Are you an empathetic and compassionate caregiver who takes pride in helping people lead more independent lives? Our busy home health agency is seeking an experienced home health aide to assist our case managers in implementing patient-specific care plans.


You’ll give our patients companionship and help with basic medical care, light housekeeping duties, personal care, and hygiene tasks so that they are empowered to live happily at home. You must have 1-3 years of experience as an HHA or caregiver and possess excellent communication skills. If you enjoy a position that is varied and rewarding every day, we want to hear from you!

Compensation:

$16 - $18 hourly

Responsibilities:
  • Facilitate personal care tasks like scheduling appointments, calling doctors’ offices, coordinating Medicare and other insurance details
  • Ensure quality companionship and provide psychological and emotional support for patients and family members
  • Drive patients as needed, for grocery shopping, running errands, or doctors’ visits, when necessary
  • Implement care plans as directed by the case manager, and keep updated and accurate care records
  • Stay on top of light housekeeping tasks including but not limited to laundry, bed making, vacuuming, etc.
  • Administer Medications: Dispense medications as directed by the Registered Nurse (RN), maintaining an annual error rate of no more than one mistake.
  • Resident Response: Attend to resident call lights within 10 minutes to ensure responsive and attentive care.
  • Light Housekeeping: Perform regular light housekeeping in resident rooms at least once weekly to promote a clean and comfortable living space.
  • Service Charting: Accurately document all services provided without missing tasks, ensuring compliance with company standards.
  • Assistance with ADLs: Support residents in Activities of Daily Living (ADLs) according to each individual’s Client Service Plan, with a zero-tolerance policy for missed tasks.


Qualifications:
  • Capable of lifting up to 100 lbs, you may also be required to stoop, bend, reach, and transfer patients up to 250 lbs as needed
  • Compassionate, kind, and empathetic personality is essential, with superb organization and communications skills
  • 1-3 years of experience as an HHA/ home health aide, personal caregiver, nursing assistant, or personal care aid for a home health agency, at patient homes or care facilities settings is needed
  • Possess a valid state driver’s license, current auto insurance, and reliable transportation
  • A high school diploma or GED equivalent is required - some college is an asset

  • Maintains current progress and service notes and submits all required documentation and verbal feedback to Supervisor.
  • Attends and participates in multi-disciplinary team meetings, training and other meetings as required.
  • Reports incidents of maltreatment in accordance with Sevita policy.
  • Promotes positive relationships with individuals receiving services, families, payers, and community representatives.
  • Performs other related duties and activities as required.
Education and Experience:
  • High School diploma or equivalent.
Certificates, Licenses, and Registrations:
  • State-issued Home Health Aid certification or CNA license is required
  • All state required training and certification completed in mandated timeframes.

Provides in-home support services to individuals receiving services, including personal care such as assistance with bathing, dressing, eating, respite care, life skill activities, and shopping for participants. May schedule visits directly with family of participants.

ESSENTIAL JOB FUNCTIONS OF A HHA/CAREGIVER:
