- Develop individualized patient care plans and continuously monitor their effectiveness.
- Provide leadership and guidance to nursing staff, fostering an environment of continuous improvement and professional development.
- Ensure accurate and timely documentation of patient care activities.
- Act as a liaison between patients, families, and healthcare providers to facilitate communication and address any concerns.
- Graduated from an approved school of professional nursing.
- One year experience as a registered nurse, preferred.
- Home Health leadership experience required.
- Proven experience in a home health setting, with a strong understanding of regulatory and compliance requirements.
- Excellent leadership and communication skills.
- Strong organizational skills and the ability to manage multiple priorities effectively.
