of resources.
+ Assists manager to lead equipment inventory and supply management within operational budget.
+ Collaborates with nursing, provider leadership and other partners to implement and evaluate standards of practice.
+ Collects and analyzes quality data and applies process improvement methodology to insure delivery of safe and quality patient care.
+ Assists with department change and acts a change agent in of system change, procedure change and process changes.
+ Demonstrates skills/abilities in provision of patient care.
+ Ensures unit protocols, benchmarks, and hospital procedures are initiated appropriately.
+ Assists the manager to develop, implement, supervise and evaluate the quality improvement initiatives.
+ Assists with human resources management to achieve quality service and positive employee relations.
+ Ensures compliance with organizational policies and procedures.
+ Advocates for patient, family and staff needs.
+ Drives improvement activities that promote patient centered care and positive customer experience.
+ Assists manager to ensure staff compliance in areas of job requirements, education and clinical competency.
+ Promotes a culture of high employee engagement, professional development, nursing education and research.
**Required Qualifications:**
+ Bachelor's degree in Nursing. If Master's degree is in nursing or healthcare, Bachelor's degree in other area of study will be accepted.
+ Two years of nursing experience, at least one of which in a formal or informal (charge, lead, preceptor) leadership role. Knowledge of patient population served in area of responsibility.
+ Licensed Registered Nurse. Ability to obtain Minnesota nursing license at time of hire.
+ BLS certification
+ NRP Certification
**Preferred Qualifications:**
+ Master's degree in nursing, health care administration, business administration or related field.
+ ANCC or AONL Certification
+ Prior NICU experience

**Overview:**
This Patient Care Management Coordinator provides comprehensive care coordination of patients as assigned. The care coordinator assesses the patient's plan of care and develops, implements, monitors, and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patient's health care needs. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements. This position is accountable for the quality of clinical services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care. This position has responsibility to determine how to best accomplish functions within established procedures, consulting with leader on any unusual situations. Internal customers include all levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External customers include physicians and their office staff, payers, community agencies, provider networks, and regulatory agencies.
**Responsibilities**
+ Manages patients across the health care continuum to achieve the optimal clinical, financial, operational, and satisfaction outcomes.
+ Acts as one point of contact for patients, physicians, and care providers throughout the patient's hospitalization.
+ Initiates/implements transition functions and activities for patients communicating with patients, families, and the health care team to ensure seamless transitions.
+ Assesses patient admissions and continued stay utilizing evidence-based criteria.
+ Contributes to the development and implementation of individualized patient care plans.
+ Collaborates with health care team partners and patients/family to manage the patient discharge plan.
+ Effectively communicates the plan across the continuum of care.
+ Assist in the development and implementation of process improvement activities to achieve optimal clinical, financial and satisfaction outcomes.
+ Enables efficiency in care by identifying and reducing delays, ensuring appropriate level of care, facilitating length of stay reductions, and identifying resources to promote a safe and effective discharge.
+ Collects data and other information required by payers to fulfill utilization and regulatory requirements.
+ Identify and communicate, to appropriate leader, all issues related to case escalation.
+ Establishes a collaborative relationship with physicians, medical directors, nurses and other unit staff, and payers.
+ Demonstrates effective communication by being a critical link with attending and consulting physicians and all health care team members and payers. Facilitates resolution to any identified issues.
+ Mentors internal members of the health care team on case management and managed care concepts.
+ Understands and focuses on key performance indicators.
+ Actively tracks outcomes and participates in quality planning.
+ Facilitates integration of concepts into daily practice.
**Required Qualifications**
+ Bachelors Degree in Nursing

Our unit offers 29 LDRP (labor, delivery, recovery & post-partum) beds plus 2 triage rooms. We have 1 water birth room and 2 OR suites. We are pleased to provide single room care, which provides patients with excellent care all within the same room throughout their stay. We average 200+ deliveries a month and manage many outpatients daily for antenatal testing. Daily census is 18 mom and baby couplets. Our NICU is newly remodeled and have expanded our care to higher risk patients (28 weeks gestational age).
**Schedule:** This position will be working **1.0** **FTE** (80 **hours** every two weeks). The shift length will be 8 **hours** with day/evening hours and weekends as needed.
**Job Expectations:**
+ Manages day-to-day operations of assigned area(s) to ensure support of the mission, vision, values and goals of the organization.
+ Plans, manages, coordinates and evaluates patient care activities including staffing, resource management, quality and safety, regulatory compliance, and financial management.
