The Sr. Health Promotion Specialist is responsible for driving education, participation and sustained engagement across the client population in the well-being improvement program. S/he works closely with the Sharecare Account Manager and the client's Plan Administration team to develop monthly/quarterly challenges and will engage Plan participants and their families for the purpose of inspiring sustained program participation and behavior change. S/he will also work closely with the Sharecare Engagement Marketing team to develop and implement engagement materials.
**Essential Job Functions:**
+ Understanding plan participant needs, contract obligations, and developing a program plan that includes the scheduling of programming, well-being improvement activities and events that support the areas of greatest well-being need.
+ Partner and interact with client Plan Administrators, other onsite personnel, and external vendor partners across the client network to ensure customer satisfaction and provide additional client reporting and education.
+ Work with field sites' wellness champions to continue growth of the program.
+ Provide ongoing communication, education and training regarding wellness programs, health risks and participant engagement.
+ Develop relationships with OCC health nurses to understand health risks specific to assigned worksite
+ Collaborate with other site-specific Health Promotion Specialists and Engagement Marketing lead to optimize outreach based on data-driven learnings and ensure cohesive overarching strategy.
+ Collect, evaluate, and report appropriate program data to assist with evaluation of program effectiveness.
+ Work direction will be provided primarily by the site contact responsible for the assigned territory.
**Specific Skills/ Attributes:**
+ Excellent communication/presentation and leadership skills (verbal and written).
+ Strong program planning and marketing skills.
**Qualifications:**
+ Degree in related health field, Master's degree preferred.
+ Certified Health Education Specialist (CHES) or other health-related professional certification is a plus.
+ 5 years of experience in health promotions services, supporting business clients.
+ Strong understanding of health behavior change theory.
+ Excellent ability to make connections between certain aspects of the program and client culture and leverage connections to achieve desired results.
+ Must live in Jaffrey, New Hampshire or within commuting distance, with flexibility to work onsite at different times to accommodate all shifts.

**Job Specific Responsibilities and Tasks: Duties may include but are not limited to** :
**Core Duties:**
- Provide health promotion and education opportunities including, but not limited to, tobacco awareness; tobacco cessation classes or appointments consistent with the MTF tobacco cessation program; and nutrition education and weight management.
- Develop population-based programs targeting high-risk groups and individuals, implementing programs through group and individual counseling and education.
- Plan and implement health education programs as specified in the TO.
- Perform limited administrative duties, which include maintaining statistical records of workload and data entry and analysis of patient outcomes.
- Maintain educational materials to support health promotion and education.
**Additional Duties:**
- Directly support MHP efforts in primary, secondary, and tertiary prevention to appropriate beneficiaries through marketing, NHCP/NMRTC Camp Pendleton PHP created HPN, curriculum development and streamlined processes.
- Develop and execute mass and individual health promotion communications in a hospital setting, train hospital and Naval staff in health promotion program protocols and processes to ensure sustainability, develop educational seminars and events; implement health programs, internet-based tools and mobile applications. This will be done through the creation of biweekly editorial calendars, brochures, Plan of the Week, posters, flyers and any other educational and mass health communication products. PHP will also facilitate the third year Family Medicine Resident mandatory Population Health curriculum, which will include training in regards to vulnerable populations.
- Collaborate with the Command Marketing Team to execute a comprehensive NHCP/NMRTC Camp Pendleton PHP social media strategy using best practices aligned with the overall Navy/DHA mission and vision that result in increased beneficiary engagement with PHP and initiatives. This strategy should include editorial calendar usage, social media posting recommendations, and emergency response protocols. Social media metrics will be tracked with an analytics report that will be delivered on the 1st of each month to HPW Public Health Educators.
- Collaborate with the Command Marketing Team to train, create, and deliver marketing, communications, and social media strategies effective for each audience in order to market and strengthen the awareness of available electronic resources.
- Collaborate with PHP Project Manager/Senior Strategist to construct an annual strategic plan to target DON mandated and non-mandated programs and revise as needed to meet the HPW Department program needs. This annual plan will include a comprehensive needs analysis, a Strengths Weaknesses Opportunities and Threats analysis, a plan of action and milestones, monthly health observances defined by the Defense Center for Public Health, Portsmouth, the HPW committee, Med/IG requirements, HPW events and classes, the NHCP/NMRTC Camp Pendleton PHP-created HPN, and will be followed up through reporting and meeting on a quarterly basis.
- Collaborate with NHCP/MHP/DPH/NMRTC Camp Pendleton Marketing team, and partner with HPW and Public Health as needed to target audiences for communication, promotion, social media and training efforts to include both internal and external audiences such as: command personnel (leadership, clinical staff, and non-clinical staff); Navy medicine, DHA, and military health system leadership. Monitor, manage, and contribute daily to all command social media platforms (Facebook, Instagram, Twitter, and Pinterest).
- Collaborate with HPW, DHB, or the Navy and Marine Corps Force Health Protection Command staff to create health communication materials for beneficiaries that specifically address criteria for cancer prevention, diabetes prevention, sexually transmitted infections detection/prevention, obesity, tobacco use/cessation, metabolic syndrome, and other conditions targeted by the command.
- Identify barriers, vulnerabilities, and other challenges facing NHCP/NMRTC Camp Pendleton Military Health System beneficiaries in being able to achieve optimal health/wellness by conducting needs assessments within the NMRTC Camp Pendleton community. For example, gathering information through the commissary tours, and HRA 2.2 surveys.
- Collaborate with NHCP/NMRTC Camp Pendleton subject matter experts to incorporate emerging best practices from industry and public health experts to implement appropriate health and wellness programs with NHCP/NMRTC Camp Pendleton Military Health System beneficiaries.
- Provide guidance and support to MHP staff members, HPW Department staff, Mental Health staff, and other personnel responsible for delivering health interventions to NHCP/NMRTC Camp Pendleton Military Health System beneficiaries. Including, but not limited to, all command specific HEDIS metrics.
- Track metrics of success as informed by command for primary, secondary, and tertiary efforts such as breast cancer, chlamydia, cervical cancer, and colorectal cancer screenings as well as the improvement of well-child checks and command no-shows through mass health communication/education products i.e., brochures, posters, flyers, and social media.
- Collaborate with PHP Project Manager/Senior Strategist to periodically present activities and outcomes to command Executive Steering Committee, participating personnel and departments, community partners, and targeted population as appropriate.
- Expand targeted populations to include outlying clinics as identified interventions demonstrate success and sustainability. This is done through educational material distribution including but not limited to brochures, flyers, posters, videos, and magazines. Additionally, including all command specific events, which will incorporate all five main branch health clinics for NHCP/NMRTC Camp Pendleton.
- Collaborate with HPW and the Director of Public Health to design community-based intervention strategies for NHCP/NMRTC Camp Pendleton beneficiaries, utilizing best practices of cultural competency.
- NHCP/NMRTC Camp Pendleton PHP will create and maintain the HPN through quarterly meetings/interventions with base wide/community wide partnerships.
- Ensure timely update of HPW content on the intranet and social media platforms as needed.
- Assist the PHP Project Manager/Senior Strategist in the review, development, and implementation of policies, procedures, processes, and guidelines.
- Coordinate special projects such as health promotion specific education-focused workshops, health fairs, informative displays and programs for special groups.

**Minimum Qualifications** :
**- Degree/** **Education:** Masters level education in one or more of the following areas preferred: Health Promotion, Health Policy, Public Health, Community Health, or nursing degree with Masters level preparation in nursing administration or nursing education.

This includes actively participating in divisional meetings, sharing insights, and contributing to collaborative decision-making processes. Population Level Outreach - Develop, implement and assess data-driven and evidence-informed, population-level outreach throughout campus aimed at preventing suicide and associated risk factors and promote positive mental health. Responsibilities include but not limited to:

Coordinating and conducting the QPR bystander intervention training program.

Hosting engaging large-scale events that help students develop an understanding of the signs of suicide and how to help a friend, tools and skills to identify and address distress, develop coping skills for college and life stressors and the overall connection between mental health and other aspects of wellness.

Working with CPO team members to create and implement social norming campaigns aimed at reducing stigma, suicide and promoting positive mental health skills.

Conduct workshops, tabling, and other events to educate students on topics like anxiety, coping skills, depression, healthy screen use, suicide prevention, bystander intervention/helping a friend with the goal of increasing knowledge and changing attitudes and behaviors.

Review current and upcoming suicide prevention or mental health promotion legislation and work with senior leadership on modifying prevention and outreach efforts to ensure compliance.

Conduct annual review of mental health outreach and education efforts and work with leadership to identify gaps and problem solve solutions.

Create and implement skill-based workshop series to address mental health concerns and provide practical skills for students to use to decrease risk.

Coordinate the department's pet assisted therapy program, PALS, in collaboration with on and off-campus partners.

Coordinate awareness month-based programming including but not limited to Suicide Prevention Month and Mental Health Awareness Month.

Individual/Group Level Outreach - using a BASICS / SBIRT based approach, provide individual outreach and psychoeducation to students in need, including but not limited to:

Coordinate and conduct mental health focused screenings to identify high-risk students and refer them to resources, including but not limited to training student assistants, peer educators and professional staff on implementing screening tools and providing tailored interventions.

Coordinate mental health focused support spaces including but not limited to Global CommuniTea (a peer led, student support space focusing on the mental health needs of global students).
-
Coordinate the department's mindfulness program including training peer educators and overseeing student assistants that implement peer led mindfulness exercises for the campus community.
-
Conduct mandated alcohol and other drug psychoeducational interventions with students including one-on-one and group settings when needed.

Peer Education & Supervision - using a student-centered approach, provide supervision and training to peer education interns and student assistants within CPO. This includes but is not limited to:

Overseeing a cohort(s) of healthy screen use, mental health promotion and suicide prevention focused on interns, including organizing their outreach schedule, hosting weekly team meetings and providing training on best practices around health education and outreach in a college setting.

Supervise student assistant(s) with a role focused on mental health promotion, resource education, mindfulness, suicide prevention or associated topics.

Collaboration and Partnerships - this position requires continuous collaboration with on and off campus patterns to ensure we make a large and effective impact. This includes but is not limited to:

Extensive collaboration with the members of the CPO team to ensure departmental goals and initiatives are met including conducting non-mental health/suicide prevention focused bystander trainings, workshops and events. Collaborations will also focus on weaving mental health and other topics to educate students on the connections between the different dimensions of wellness.

Collaboration with partners within Student Health, Wellness and Prevention Services to empower students to take care of all aspects of their health and wellness.

Collaborate with Counseling and Psychological Services (CAPS) to understand the mental health treatment practices and mental health trends on campus and align outreach and education accordingly.

Working with community partners to bring experts and external resources to campus to grow the health and wellness safety net for students.

This position is based on Stony Brook University's main (West) campus but will require occasional work/travel to the East (hospital) and Southampton campuses as the position serves all Stony Brook students. Travel to regional, state or national professional development opportunities and/or training may also be required at times.

This role encompasses population-level and individual-level outreach, peer education and supervision, collaboration with campus and community partners, and professional development.
The incumbent plays a crucial role as a team member within the overall campus health and wellness departments. In this capacity, they collaborate with colleagues across various units to enhance the effectiveness and integration of health, wellness, and prevention services. This includes actively participating in divisional meetings, sharing insights, and contributing to collaborative decision-making processes. The incumbent fosters a cooperative and supportive environment, ensuring seamless teamwork and alignment with the shared goals and objectives of campus health and wellness initiatives. This position may be required to provide support in other areas of student health, wellness, and prevention services, as needed. - Population Level Outreach - Develop, implement and assess data-driven and evidence-informed, population-level outreach throughout campus aimed at preventing suicide and associated risk factors and promote positive mental health. Responsibilities include but not limited to:
- Coordinating and conducting the QPR bystander intervention training program.
- Hosting engaging large-scale events that help students develop an understanding of the signs of suicide and how to help a friend, tools and skills to identify and address distress, develop coping skills for college and life stressors and the overall connection between mental health and other aspects of wellness.
- Working with CPO team members to create and implement social norming campaigns aimed at reducing stigma, suicide and promoting positive mental health skills.
- Conduct workshops, tabling, and other events to educate students on topics like anxiety, coping skills, depression, healthy screen use, suicide prevention, bystander intervention/helping a friend with the goal of increasing knowledge and changing attitudes and behaviors.
- Review current and upcoming suicide prevention or mental health promotion legislation and work with senior leadership on modifying prevention and outreach efforts to ensure compliance.
- Conduct annual review of mental health outreach and education efforts and work with leadership to identify gaps and problem solve solutions.
- Create and implement skill-based workshop series to address mental health concerns and provide practical skills for students to use to decrease risk.
- Coordinate the department's pet assisted therapy program, PALS, in collaboration with on and off-campus partners.
- Coordinate awareness month-based programming including but not limited to Suicide Prevention Month and Mental Health Awareness Month.
- Individual/Group Level Outreach - using a BASICS / SBIRT based approach, provide individual outreach and psychoeducation to students in need, including but not limited to:
- Coordinate and conduct mental health focused screenings to identify high-risk students and refer them to resources, including but not limited to training student assistants, peer educators and professional staff on implementing screening tools and providing tailored interventions.
- Coordinate mental health focused support spaces including but not limited to Global CommuniTea (a peer led, student support space focusing on the mental health needs of global students).
- Coordinate the department's mindfulness program including training peer educators and overseeing student assistants that implement peer led mindfulness exercises for the campus community.
- Conduct mandated alcohol and other drug psychoeducational interventions with students including one-on-one and group settings when needed.
- Peer Education & Supervision - using a student-centered approach, provide supervision and training to peer education interns and student assistants within CPO. This includes but is not limited to:
- Overseeing a cohort(s) of healthy screen use, mental health promotion and suicide prevention focused on interns, including organizing their outreach schedule, hosting weekly team meetings and providing training on best practices around health education and outreach in a college setting.
- Supervise student assistant(s) with a role focused on mental health promotion, resource education, mindfulness, suicide prevention or associated topics.
- Collaboration and Partnerships - this position requires continuous collaboration with on and off campus patterns to ensure we make a large and effective impact. This includes but is not limited to:
- Extensive collaboration with the members of the CPO team to ensure departmental goals and initiatives are met including conducting non-mental health/suicide prevention focused bystander trainings, workshops and events. Collaborations will also focus on weaving mental health and other topics to educate students on the connections between the different dimensions of wellness.
- Collaboration with partners within Student Health, Wellness and Prevention Services to empower students to take care of all aspects of their health and wellness.
- Collaborate with Counseling and Psychological Services (CAPS) to understand the mental health treatment practices and mental health trends on campus and align outreach and education accordingly.
- Working with community partners to bring experts and external resources to campus to grow the health and wellness safety net for students.
- This position is based on Stony Brook University's main (West) campus but will require occasional work/travel to the East (hospital) and Southampton campuses as the position serves all Stony Brook students. Travel to regional, state or national professional development opportunities and/or training may also be required at times.
- Professional Development and Committee Involvement
- Participates in opportunities that promote their learning, growth, and development (i.e., classes, training, committees, webinars).
- Contributes to the Division of Student Affairs and Stony Brook University by attending and/or supporting events, activities, and programs sponsored by areas within and outside of your department/unit (i.e., campus-wide traditions, events, programs).
- Other duties or projects as assigned as appropriate to rank and departmental mission.
- Evening and weekend work will be required at times.
Special Notes:
This is a full-time appointment. FLSA Exempt position, not eligible for the overtime provisions of the FLSA. Minimum salary threshold must be met to maintain FLSA exemption.
Salary : $64,000-$7,000 + 4,000 Location Pay = 68,000- 71,000 Total Compensation
In addition to the employee's base salary, this position is eligible for 4000 UUP annual location pay, paid biweekly.

Position Summary:
The Refugee Health Promotion (RHP) Outreach Coordinator plays a vital role in improving the health and well-being of refugees and other ORR-eligible populations in New Jersey. This position is responsible for providing culturally and linguistically appropriate health education, facilitating healthcare navigation, organizing wellness groups, supporting mental health outreach, and contributing to the implementation of the Refugee Health Promotion program. Bilingual in English and Spanish, or English and Haitian Creole preferred.
The staff member will also support event planning as well as data collection and management.
Key Responsibilities:
1. Health Education & Outreach
• Plan and facilitate group and individual health education sessions for ORR-eligible populations.
• Create and distribute health outreach materials in appropriate languages and formats.
• Use validated, culturally relevant curricula and tools to increase health literacy.
• Conduct pre- and post-assessments to measure knowledge gained.
2. Medical and Mental Health Navigation
• Assist clients with navigating healthcare systems, including scheduling, attending, and understanding medical and mental health appointments.
• Develop and implement individualized Plans of Care (POC) for clients.
• Provide interpretation support when necessary or coordinate with trained interpreters.
• Help mitigate barriers to accessing healthcare, including transportation, language, and system complexity.
3. Wellness and Social Connection Activities
• Organize and facilitate community-based wellness groups (e.g., skill-building, peer support, adjustment groups).
• Engage participants in culturally relevant and trauma-informed activities to promote social inclusion and mental well-being.
• Ensure wellness activities align with community needs and preferences.
4. Event Planning & Community Engagement
• Support the planning and execution of community health events, workshops, and trainings.
• Coordinate logistics, outreach, and collaboration with community partners and refugee-serving organizations.
• Promote events through culturally appropriate channels.
5. Data Collection & Case Management
• Enroll eligible clients and maintain accurate, timely documentation in the ClientTrack database for individual services and group events.
• Maintain case notes, POCs, assessments, and closure documentation in compliance with NJOR guidelines.
• Participate in monthly data reviews and ensure reporting deadlines are met.
• Collect and store pre- and post-assessments for health education and wellness groups.
6. Collaboration & Compliance
• Work closely with the Refugee Health Program Manager, community partners, and health service providers.
• Attend required NJOR trainings, meetings, and technical assistance sessions.
• Uphold trauma-informed, client-centered, and culturally responsive practices in all services.
• Ensure services are equitable and inclusive, aligned with ORR policy and guidelines.
Qualifications:
• Bachelor's degree in human services, social work, international relations, or related field (or equivalent experience).
• Experience in outreach, community engagement, or partnership building.
• Strong interpersonal and networking skills with the ability to engage diverse communities.
• Ability to work independently, manage relationships, and follow through on commitments.
• Excellent communication, organizational, and record-keeping skills.
• Familiarity with immigrant and refugee populations and the challenges they face.
• Bilingual in English and Spanish, or English and Haitian Creole preferred.
