Work Shifts

Duties & Responsibilities

The Patient Care Associate, under the direction of a licensed nurse or other medical staff, in the health care setting assists the professional staff by performing, veni-puncture, finger-sticks, HIV Pre and Post Test Counseling, Rapid HIV Testing, responding and assisting with emergencies, facilitating telehealth encounters, various clerical and health-related duties associated with patient care (ADLs as indicated) and contributing to the safety, care and comfort of patients. The duties are assigned by and are performed under the supervision of a Registered Nurse. The Patient Care Associate may be required to perform the duties listed below and other duties involving same type, area of responsibility. Patient care associates also observe their patient's mental, emotional and physical conditions and report any changes to the nursing staff.

Responsibilities:
1. Facilitate Telehealth encounters as needed
2. Administer total care (bed bath, partial or complete)
3. Assist with patient's personal hygiene - including skin care, mouth care,

shaving, etc.
4. Transfers patients - in and out of bed, stretcher and wheel chair. Including

physical and mechanical lifting
5. Positions patients and carry out range of motion exercise
6. Assists with ambulating and all ADL as directed
7. Prepares patients for meals and assist with feeding as needed
8. Assists with unit admission and transfer of patient
9. Makes beds and tidies patient area for bed ridden patients
10. Takes and document in the electronic medical records all patient's vital signs -

B/P, TPR, weights, intake and output and performs EKGs as required and follow

outlined procedures for submitting abnormal EKGs
11. Administers hot and cold applications to patients, under supervision
12. Maintains intake and output for oral and body excretions
13. Notify MD/Nurse Staff of any changes in patient's status
14. Performs urine testing by dipstick and fractionalization
15. Chaperon staff in all clinical areas when requested
16. Maintains competency in nursing skills as detailed in the clinical

Performance checklist for Nursing Assistant
17. Assist with Nursing Follow-ups and transmit necessary information to

laboratory electronically.
18. Anticipates and acts to maintain a safe environment
19. Performs job functions in a manner that promotes efficient use of resources
20. Must wait for relief at change of tour by staff with same title (relieving PCA)
21. Document all encounters in Electronic Medical Records
22. Complete Encounter forms every shift
23. Performs dry sterile dressing changes and assists with other patient

Treatment

Activities

Maintenance of Unit:

2. Stock and replenish supplies in all clinic areas, including stationery supplies
3. Maintain cleanliness of surface area of treatment and exam rooms
4. Maintain security precautions with keys, locked areas, and equipment

Human Relations Skills:
1. Assists other employees where needed.
2. Is responsible and cooperative with supervisors, fellow associates, patients,

visitors, and Department of Correction personnel.
3. Maintains friendly working atmosphere.
4. Maintains professional attitude.
5. Maintains professional appearance and adheres to uniform policy of the

Department of Nursing.

Working Conditions:

Because patents require round the clock care, working hours include days, nights

weekends and holidays. PCAS are required to stand for long periods of time.

Employees are encouraged to safely move patents and use alternative methods

whenever possible; alternative methods include requesting assistance to roll/lift/

transfer patients. PCAs may face hazards from exposure to chemical and infectious

diseases. In addition, they have to treat patients that are confused, irrational, agitated

and/or uncooperative

Physical Demands
1. Ability to lift fifty (50) pounds. Moving, lifting or transferring of patients may

involve lifting of up to 100 pounds
2. Ability to stand for extended periods
3. Fine motor skills
4. Visual acuity

Minimum Qualifications

Qualification Requirements:
1. High school diploma or its educational equivalent, approved by a State's Department of Education or a recognized accrediting organization; and
2. Successful completion of training courses including the performance of phlebotomy and
electrocardiography (EKG), or completion of a training program or programs that include the performance of phlebotomy and EKG, such as Phlebotomy Technician and EKG Technician training programs, a Patient Care training program, or a Medical Assistant training program; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Special Note:
In addition to meeting the qualifications above, assignment to a long-term care facility requires New York State Nurse's Aide certification.

Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support.

Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient's clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management.

The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. *To help support continuity of operations and care, stff selected to work in the OTxHU may also be required to work in CHS locations within the jails. Duties & Responsibilities

Position Overview

POSITION SUMMARY

Coordinate the scheduling of all medical appointments and follow up. Ensure that all appointments are kept or rescheduled. 4. Follow up on all referrals and recommendations from the health care professionals. 5. Ensure that all person supported records meet DIDD requirements. 6. Review and sign off on all medication variances weekly. 7. Monitor and maintain medication refill request and order supplies as needed. 8. Coordinate AT equipment. Bed, wheelchairs, hoyer lift repairs and ensure AT clinic packet is completed. 9. Filing of all medical documents and encounters in chart daily 10. Ensure that all consents are updated and uploaded to the person supported file 11. Review, Complete and upload all encounters in EHR 12. Thinning charts of all persons supported and storing them according to office procedure 13. Monitor Lab tracking, request copies for file, insure that all ordered labs are completed. 14. Track all annual physicals, dentals and vision exams and ensure they are completed within the DIDD guidelines. 15. Work in the homes at least 16 hours a month or as needed. 16. Be available and on call when the Case Manager is on Vacation or unavailable. 17. Coordinate (as needed) between and among health care professionals, such as Primary Care Physician, CSN Case Mangers, and medical specialist, i.e. (cardiologist, orthopedists, psychiatrist, etc.) to assure the effective flow of information. 18. Performance quality improvement (PQI) duties and reports as assigned by supervision & PQI committee. 19. Any other job duties as assigned by supervision. JOB SUMMARY AND QUALIFICATIONS:
To assist service recipients with health related activities that the service recipient would self- manage if able. Under the direction and supervision of the Sr. Director. The LPN HSC will provide clerical and office support in maintaining health care services and insuring that all documentations and medical supply needs of the persons supported are provided. The LPN HSC will work closely with the Contracted Oversight RN.

WHAT YOU SHOULD HAVE FOR THIS ROLE:
The position requires current license as a Licensed Practical Nurse in the State of Kentucky and at least two years of nursing experience preferably working with persons with intellectual and developmentally disabilities or in a home health environment. The wage for this position varies based on qualifications and experience. Candidates without a Registered Nurse License through the Minnesota Department of Nursing will earn between $20.98 and $0.27 per hour. Candidates with a Registered Nurse License through the Minnesota Board of Nursing will earn between 28.78 and 33.69 per hour. New hires will typically start at the entry-level rate within the applicable wage range, while candidates with relevant experience may be eligible for a starting rate of up to 25.60 per hour (non-RN) or 32.67 per hour (RN). Wage decisions will be made on a case-by-case basis, considering the individual's experience and the school district's budget.
