Summary:
+ The Clinical Care Manager is responsible for promoting organizational values of patient and visitor satisfaction by demonstrating courtesy and respect for families, visitors and other employees. Collaborates with practices and actively provides clinical support to "high-risk" patient populations for multiple practices - in all age groups ranging from young adulthood through geriatrics. Demonstrates competence in the assessment, treatment and care of patients serviced. Will include face to face (travel required), telephone or telehealth encounters as appropriate to patient needs.
Responsibilities:
+ Designs and implements care management support activities for high risk patients with chronic diseases and high medical service utilization.
+ Collaborates with physician and practice staff in identifying appropriate patients for care management utilizing established criteria. Collaborates with the patient and provider to establish and implement a care management plan using evidencebased guidelines. Develops patient objectives and measures progress according to standards of care.
+ Triages patient telephone calls and provides consultation regarding ongoing symptom management resulting from treatment and disease, analysis of intervention regarding diagnostic work-up, assessing and managing acute emergencies, managing family concerns as needed, liaison activities with affiliated physicians, nurses, pharmacists and emergency rooms.
+ Initiates appropriate evaluation and management of emergency situations.
+ Coordinates patient's management/treatment plan throughout the institution.
+ Instructs patient/family regarding medications and treatments. Educates patients regarding health and illness prevention. Recommends community resources to meet patient and family needs. Develops patient education programs.
+ Creates, uses, and maintains patient-specific care management tracking system that identifies/ reports on patients who will benefit from care management services, the subsequent patient progress, and associated interventions.
+ Supports QA and maintenance efforts for patient registries and quality reporting. Works with the Clinical Analyst to summarize and analyze data from registries
+ Promotes patient self-management and empowers patient and families to achieve optimal health outcomes. Assists patient to identify and/ or develop and utilize their support systems.
+ Evaluates the effectiveness of care management plan in meeting established care goals and revises the plan as needed to reflect changing needs, issues and goals.
+ Prepares and maintains appropriate documentation as required.
+ Collaborates with all members of the health care team to resolve patient health, social or behavioral health needs.
+ Provides monitoring and continuity of care between physician visits. Maintains and reviews patient records, charts and other pertinent information.

Credentials:
+ Active PA or NJ RN license based on position location. Dual (NJ & PA) RN licensure required within 3 months of hire (Required)
Education or Equivalent Experience:
+ Associate's Degree NLN- approved school of nursing (Required)
+ Bachelor's Degree Nursing And 1+ years prior nursing experience in internal/family medicine and care management experience (Preferred)

Job Responsibilities:
+ Providing general patient care including grooming, daily stall cleaning and disinfecting, feeding and watering, walking, applying standing wraps, and stocking clinical areas.
+ Horse handling and restraint for procedures, medical exams and surgery, hydrotherapy, and additional animal husbandry duties as assigned.

The Clinical Care Management Specialist/Integrated Care Specialist provides clinical recommendations and collaborative support to team members and physical health care staff involved in the Integrated Care Plan (ICP) Program. The Clinical Care Management Specialist reviews clinical information using a holistic care framework, makes recommendations for appropriate levels of care, creates Integrated Care Plans and coordinates services for individuals referred to the ICP team.
Essential Functions :
- CCMS will travel amongst the identified community partnership sites and participate in case information sharing as scheduled.
- CCMS will develop a schedule with each practice site, PH MCO and CBH team staff for onsite visits to consent members for participation in the Integrated Care Plan Program, share released information with the PH MCO and discuss case recommendations with the appropriate internal staff.
- CCMS will complete Integrated Care Plans as outlined by the policies and regulations of CBH and the state.
- CCMS will create Integrated Care Plans for members referred from community partners, internal referrals for ICP development and for members who meet appropriate criteria for ICP planning.
- During the case collaborations with sites and PH MCO's, CCMS will provide behavioral health treatment utilization and develop care plans tailored to the individual's current needs as part of the treatment team.
- CCMS will coordinate with internal CBH staff to ensure communication and collaboration is clear and the individual's developed care plan is carried out as such.
- CCMS will participate in the problem solving process and provide information on additional community resources as needed.
- CCMS will do utilization review as necessary for assigned members, implementing medical necessity criteria to inform level of care determinations.
- CCMS will document and track their interventions and share data internally and with associated partner's, like PH MCO's and community partnership sites.
- Education: Master's degree in Social Work or related field
- Clinical Licensure: Qualified candidates are required to obtain license within the first 18 months (1.5 years) of employment; PA Child Abuse Clearance required

The Clinical Care Manager (CCM) Specialist- Children's Complex Care (High-Fidelity Wraparound) is part of a specialized team, responsible for providing high intensity care management, that works collaboratively and directly with members and providers to ensure effective, holistic, member-centric care. As a member of CBH's Complex Care team, training and certification in High-Fidelity Wraparound, an evidence-based care management intervention, will be provided.
The CCM Specialist will also be responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, and treatment quality monitoring. As a member of the Children's Complex Care team, you will have an opportunity to apply your understanding of behavioral health and your cultural competence to work collaboratively with CBH members, their caregivers, their supports, other professionals, and community partners to screen/assess, and develop individualized, family-anchored, comprehensive, and strength-based plans of care. The CCM Specialist must utilize clinical knowledge of best practices and evidence-based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, CCM Specialists consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized.
As a High-Fidelity Wraparound (HFW) Facilitator, the CCM Specialist assists members and their families in learning to accomplish goals they have identified by exploring the strengths and culture around each need they have. The CCM Specialist facilitates monthly interagency planning meetings to assist the member and their family in planning around and working towards meeting their needs. The CCM Specialist will transfer their Wraparound skills to the member and their family, by teaching them to think outside of the box and use natural and community supports.
Essential Functions :
- Work directly with members, natural supports and providers in the community, including in members' homes.
- Facilitate care coordination. Using an inter-generational approach, with system partners to address social determinants, barriers to wellness, and other needs of members, including connection to appropriate community - based services.
- Create and maintain a team of system partners and natural supports for each member and their family.
- Complete an effective Crisis Prevention Plan in collaboration with each assigned member and their team
- Be knowledgeable about behavioral health care services, system partners, service continuum and established medical necessity criteria and apply that knowledge in all work activities.
- Attend up to 3 hours of group or individual skills-based coaching/supervision per week
- Complete Utilization Management activities including but not limited to: interagency meetings, social determinants interviews with members, treatment team meetings, case consultations, care coordination, provider meetings, reviews, treatment linkage, within established timeframes.
- Authorize treatment when Medical Necessity Criteria is met.
- Consult with Physician Advisor, Psychologist Advisor and/or Clinical Leadership as clinically appropriate.
- Generate denial letters based on physician reviews and in accordance with state regulations as needed.
- Demonstrate an understanding of Evidence-Based Practices (EBPs) and connect members to appropriate evidence-based treatment.
- Collaborate with other CBH departments to ensure quality of care, promote placement of members in the most appropriate services and to facilitate consistent longitudinal care management.
- Document provider quality concerns in accordance with CBH policy/procedure.
- Use data to inform clinical decision-making.
- Maintain documentation in CBH's electronic system consistent with agency and industry standards.
- Respond to care coordination correspondence within 24-48 hours or sooner if necessary.
- Attend all meetings as assigned and actively prepare for and participate in supervision and clinical case consultations.
- Ensure the highest quality of customer service when interacting with members, providers, and other stakeholders.
- Display commitment to member/family-driven care and principles of resiliency and recovery.

- Facilitate monthly interagency HFW Planning Meetings (includes youth/family participation) for a caseload of approximately 10 members and their families.
- Write, revise and distribute individualized Wraparound Plans and Discovery documents to member's teams monthly.
- Ensure that the principles and steps of the High-Fidelity Wraparound process are delivered with the highest possible fidelity.
- Provide evidence-based intensive care management interventions directly to members in the community.
- Maintain a care management caseload of approximately 10 members (or as otherwise directed).
- Facilitate care coordination with external entities including behavioral and physical health providers, Child Welfare, Juvenile Justice, School district of Philadelphia, and Targeted Case Management (TCM), as well as any other providers or systems involved with assigned members' families.
- Address member/caregiver engagement in treatment by reporting or addressing any reported/observed quality concerns, resolving communication barriers between member (and/or caregiver) and providers, assisting member/caregiver with mediation issues/barriers, assisting with any clarification/prioritizing issues, increase positive collaboration with providers and other supports.
- Facilitate longitudinal care management of all assigned members.
- Complete UM activities as assigned and within required timeframe
- Participate in and complete training and credentialing in the High-Fidelity Wrapround model of care management.
- Maintain a flexible schedule, which may include some evenings or occasional weekends, in order to accommodate the scheduling needs of assigned members and their families

Focus on procedures, HLD locations to align with ambulatory assignment- Spearhead Disease-Specific Survey: Hip/Knee; VAD; participate in Bariatric Survey- Committee/Workgroup Participation based on expertise to include but not limited to the following Perioperative ServicesPt. Safety ritical Care nterprise IT/EPIC optimization edation nfection Control Committee ursing leadership (PRN) ajor Lower Joint (CET) urgery Center Quality, Pt Safety and IC DLH Quality, Pt. Safety and IC ode Committee/Airway adiation SafetyPerform regulatory functions for assigned areas; attend other meetings as needed
Responsibilities:
+ Supports vision of PPMC CMO and Regulatory Department Maintains a solid working knowledge of regulatory body regulations, including but not limited to, TJC, CMS, state regulationsActs as liaison with state and other accrediting bodies as neededResearches and completes surveys which describe institutional demographics, volumes of services, and other activitiesAssesses educational needs, provides information and instructional materials to services/departmentsEnsure internal clients are aware of services offered Educates staff on key policies & procedures & regulatory requirementsDevelops, documents in, and maintains regulatory related databases necessary to track compliance activities and outcomes.Coordinates and participates in regulatory survey readiness activities to include environment of care rounds and tracers Ensures that all initiatives are in compliance with accrediting, regulatory, & institutional standardsDevelop relationships within the organization to ensure effective communication & partnership / cooperation in all facets of the licensure & accreditation process promote use of tracer program.Collaborate with leaders & staff to evaluate proposed rules / regulations & assess, develop, or participate in, as appropriate, regulatory advocacy approachesCoordinate with others, as appropriate, to resolve highly complex or unusual business problems impacted by proposed or finalregulatory action or inaction affecting major functions or disciplinesDevelop effective approaches to assess regulatory readiness, collaborating with entity operations in the development of standard processes and procedures. Provide strategic consultation and coordination in the prioritization of major areas of opportunity incorporating both internal best practices as well as external regulatory developments. Convene regulatory workgroups/teams to seek feedback and maximizing transparent sharing of efforts and findings.Support clinical integration across the system by standardizing the writing, review, updating and management of regulatory related policies. Collaborate with entity operations and other departments to develop standard processes and procedures related to regulatory readiness.Facilitates identification of future regulatory-related improvement initiatives and organizes appropriate team members.Coordinates team projects and brings groups to consensus on methods necessary to improve clinical outcomesWith extensive input from and collaboration with entity regulatory directors, prepare routine reports on the state of regulatory readiness at Penn, including major risks and opportunities.Works with individual clinicians & teams to identify methods to improve compliance with clinical performance indicators required By regulatorsMaintains a working knowledge of outcome related publicly reported data and identifies areas where compliance is Problematic Monitors consistency, validity, & reliability of data. Edits as appropriatePrimary liaison with state agency, including but not limited to PA DOHServes as a liaison with departments such as Pharmacy, Clinical Effectiveness and Quality Improvement (CEQI), Case Management, Infection Control and Prevention, the Medical Staff Departments to identify & solve patient care issues with PPMC managers & staff

Education or Equivalent Experience:
+ Bachelor of Arts or Science (Required)
+ Education Specialization:Equivalent Experience:-And 5+ years Clinical experience in a healthcare setting is preferred. Minimum 3-5 years of experience in healthcare management or healthcare education required. Must be highly organized with the ability to work well with people. Must have ability to work with highly sensitive & confidential information.
