Definition & Duties

Under general supervision provides health education, health promotion, and outreach activities to individuals and communities to promote, maintain, and improve health. Minimum Education, Training & Experience & Special Requirements
Bachelor's degree in public health, health promotion, or closely related health field and at one (1) year experience in conducting health education public health program activities; OR any equivalent combination of education and experience which provides the required knowledge, skills and abilities.

  • Guides and evaluates their work (individually or in teams).
  • Assists with data collection and writes related descriptive narrative for local community health assessments.
  • Assists with grant applications and related reports while maintaining and evaluating program data.
  • Coordinates and/or participates in coalitions/committees on county, local and state levels as directed by grant objectives and division manager.
  • Links persons living with infectious and/or chronic disease to health care and support services and serves as a community resource for health promotion and referral.

Compensation
Employees serve a six-month probationary period and are eligible for meritorious performance compensation increases based upon performance evaluations. Across the board compensation increases are considered by the Board of Supervisors, and are made effective July 1 annually.

  • PPO with 90/10 in-network and 80/20 out-of-network co-insurance; 275 deductible single or 550 deductible family; 1000 out-of-pocket maximum single or 2000 out -of-pocket maximum family
  • Single coverage is 32/month for eligible full-time employees; family coverage is 112.00/month.
Dental Care Coverage
Eligible employees may elect dental care coverage the first of the month following one full month of employment. Single coverage is paid by the County for eligible full-time employees; family coverage is a cost between the County and employee.

Vision Care Coverage
Eligible employees may elect vision care coverage the first of the month following one full month of employment. Single coverage is paid by the County for eligible full-time employees; family coverage is a cost between the County and employee.

Flexible Spending
Eligible employees may set aside pre-tax dollars to pay for out-of-pocket health, dental and vision expenses that are not reimbursable through another source. Employees may also set aside pre-tax dollars for dependent care expenses.

Life Insurance
Eligible full-time employees are covered after 60 days of employment. Coverage is 1 times annualized compensation rounded up to the nearest thousand in group term life insurance and AD&D. Max covered salary 100,000.

*Supplemental Life Insurance
The County offers to all benefit-eligible employees additional life insurance coverage that may be purchased for employees, spouses and children at guarantee basis.

Long-Term Disability
Eligible full-time employees are covered after 60 days of employment. County provides up to 2 years of long term disability coverage at 66 2/3% of the employees wages as a result of injury, sickness or pregnancy; employees may opt to purchase additional coverage.

IPERS
Iowa Public Employee Retirement System is a mandatory defined benefit retirement plan designed to supplement both Social Security and private savings. Non law-enforcement employees contribute 6.29% and the County contributes 9.44%.

Deferred Compensation (457)
A deferred compensation plan is offered that provides tax advantages and an additional means in which to save for retirement with an easy payroll deduction. Employees may select from several companies offering deferred compensation. The County will match up to 40/biweekly, and employees are vested on day 1.

Discounted Bus Pass
Full-time and part-time employees are eligible to purchase discounted bus passes for Iowa City Transit and Coralville Transit for commuting to/from their jobs as Johnson County employees. Bus pass availability is subject to available funds and continuation of the program.

**Key Accountabilities:**
+ Plans, schedules, and coordinates on-site flu vaccine clinics and biometric screening events with the wellness champions, screening team, and vendor partners. Local travel involved.
+ Develops, distributes, and monitors all program communications within the client wellness program.
+ Establish schedule for data collection, evaluation process, and distribution to client; Maintains accurate records and ensures confidentiality of data collected.
+ Assists PM with designing and developing outcomes focused programs and interventions to increase engagement, facilitate behavior change, and reduce health risks. Contributes to the yearly strategic business plan to identify contract deliverables, program/service offerings and initiatives, targeted outcomes, and marketing strategy to meet client objectives.
+ Manages a wellness champion network in order to develop, recruit, train, engage and communicate details of the client wellness program.
+ Serves as day-to-day liaison between HealthFitness, Duke Energy, vendors partners, and internal teams to identify synergies within the programs and the services offered to participants. Collects, reviews, and submits participation data files for the wellness program as needed, ensuring accuracy and timely submission.
+ Interacts directly with client and participants; establishes and conducts client and vendor meetings and captures meeting notes to distribute to appropriate teams and individuals.

**Minimum Qualifications:**
+ Bachelor's degree in health promotion or related field.
+ OSHA compliant Adult First Aid/CPR/AED from American Red Cross, American Heart Association, National Safety Council, or American Safety & Health Institute.
+ Two or more years of experience in delivery and coordination of corporate-based health promotion programs, or equivalent combination of education and experience.

Required Qualifications

Education:

**Job Specific Responsibilities and Tasks: Duties may include but are not limited to** :
**Core Duties:**
- Provide health promotion and education opportunities including, but not limited to, tobacco awareness; tobacco cessation classes or appointments consistent with the MTF tobacco cessation program; and nutrition education and weight management.
- Develop population-based programs targeting high-risk groups and individuals, implementing programs through group and individual counseling and education.
- Plan and implement health education programs as specified in the TO.
- Perform limited administrative duties, which include maintaining statistical records of workload and data entry and analysis of patient outcomes.
- Maintain educational materials to support health promotion and education.
**Additional Duties:**
- Directly support MHP efforts in primary, secondary, and tertiary prevention to appropriate beneficiaries through marketing, NHCP/NMRTC Camp Pendleton PHP created HPN, curriculum development and streamlined processes.
- Develop and execute mass and individual health promotion communications in a hospital setting, train hospital and Naval staff in health promotion program protocols and processes to ensure sustainability, develop educational seminars and events; implement health programs, internet-based tools and mobile applications. This will be done through the creation of biweekly editorial calendars, brochures, Plan of the Week, posters, flyers and any other educational and mass health communication products. PHP will also facilitate the third year Family Medicine Resident mandatory Population Health curriculum, which will include training in regards to vulnerable populations.
- Collaborate with the Command Marketing Team to execute a comprehensive NHCP/NMRTC Camp Pendleton PHP social media strategy using best practices aligned with the overall Navy/DHA mission and vision that result in increased beneficiary engagement with PHP and initiatives. This strategy should include editorial calendar usage, social media posting recommendations, and emergency response protocols. Social media metrics will be tracked with an analytics report that will be delivered on the 1st of each month to HPW Public Health Educators.
- Collaborate with the Command Marketing Team to train, create, and deliver marketing, communications, and social media strategies effective for each audience in order to market and strengthen the awareness of available electronic resources.
- Collaborate with PHP Project Manager/Senior Strategist to construct an annual strategic plan to target DON mandated and non-mandated programs and revise as needed to meet the HPW Department program needs. This annual plan will include a comprehensive needs analysis, a Strengths Weaknesses Opportunities and Threats analysis, a plan of action and milestones, monthly health observances defined by the Defense Center for Public Health, Portsmouth, the HPW committee, Med/IG requirements, HPW events and classes, the NHCP/NMRTC Camp Pendleton PHP-created HPN, and will be followed up through reporting and meeting on a quarterly basis.
- Collaborate with NHCP/MHP/DPH/NMRTC Camp Pendleton Marketing team, and partner with HPW and Public Health as needed to target audiences for communication, promotion, social media and training efforts to include both internal and external audiences such as: command personnel (leadership, clinical staff, and non-clinical staff); Navy medicine, DHA, and military health system leadership. Monitor, manage, and contribute daily to all command social media platforms (Facebook, Instagram, Twitter, and Pinterest).
- Collaborate with HPW, DHB, or the Navy and Marine Corps Force Health Protection Command staff to create health communication materials for beneficiaries that specifically address criteria for cancer prevention, diabetes prevention, sexually transmitted infections detection/prevention, obesity, tobacco use/cessation, metabolic syndrome, and other conditions targeted by the command.
- Identify barriers, vulnerabilities, and other challenges facing NHCP/NMRTC Camp Pendleton Military Health System beneficiaries in being able to achieve optimal health/wellness by conducting needs assessments within the NMRTC Camp Pendleton community. For example, gathering information through the commissary tours, and HRA 2.2 surveys.
- Collaborate with NHCP/NMRTC Camp Pendleton subject matter experts to incorporate emerging best practices from industry and public health experts to implement appropriate health and wellness programs with NHCP/NMRTC Camp Pendleton Military Health System beneficiaries.
- Provide guidance and support to MHP staff members, HPW Department staff, Mental Health staff, and other personnel responsible for delivering health interventions to NHCP/NMRTC Camp Pendleton Military Health System beneficiaries. Including, but not limited to, all command specific HEDIS metrics.
- Track metrics of success as informed by command for primary, secondary, and tertiary efforts such as breast cancer, chlamydia, cervical cancer, and colorectal cancer screenings as well as the improvement of well-child checks and command no-shows through mass health communication/education products i.e., brochures, posters, flyers, and social media.
- Collaborate with PHP Project Manager/Senior Strategist to periodically present activities and outcomes to command Executive Steering Committee, participating personnel and departments, community partners, and targeted population as appropriate.
- Expand targeted populations to include outlying clinics as identified interventions demonstrate success and sustainability. This is done through educational material distribution including but not limited to brochures, flyers, posters, videos, and magazines. Additionally, including all command specific events, which will incorporate all five main branch health clinics for NHCP/NMRTC Camp Pendleton.
- Collaborate with HPW and the Director of Public Health to design community-based intervention strategies for NHCP/NMRTC Camp Pendleton beneficiaries, utilizing best practices of cultural competency.
- NHCP/NMRTC Camp Pendleton PHP will create and maintain the HPN through quarterly meetings/interventions with base wide/community wide partnerships.
- Ensure timely update of HPW content on the intranet and social media platforms as needed.
- Assist the PHP Project Manager/Senior Strategist in the review, development, and implementation of policies, procedures, processes, and guidelines.
- Coordinate special projects such as health promotion specific education-focused workshops, health fairs, informative displays and programs for special groups.
**Qualifications**
**Minimum Qualifications** :
**- Degree/** **Education:** Masters level education in one or more of the following areas preferred: Health Promotion, Health Policy, Public Health, Community Health, or nursing degree with Masters level preparation in nursing administration or nursing education.
**- Experience:** Three or more years of relevant experience in conducting health promotion programs.

Key Responsibilities
100% Teaching:
Teach and administer undergraduate and/or graduate courses in Health Promotion, Health Behavior, or related areas.
Hold regular office hours to assist students with coursework and answer questions regarding curriculum. May supervise graduate and undergraduate teaching assistants.
What You Will Need
* Master's degree in Health Promotion, Health Behavior, Public Health, or closely related discipline appropriate to the field of teaching/instructional tasks.
* Experience with modern pedagogical techniques.
* Ability to communicate effectively with a wide variety of audiences verbally and in writing.
* Evident commitment to educational equity.
What We Would Like You to Have
* Doctoral degree in Health Promotion, Health Behavior, Public Health, or closely related discipline appropriate to the field of teaching/instructional tasks.
* Teaching experience at the college or university level.
* Demonstrated ability to design, teach and assess courses that support inclusive learning principles and promote equitable teaching practices, to the benefit of all learners.
