POSITION SUMMARY:
We are seeking a full-time therapist to join our Functional Family Therapy (FFT) team. As a member of our FFT team, you will receive specialized training in this evidence-based model focused on collaborative, integrated in-home treatment services for families with children or adolescents at risk of or transitioning from out-of-home placement.

RESPONSIBILITIES INCLUDE:

  1. Accepting cases assigned for home or community-based FFT services.
  2. Providing, at a minimum, weekly one-hour home-based family therapy sessions.
  3. Utilizing the FFT model to work with families who have adolescents aged 10 to 18.
  4. Maintaining a caseload of clients as determined by the model or supervisor, offering services for three to six months.
  5. Engaging in a variety of services, including but not limited to:
    1. Conducting evaluations and assessments
    2. Participating in the development of clients' Individual Treatment Plans (ITP)
    3. Providing crisis intervention and support
    4. Educating and consulting with families and significant others regarding clients' mental health
    5. Facilitating family interventions and monitoring symptoms
    6. Coordinating necessary evaluations and service delivery as per ITP
    7. Following up to ensure services meet client needs
    8. Connecting families with community resources
  6. Working a flexible schedule that includes evening and weekend on-call coverage on a rotating basis.
  7. Maintaining current training, certifications, and licensure according to applicable regulations.
  8. Contributing to clinical documentation through timely completion in compliance with standards.

OTHER DUTIES:

  1. Attending staff meetings, consultations, individual supervision, and ongoing training.
  2. Coordinating with other child-serving organizations based on the specific needs of families.
  3. Upholding client privacy and confidentiality in all treatment-related matters.
  4. Maintaining high ethical and professional standards and adhering to agency policies.
  5. Performing additional responsibilities as assigned by management.

QUALIFICATIONS:

  • A Master's Degree and valid Ohio License in Marriage and Family Therapy, Social Work, or Counseling (LSW, LPC, MFT, LISW, LPCC, IMFT) is required.
  • Experience with systemic family therapy or evidence-based models is preferred. Flexible work hours with some evenings and weekends required.
  • Familiarity with the complex needs of clients with co-occurring mental health and substance abuse disorders is essential, as is competency in Family Therapy, Individual Therapy, Case Management, and a Multidisciplinary Team approach.

Our Portage County office is looking for a full time Therapist to join our Functional Family Therapy (FFT) team. SCOPE OF ROLE

Reporting to the Supervisor, Behavioral Health Services, the Counselor, Community-Based Family Therapy will provide therapeutic services to high-need children and adults experiencing serious emotional challenges, primarily within the home environment, as well as other clinically appropriate settings. Individuals served are multi-system involved, at risk of being psychiatrically hospitalized and/or placed in a residential treatment facility or transitioning back to their home environment from residential placement. Services may be delivered in individual or family sessions, depending on client needs. The Counselor will utilize a team-based, treatment plan-driven approach to deliver high-quality, client-centered care.

HOW YOU'LL SUCCEED:
  • Provide therapeutic intervention services to children, adolescents, and adults using individual, family, and group treatment.
  • Provide crisis stabilization services to families as is needed.
  • Identify strengths and needs of families to promote treatment.
  • Act as a family advocate; work with social service agencies to provide and coordinate services.
  • Develop and implement Care Plans, Comprehensive Assessments, Treatment Summaries, Safety Plans and Risk Assessments.
  • Monitor families' participation in programs through coordination of agencies' reports and continue direct contact as needed.
  • Participate in regular treatment team meetings with external stakeholders to assist in the coordination of care across multiple service providers.
  • Maintain client charting in accordance with Medicaid, OhioMHAS, CARF and professional standards.

Requirements

KNOWLEDGE & EXPERIENCE:
  • Bachelor's Degree required.

SCOPE OF ROLE

Reporting to the Supervisor, Behavioral Health Services, the Counselor, Community-Based Family Therapy will provide therapeutic services to high-need children and adults experiencing serious emotional challenges, primarily within the home environment, as well as other clinically appropriate settings. Individuals served are multi-system involved, at risk of being psychiatrically hospitalized and/or placed in a residential treatment facility or transitioning back to their home environment from residential placement. Services may be delivered in individual or family sessions, depending on client needs. The Counselor will utilize a team-based, treatment plan-driven approach to deliver high-quality, client-centered care.

HOW YOU'LL SUCCEED:

  • Provide therapeutic intervention services to children, adolescents, and adults using individual, family, and group treatment.
  • Provide crisis stabilization services to families as is needed.
  • Identify strengths and needs of families to promote treatment.
  • Act as a family advocate; work with social service agencies to provide and coordinate services.
  • Develop and implement Care Plans, Comprehensive Assessments, Treatment Summaries, Safety Plans and Risk Assessments.
  • Monitor families' participation in programs through coordination of agencies' reports and continue direct contact as needed.
  • Participate in regular treatment team meetings with external stakeholders to assist in the coordination of care across multiple service providers.
  • Maintain client charting in accordance with Medicaid, OhioMHAS, CARF and professional standards.
  • Demonstrate the ability to work efficiently, meeting workpace expectations (i.e.: productivity and complete all required documentation).
  • Prepare all documentation for discharge and aftercare referral.
  • Acts to understand and safeguard the welfare of others.
  • Actively consults with supervisor to act upon ethical and legal aspects of the organization.
  • May be required to perform telehealth services as determined by Signature Health.
  • Actively engage in supervision with your BH Supervisor.
  • Other duties as assigned.

Requirements

KNOWLEDGE & EXPERIENCE:

  • Bachelor's Degree required.
  • Valid unencumbered LSW, LISW, LPC, LPCC, LIMFT or LMFT License in the state of Ohio is required.
  • Previous experience working with children and youth.
  • American Heart Association (AHA) Basic Life Support (BLS) certification required.
  • Competency in the treatment of mental health and/or substance use disorders within the context of individual, group, couples and family systems therapy.
  • Experience collaborating with other team members to optimize outcomes clients.
  • Familiarity of state and federal healthcare regulations.
  • Awareness of community and state support resources for population served.

WORKING CONDITIONS:

  • Work is normally performed in client or family home environment but may be performed in other settings as clinically necessary.
  • While hours of operation are generally during standard business hours, evening and extended hours will be required to accommodate client needs
  • Requires periods of sitting, standing, telephone, and computer work.
  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
