**KEY RESPONSIBILITIES:**
+ Warmly Greet Patients and Guests: Greet all patients, guests, and employees entering the facilities with a smile and appropriate salutation (e.g., good morning, good afternoon ).
+ Ensure a Positive Experience: Welcome all customers following the Emory Healthcare customer service model to create a warm, positive environment.
+ Assist with Check-In Process: Support patients with check-in, including using online applications and troubleshooting technical issues.
+ Effective Communication and Service Recovery: Communicate with patients, families, and staff to address concerns and resolve complaints in real-time, involving leadership as needed.
+ Maintain a Clean Environment: Keep entrances, waiting rooms, and storage areas tidy and sanitized, restocking supplies as needed.
+ Complete Administrative Tasks: Confirm and update patient demographics, manage registrations, update insurance information, schedule appointments, handle referrals, and collect copays.
+ Proactive Patient Rounding: Round on patients in waiting areas to ensure comfort and address immediate needs.
**MINIMUM QUALIFICATIONS:**
+ High school diploma, GED, or equivalent.
+ Must be at least 18 years of age and authorized to work in the United States.
+ Proficiency in English, with strong communication skills.
+ Strong interpersonal skills with the ability to work collaboratively in a team environment.
+ Basic digital literacy and comfort navigating software applications.
+ Ability to learn new processes, procedures, and software quickly, with attention to detail and accuracy.
+ Successful completion of software training upon hire.

Atlanta Center for Reproductive Medicine is looking for a full time Patient Care Coordinator to join our growing team. This position is an integral part of our team and vital to helping us create a smooth and personal experience for each of our patients. POSITION OVERVIEW
The main job of a patient care coordinator is to ensure a smooth patient flow and to be a liaison between the patient and the clinical team. A coordinator helps to ensure patients understand every aspect of their care while also working with the physician team to coordinate communications and provide administrative services to support the clinical team. As a patient care coordinator, you would ensure that patients receive the care they need and that they understand their medical condition, medications, and other instructions.
The Patient Care Coordinator will also be responsible for performing high volume scheduling functions in a streamlined environment, which is fully computerized. Position requires application of special instructions and procedural information to identify type of procedure requested and schedule patients accordingly.
The Patient Care Coordinator will be responsible for scheduling of patient appointments for medical procedures. Interacts extensively with physicians/diagnostic staff, office staff, and patients via phone, fax, and email, ensuring accurate and effective scheduling of appointments for needed medical procedures. Independently and accurately handles a wide range of scheduling duties in a fast-paced environment. Works collaboratively with team members to provide quality service to proactively support efforts that ensure delivery of safe patient care and services and promote a safe environment.
ESSENTIAL DUTIES
- Schedule all patient appointments. Make appropriate scheduling changes, canceling, rescheduling and confirming appointments.
- Provide compassion and emotional support to patients throughout their experience
- Provide patients with a high level of customer service
- Answer phone calls in a prompt, courteous and efficient manner.
- Respond to patients via telephone by assessing needs and answer general questions regarding their plan of care.
- Triage patient inquiries and send to the correct team member or department
- Responsible for handling physician correspondence.
- Provide excellent customer service by anticipating patient needs.
- Work with schedulers to confirm questionnaire and previous medical records are received prior to a new patient appointment
- Greet new patients and escort them to meet with the physician
- Prepare informational packets to be given to the patients
- Coordinate with the clinical team to inform and educate the patient about their plan of care.
- Completes request for copies of patient’s medical record
- Witness consent forms as needed
- Send correspondence to referring doctors
- Send correspondence to patients while going through treatment
- Provide retention calls to established patients
- Create patient charts and order labs as indicated by the plan of care
- Maintain operations by following policies and procedures. Report changes as needed.
PREREQUISITE KNOWLEDGE, SKILLS, AND EDUCATION
- Required to work one weekend a month

Patient Care Technician (PCT)
Patient Care Technicians (PCTs) at Health Systems Management support and work under the guidance of RNs to provide dialysis treatment to multiple patients at a time. PCTs have a combination of technical skills and compassion needed to improve the quality of life for our patients while promoting positive patient outcomes. Successful candidates will have the opportunity to learn the discipline of dialysis. To be considered for this position q ualified candidates are required to have a previous dialysis experience, CCHT or CHT (Bonent) certifications.

PCT Responsibilities and Physical Demands:
The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of the job.
- Set up of the hemodialysis machine.
- Greet patients and collect details regarding their previous treatment. Report any concerns or observations to the lead nurse.
- Assist with and prepare the patient's vascular access for treatment.
- Initiate, monitor, and discontinue dialysis treatment per written policies, procedures, and standards of care.
- Obtain patient's vital signs before, during, and after dialysis treatment and appropriately document in patient medical record.
- Responsible for appropriately calculating and documenting patient's fluid removal.
- PCT performs appropriate access cannulation and reports any difficulties or unusual findings to the lead nurse.
- Monitor the patient for any adverse reaction(s) and conduct appropriate intervention.
- Monitor the dialysis machine for any malfunction. Report any malfunctions to the lead nurse.
- Demonstrates knowledge of infection control as related to hand washing, personal protective equipment (PPE), and universal precautions.
- Maintain patient safety and comfort, which may include assisting with personal care needs and activities of daily living.
- Responds appropriately to emergency situations.
- Note you will have exposure to blood, needles, and various other bodily fluids.
- This position provides direct patient care that regularly involves heavy lifting, moving of patients, and assisting with ambulation.
- PCT will spend considerable time walking, bending, stretching, and standing.
- The employee may occasionally be required to move machines, equipment, and may lift chemical and water solutions of up to 30 pounds and high as five feet.
- You are required to acknowledge the inter-facility coverage policy that supports floating to various clinics during and after your training, if needed.

Education Requirements and Position Qualifications:

- 6 Months of healthcare and/or direct patient care experience is required.
- Continued employment is dependent on successful completion of the Health Systems Management training program in its entirety.
- The ability to work long hours is required. You could be on your feet for extended periods of time. Our day ends when patients’ care is completed.
- You must have a flexible schedule and be able to work mornings, evenings, weekends, and holidays.
- Georgia Applicants - Previous dialysis experience is required. CCHT is preferred.
- Must have or obtain Basic Life Support certification approved by the American Heart Association
- Must be comfortable working around blood and needles, including cannulation, and obtaining labs.
- Excellent bedside manner and communication skills.
