Job Summary: We are seeking a skilled Healthcare Coordination Specialist to manage, coordinate, and integrate care coordination services within our health system.
This role involves providing patient-centered care, coordinating services in hospital or clinic settings, and promoting optimal care that respects individual choices and meets healthcare needs.
The ideal candidate will work collaboratively with clinical leaders to develop and implement protocols, and contribute to a cooperative working relationship with other team members.
Key Responsibilities:
- Coordinate care coordination services for patients based on acuity level
- Work closely with clinical staff to promote person-centered care and address healthcare needs
- Develop and implement protocols to improve care coordination and patient outcomes
- Maintain effective communication with patients, families, and healthcare teams
- Collaborate with clinical leaders to achieve system goals and objectives
Required Qualifications:
- Registered Nurse (BSN) degree preferred
- Current New Mexico Nursing License required
- Considerable knowledge of healthcare facilities, policies, and issues
- Ability to coordinate functions and activities for maximum integration of services
- Strong communication and interpersonal skills
- Ability to work effectively in diverse settings and populations
Experience: Minimum of two years of experience in a healthcare setting, with related experience in quality, care coordination, and population health preferred.

The Registered Nurse is a key member of our healthcare team. This role requires a strong focus on patient care and coordination, ensuring high-quality treatment in an efficient and safe environment.
Responsibilities:- Coordinate and implement patient care plans tailored to the specific needs of each patient population served.
- Collaborate with interdisciplinary teams to ensure seamless care delivery and effective communication.

A graduate degree from an accredited Registered Nursing program is required.
Certification/Licenses:A valid New Mexico RN license or compact state license is mandatory. BLS certification is also required within 2 weeks of hire date. All other necessary certifications must be obtained within 6 months of employment.
Skills:Current knowledge and skills appropriate to the age/type of patient population served. Knowledgeable and sensitive to patients' rights in the delivery of care. Communicates effectively in a clear, concise manner suitable for the developmental age of the patient.
Working Environment:This role operates in a clean, well-lit, ventilated smoke-free environment. It may involve exposure to infectious diseases and x-rays. The working hours are varied, with flexibility due to unexpected changes in schedule and emergencies.
Physical Requirements:Must be able to handle emergency/crisis situations, perform prolonged standing, walking, and heavy physical effort to position, push, and/or transfer patients or equipment and supplies. Requires considerable reaching, stooping, bending, kneeling, and crouching. Ability to judge distance and space relationships, see peripherally, distinguish, and identify different colors. Hearing and visual acuity within normal or correctable limits. Manual dexterity and fine motor coordination are essential.
