MHS is seeking a Part-Time Health Services Staff Assistant (HSSA) to join the Health Center Clinic. The HSSA provides front desk support to the nurses and physicians through various administrative duties, such as receiving and transferring calls regarding student concerns, tracking paperwork, scanning medical records, filing, fax. Furthermore, assists with the coordination of various projects related to student medical data retrieval, reporting, record retention and working with several electronic systems.
This is a year-round position Monday through Friday up to 29 hours per week and hours will fall between 12:00 pm - 9:00 pm with a primary focus from 3:00 pm - 8:00 pm. Hourly rate: $19.45 - $25.96 (plus 5% shift differential), pay is determined based on experience. Assists in monitoring the annual budget for inside and outside medical services; plans and coordinates purchases and/or contracts for supplies, equipment, specialty care or ancillary services; and maintains par inventory levels.
Assists the HSA and Regional HSA to implement the Bureau's re-entry initiates including timely Inmate Skills Assessment data entry at admission and evaluation of health status documented in an updated electronic exit summary of release.
Organizes training for medical personnel within the guidelines of BOP medical program standards, American Correctional Association (ACA) standards, and individual medical program standards.
Along with all other correctional institution employees, incumbent is charged with responsibility for maintaining security of the institution. The staff correctional responsibilities precede all others required by this position and is performed on a regular and recurring basis.

Education:

Specialized Experience:

- Assignments providing a knowledge of basic health system administration philosophies, practices, and procedures, and basic government administrative policies and requirements;
- Practical assignments providing an opportunity to apply health system administration skills and principles (as the individual progresses, work assignments must be characteristic of the grade level to which he or she is assigned); and
- Oversight by an experienced health system administrator with periodic evaluation of the individual's progress and appropriate adjustment of the training program.

You will manage and evaluate facility health services activities based on company policies, contractual agreements, and healthcare goals and objectives. You will also manage your department's budget and improve the operational efficiency of your facility's health service program.
- Maintains a good working relationship with correctional personnel, nursing staff, contract providers, and outside provider agencies.
- Utilizes established corporate and correctional policies and procedures in making decisions.
- Responsible for staff scheduling and supervision including:
- Assisting management in recruiting, orienting, supervising and evaluating the performance of nursing and clerical staff.
- Directs the work of other nursing and clerical staff. This includes the selection, hiring, evaluating job performance, employee training/development, promoting and any disciplinary action, including termination
- Staff Scheduling:
- Ensures adequate coverage on all shifts.
- Fills in as needed during periods of short staffing
- Monitors call-ins and use of overtime.
- Monitors staff compliance with facility policies and unit directives by counseling employees who violate policies established by the company or the facility, after consultation with management.
- Ensures the proper flow of relevant information (policies, procedures, in-service, problems, etc.) to supervisor and supervised personnel.
- Monitors and ensures strict staff compliance with all applicable pharmacy laws.
- Organizes monthly in-service education. Ensures that staff receives in-service as required.
- Arranges for staff participation in off-facility workshops for CEUs and current nursing practice.
- Maintains current licensure on all staff requiring such licensure.
- Organizes daily activities and ensures adequate staffing for these activities.
- Maintains shift assignments.
- Reports any staff problems to management on a regular basis.
- Ensures adequate supplies and pharmaceuticals to meet the needs of the detainee/inmate population. Reports any over-usage, vendor problems, needs, recommendations and budgetary concerns to management.
- Ensures that detainees/inmates arriving at the facility are adequately oriented to services available and procedure for obtaining medical services.
- Ensures that all detainees/inmates assigned to work as medical orderlies are trained in the handling of contaminated waste and proper cleaning of contaminated surfaces. Monitors their compliance with instructions.
- Performs random audits of medical records, logs, sharps, narcotic counts, outside referrals and sick call, etc. Counsel employees or provide in-service for noted errors. All findings shall be submitted to the management for review and recommendations.
- Participates in quality assurance activities. Makes recommendations for monitoring needs and plans activities for correcting deficiencies. Reports problem areas in writing to management.
- Monitors the assignments, review of records, and care plans of the chronic disease clinics ensuring follow through per policies and procedures.
- Reviews activities of infection control nurse and makes recommendations for containment of infections.
- Assists management in monitoring environmental sanitation, safety and health related issues by performing inspections. Generates recommendations based on inspections and review of other inspections to management for further management dissemination.
- Responds to informal grievances from inmates to management’s designee. Reports any grievance with potential liability to the management for his/her information and consideration of action.
- Monitors care of serious medical conditions with the potential for life threatening or liability issues for appropriate follow-through.
- Assumes on-call status for staff problems. Assumes on-call status in absence of direct management.
- Performs other duties as assigned.
Qualifications
Competencies Required:

Under the general direction of the Director of Health Services, with established company and departmental policies and procedures, some key functions include but are not limited to:
- Supports nursing staff in the day-to-day operations of The Lodge by making daily rounds.
- Monitor care planning, documentation and the care provided to ensure that the standard care rendered to all Lodge residents is of the highest quality and the staffs performance is in accordance with the established Standard of Clinical Practice and Performance.
- Organizes and leads clinical outcome committees as assigned by the Director of Health Services.
- As part of the nursing management team, shares responsibility of being on call for in-house staff. This may mean assuming coverage as a staff nurse on the nursing units in the event of inadequate numbers of staff to care for residents.
- Assumes leadership of nursing department in the absence of a Director of Health Services.
- Provides in-service education and attendance logs
- Responsible for orienting new employees as directed by a Director of Health Services.
- Participates in the review, development and update of the Nursing Service Policy and Procedures Manual and nursing objectives and philosophies.

Education: Must possess, as a minimum, a Nursing Degree or Diploma from an accredited college, university or nursing school. BSN preferred.
Experience: Minimum of (3) years equivalent in nursing within a long term care facility. Specialized knowledge of nursing care of the elderly and rehabilitative nursing is preferred. Experience in teaching adults, infection control and quality assurance preferred.
Certification: Must hold a registered nursing (RN) license valid within the State of New Hampshire. Must be CPR certified. Must be certified and proficient in intravenous administration or eligible.

Qualifications: UnencumberedLPN License and a Bachelor Degree required. Minimum ofthree years of experience working with individuals diagnosed with developmental and with intellectual disabilities, managing a Day Habilitation program-serving individuals with intellectual disabilities or experience working in a long-term care setting with significant clinical experience in a related field serving individuals with developmental disabilities and supervisory responsibilities with proven success encompassing all areas above. Minimum 5 years of successful middle level to upper management supervisory, programmatic, operations and administrative experience required. Major Position Responsibilities
Provide oversight in service delivery to individuals with competencies in the following areas: Sensory/motor, cognitive abilities, communication, social interaction and sociability, self-direction, self-care/independent living skills, behavioral concerns, health, school experience, and employment and determine relative need for services and supports.
Provide oversight to ensure service recipients are supported and assisted in educational, enrichment or recreational activities as outlined in his/her Service Plan including community involvement.
Plan and assure that each service recipients participates in out of program community outing and/or experiences at least three times a week.
Ensure service recipients receive age-appropriate education, support and training to acquire the skills and experience needed to participate in the community, consistent with the participant's Individual Service Plan. This may include activities to support participants with building problem-solving skills, self-help, social skills, adaptive skills, daily living skills, and leisure skills. Activities and environments are designed to foster the acquisition of skills, building positive social behavior and interpersonal competence, greater independence and personal choice.
Assure programming that promotes individual interactions and experiences by emphasizing on age-appropriate community experiences, and offering a variety of activities and choices.
Assure planning, supports and assistance are provided to service recipients in educational, enrichment or recreational activities as outlined on their Service Plans to enhance inclusion in the community.
Assure planning of service recipients' indoor and group outings to community events, leisure and educational activities in the community.
Design Annual Activity Schedule for Program Supervisors and assure that Program Supervisors have Developed/Designed varied discipline lessons for review and approval by department head.
Observe the arrival of service recipients, oversee and direct the supervision and smooth flow of service recipients into Service Delivery Rooms, lead in the organization, startup and participation the day's activities.
Spend time in the Service delivery Rooms evaluate the quality of the service delivered, provide guidance and feedback to supervisors and staff on the floor.
Observe and assure that supervisors as part of the direct support-staffing ratio, spends time on the activity floor, lead in facilitating programming activities, and provide self in supporting and relieving staff during shift sessions as well as asses, inspect the environment.
Assess therapeutic elements for the individuals served and consult with support team as needed for an appropriate resolution i.e. lack of motivation in programs, inability to socialize.
Oversee and monitor overall health status of service recipients within the scope of practice and/or as directed including but not limited to regular weighing, blood pressure monitoring, blood sugar monitoring, etc., and make appropriate recommendations to enhance the service recipients' care.
Maintain proper knowledge of service recipients' health needs.
Perform body checks as required.
Oversee and Monitor health related practices and medical procedures as indicated within the scope of practice.
Interpret medical reports under the guidance of Registered Nurses.
Plan, direct and collaborate with the team to ensure proper documentation, maintenance of service recipients' medical records and disseminate medical information as instructed, including training as needed to other staff, outside personnel, etc., within the scope of practice.
Facilitate accompanying service recipients to medical evaluations as directed.
Provide skilled nursing care/treatment to service recipients as directed and within the scope of practice, e.g., wound care, ileostomy, colostomy care, other special procedures, etc. Medication administration may be required.
Serve as nursing liaison as needed with family members, professionals and other individuals/organizations in the community when indicated and within the scope of practice.
Plan and facilitate Program preparation for inspections (i.e., review of clinical charts, prescription labels for compliance, menus for nutritional information, audit first aid kits, emergency numbers posted, assure medications are secured properly, etc.)
Perform other duties as required by the nature of the position or as requested by Supervisor within the scope of practice. The duties must be performed consistent with Agency policies and New Jersey Nurse Practice Acts.
Develop positive, cooperative relationships with service recipients, families, guardians, staff, Support staff, volunteers, all agency departments/functional components, and community members including, CARF, DDD, and other State/Federal and Local agencies.
Oversee employment of qualified personnel in a timely and efficient manner.
Complete Personnel Status Notice in a timely manner upon receiving notification of staff status change or upon executing approved status change, submit to next level supervisor consistent with protocol for review and further processing. Employ, train and maintain a list of at least five Day Habilitation Support Professional Substitutes.
Assemble proficient and consistent management and Day Habilitation Support team.
Recommend program procedures and policies which ensure safe and secure efficient program operations in accordance with agency as well as external requirements, and which meet the agency mission and "Person Centered" standards.
Visit programs routinely to get staff input/feedback, interact with staff/service recipients and observe service delivery relative to operational objectives. Assess, evaluate and review programmatic practices. Observe and assess management staff on service delivery methods, styles and practices in conformance with department objectives. Provide guidance, support and mentoring as needed.
Audit, review, and inspect programs to assure operational practices, records and program systems are consistent with CARF, DDD, DHS, CCW standards and Agency standards etc.
Develop a working knowledge of DDD, Day Habilitation Certification standards, and CARF regulations and Agency Policies and Procedures. Assure the completion of all required documentation/records and administrative assignments pertaining to service recipients, programs, staff and agency administration in accordance with agency, CARF, DDD, DHS, CCW and/or other funding sources standards and requirements.
Maintain a working knowledge of all phases of program operations, organize, direct and provide support.
Support and assure that programs maintain required certifications, standards and manuals.
Facilitate and provide on agency/departmental designed new employee on-boarding communications, expectations and administrative orientations.
Facilitate employee Administrative Orientation and ongoing training on agency policies and procedure. Secure the signed orientation documentation, file and distribute as required. Responsible for auditing training records regularly, register staff for training and assure that all staff members are fully trained in all DDD trainings meet criteria of a minimum of 12 Hours of Professional Development completed annually, State trainings, etc. assure proper re-certifications, monitoring attendance and addressing training attendance performance in a timely manner.
Ensure that staff members are fully trained on program specific trainings, department, agency and DDD trainings, and meet criteria of a minimum of 12 Hours of Professional Development completed annually, etc Orient and train all assigned staff in Program specific and agency policies and procedures; monitor, and evaluate staff performance in trainings and compliance with all policies and procedures.
Complete all supervisory trainings as designated and attend professional development management workshop as directed.
Audit new or configure Medication Administration Records (MARs) in a timely manner, comparing MAR to prescription and Bubble pack assuring accuracy. Assure ongoing monitoring and documentation of medication administration, medication counts, and reordering as well as service recipients' health needs on electronic agency platform (Therap). Identify any emerging health needs and provide appropriate intervention.
Assure adequate medication refills, adequate medication supply at all times and proper medication administration standards.
Assure staff members are writing the date, the time and their initials on the bubble pack/bingo cards after administering medication.
Conduct meticulous random medication pack/bingo cards audits to ascertain proper medication administration and charting.
Assure programs have adequate Personal Protective Equipment's, cleaning supplies, etc.
Flexible to support staff, program, department or agency in completing outstanding projects, tasks or operational needs and serve on agency committees as assigned.
Audit and maintain policies, procedures and systems which involve service recipients, agency funds and agency property, use of agency and contract resources applicable to assigned programs and assure compliance with all DHS, Day Habilitation Certification standards, governmental and agency regulations and requirements. Ensure routinely agency property and funds are safeguarded.
Responsible for obtaining from Finance/Accounts Payable monthly or frequently a print out of a list of the service recipients' recorded checks issued to the supervisors, audit the service recipients' electronic ledgers and account for accurate and timely entries of the checks received/associated cash entered in each service recipient's Therap electronic ledger.
Assure the development, maintenance and accounting of all financial records and reports for program and service recipients' finances. Ensure effective management of agency petty cash and service recipients' funds, proper documentations appropriate practices consistent with agency policy. Assure financial report is submitted on time.
Monitor and assure compliance with all personnel and program policies/procedures related to staff and administrative requirements. Assure system update or record revision upon service recipient's, staffing or environmental/facilities.
Seek guidance from immediate supervisor on managerial and supervisory matters for desired administrative outcomes.
Assure required administrative and HR authorizations prior to executing personnel actions.
Assure required prior administrative review of letters, correspondences etc. intended for external distribution.

Monitor budget used and assure resources are utilized within the framework of the approved budget. Recommend budget modifications as required to assure effective and efficient service delivery.
Generate and reconcile requisitions, audit and validate expenses on receipts/invoices. Execute the agency procurement policies, procedures and duties.
Monitor and ensure that staff is following assigned schedules as well as clocking in/out payroll procedures. Review, approve and submit to payroll system time sheets for staff.
Understand and comply with Agency mission and strategic plans. Assure that program goals aligns with the agency's mission.
Partner with Agency Support Staff to enhance the quality of services and meet the Program, department and Agency goals and objectives. Follow through on recommendations from Nurses, Behaviorists, Maintenance Dept, Transportation Dept, Case managers, Day Habilitation auditors etc.
Respect confidentiality of all information pertaining to service recipients, employees and others throughout the Agency.
Active involvement in the Pre-Admission Meeting, complete the Pre-Admission Documentation, and secure all new admission documentation consistent with licensing, state and agency regulations. Assure successful admission into program.
Assure program (s) attain an overall performance rating of 88% or higher in internal and external expectation, objective, conformity audits/reviews.
Assure all required information and documentations for service recipient's placement and service delivery are obtained and maintained in service recipient's record file for effective plan development, effective service delivery, acquisition of a 3Year Day Habilitation Certification, fulfillment of Center for Medicaid Services requirements and agency objectives.
Assure program operational practices align with DDD Day Habilitation Standards, Supports Program Standards, and Agency Standards etc. Ensure each program is fully prepared for a successful day habilitation inspection/audit and attainment of a 3 Year Day Habilitation Certification or the highest level of certification/accreditation.
Run Therap activity report, audit and verify that each staff logs into Therap Daily and reviews all logs, Email, & S.Comm as required, each staff rotates daily in attending to or supporting different individuals in program, each staff rotates in various shift tasks namely: Rotation in community activities facilitation, Rotation in Med Administration & Meal prep, Rotation in Hygiene Support & Life Skills Training, Rotation in program cleaning, Rotation in Therap Data entry, Rotation in checking off on shift tasks as designated in Therap etc.
To ensure that the agency is able to bill Medicaid for the services provided, run Therap ISP Data report daily to ensure every shift has inputted data daily (Daily Tracking) on Therap for the service (s) provided to the service recipient directly i.e. personal care goals, ISP/IHP goals. Ensure immediate correction of any missed entries.
Ensure each supervisor develops and implements a weekly shift assignment schedule outlining daily staff rotations in assignments.
Assure each service recipient has a current Individual Service Plan (ISP), a Person Centered Plan (PCPT) or Service Detail Report (SDR).
Review provided information related to the individual, including the PCPT, previous ISP(s), available assessments, and evaluation data, as appropriate/relevant.
Actively participate in the planning of team meetings and work cooperatively in achieving consensus in the spirit of the ISP operating principles. Participate in IDT, ISP process and assure identified goals are accomplished in accordance with Agency policies. Ensure correct implementation of ISP objectives and Behavior Plans as written.
Oversee implementation of skill development, Behavior plans, ISP's and other plans as developed and directed.
Ensure recording of data (in Therap) relative to assigned outcomes for service recipients including ISP tracking, Daily Training Record (DTR) etc.
Ensure completion, review and timely submission of monthly reports for service recipients including ISP reporting, service recipients' finances, agency petty cash, etc.
Notify the Support Team and/or request a special team meeting to be scheduled whenever there is a significant change in the individual's status.
Review, periodically, the ISP document and report inconsistencies in service recipients' support needs (Ambulation, Self-Care, Behavioral & Medical) to appropriate agency personnel.
Responsible for receiving Nutrition Audit Report/Findings and immediately scan the report to the E Drive shared folder labeled "Department of Residential & Adult Day Prog-Nutritionist Consultant Audit Report" into the respective program's folder and notify Residential' s Assistant Directors, Supervisors, Assistant Supervisor, and healthcare supports through groups emails for access, prompt review follow up and/or timely and effective implementation.
Responsible for following up on recommendations made by the Nutrition Consultant and facilitation of implementation by front line staff.
Ensure the programs are implementing a comprehensive and person centered community integration and recreation program for the individuals served.
Facilitate donation requests annually on behalf of service recipients' invitations to Courier News Wish Book, for the Benefit of service recipients.
Facilitate staff meetings to assure a productive flow of communications and to address service recipients' and program related issues.
Attend and participate in all service and professional meetings, conferences, seminars, workshops and committees as assigned or as necessary for successful completion of agency and the Department of Day Health Services functional goals.
Maintain timely, thorough and effective communications regarding individual served and operations, to all appropriate parties including but not limited to Day Habilitation Support Staff, Supervisors, Directors, etc.
Assure timely, thorough and effective communications regarding program, employee operations, to all appropriate parties including but not limited to program staff, supervisor, director and/or executive designee.
Responsible for ensuring that, upon notification of qualifying or active incident, appropriate measures are taken consistently with agency policies, procedures and training, and support is directed toward the staff and the service recipient involved in a crisis. Provide guidance, direction, intervention, etc. and ensure that incidents, meeting the criteria of Unusual Incident Report, are reported through automated UIR/GER documentation, standardized notifications activated as indicated through DDD circular and Arc of Union County trainings.
Responsible for ensuring that upon notification of an employee accident at work, appropriate measures is taken consistently with agency policies and procedures. Provide guidance, direction, and assure claim is reported through standardized notification including Employee Accident Report, Root Cause Analysis documentation.
Ensure that staff are implementing agency Guidelines for Safe Transporting and Parking for Residential, Transportation, Day Program staff and Agency Employees, Safe Driving Policy.
Monitor agency vehicles under direct oversight, assure that they are utilized for the approved agency service or business purpose, within the approved operations timeframes and stored or parked at the approved location. Provide report to the Associate Executive Director Operations on vehicle resource mismanagement or deviations from agency policy.
Assure all vehicles are in good status and equipped to receive satisfactory inspection review by Office of Licensing, follow through on vehicle deficiencies featured on inspection/audit reports.
Assure that the agency vehicles resource will be utilized for the approved agency service or business purpose, within the approved operations timeframes and stored or parked at the approved location.
Review, resolve and follow through on Safe Driving Policy Violations Reports promptly to ensure the safety and welfare of service recipients. These include Escalated Violations Report, Coach Warnings Report, Violations Exceeding Severity, etc.
Ensure completion and submission of the Agency Vehicle Accident Report within 24 hours, Police Crash Investigation Report, damage related pictures, and Repair Cost Estimate Report promptly to the HR, and Director of Transportation.
Actively support all agency policies and procedures to all internal and external contacts.
Prepare programs for external audits including Day Habilitation Certification review, CCW, CARF, etc.
Demonstrate a thorough understanding Departmental of Operational Plan, Agency Strategic Plan, and Human Resource Policies.
Assure the appropriate review, reporting and investigating of all unusual incidents, allegations, or charges pertaining to service recipients, staff or programs.
As part of the supervisory and monitoring responsibilities of this position, the incumbent may be required to train staff as directed; this will include but not be limited to activity planning, community integration, etc.
Serve, when necessary, as backup for Supervisor, Day Health Services, Support Professional, Day Health Services to ensure proper coverage and care for service recipients.
Provide proactive planning for programs relative to unexpected events such as inclement weather; coordinate and communicate decision making to assure service recipients' and staff safety.
Develop and update the Emergency Evacuation Plan (EEP) Narrative annually and Evacuation Assessments/Evaluations upon changes on individual's abilities to evacuate/needs promptly with the Director and Supervisor. Assure the EE Plan aligns with the Evacuation Assessment/Evaluation consistently at all times.
Assure that fire drills are conducted monthly in line with the annual Fire Drill Schedule with individuals served present.
Assure all types of disaster drills are run at least once a year in line with the Disaster Drill schedule assigned by the director including Bomb Threats, Blizzard, Earthquake, Hurricane, Tornado, Utility Failures, Act of Terrorism, Violent Events, and Other Threatening Situations etc.
Audit (Medical/Financial, Facility), review, and inspect programs to assure operational practices, records and facility systems are consistent with Day Habilitation standards, CARF, DDD, DHS, CCP standards, programs are operating within departmentally established guidelines and are maintaining progressive improvements in designated areas of performance including Quality Assurance etc.
Inform Director, Day Habilitation Services and update records in a timely manner regarding guardian's refusal to comply with agency service delivery polices/procedures, provision of required documents, or declination to consent to service delivery, e.g. medical treatment, community integration activities, photograph taking/publication, etc.
Initiate "on call" procedures with respect to arising incidents including 911 emergency calls, life threatening incidents, and/or operational breakdown etc. and complete Unusual Incident Reports (UIR) in a timely manner consistent with agency policy and state law.
Responsible to report to immediate Supervisor or through the chain of command any suspected incidents of abuse, neglect or exploitation of service recipients and complete Unusual Incident Reports (UIR) in a timely manner consistent with agency policy and state law.
Monitor and safeguard agency property and funds.
Perform duties according to the methods, manner and spirit of the ideals of the field of Human Services and within profession/job specific guidelines, including wearing appropriate attire, being a role model for staff, service recipients etc.
Must fully cooperate with all investigations, inspections, etc. internally and externally including full cooperation with the Department of Human Services, the Division of Developmental Disabilities, or any other duly authorized person regarding the care and service given to individuals' served by this Agency.
The Arc of Union County, Inc. is contracted with the New Jersey Department of Human Services to provide and facilitate essential personal care and health promotion services to individuals with disabilities and other comorbid conditions. Physical Demands : The physical demands for this position must be met by an employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee may be regularly required to maintain a stationary position (standing and sitting), move/traverse, kneel, bend, reach with hands and arms for extended periods of time, twist, carry, lift up to 35 pounds, squat and drive a 6-person passenger van, may require operating manual devices such as hydraulic lifts, wheelchair lifts, vehicle doors, etc. for the purpose of transporting service recipients. Agility with wrist and finger dexterity to access, key and sort electronic information or file documents, ability to view computer and/or electronic device for extended period and other physical duties as directed by your supervisor and/or Director.
Ability to apply Agency Training Crisis Management Verbal and Physical Techniques during a crisis consistent with criteria as trained and in accordance with agency policies and procedure.
The essential Cognitive Functions of this position may include ability to analyze, compute, hear and repeat information, take notes, read documents, generate written information, independently problem solve, tolerate interruptions, maintain positive public relations, organize, verbally communicate and converse with a diverse group of people among others.

Work Environment - The agency offers safety trainings such as Universal Precautions/Infectious diseases, COVID-19 Education and Safety Practices among others to promote a safe environment for all employees and service recipients including visitors. Personal Protective Equipment's and necessary supplies such as disinfectants are supplied to all employees free or charge. Each employee is responsible to complete offered trainings and implement the health and safety protocols and mitigation practices consistently.
The agency may assign employees to any duties and to any work site it deems necessary in its sole discretion.
This is an administrative position; you are required to communicate abrupt absence directly to your supervisor/operation management and verbally handover (not necessary physically) programs to your supervisor for the smooth operation of programs and the safety and wellbeing of the service recipients. This position requires four (4) weeks' notice prior to leaving or resignation.
