Position Summary

Using a community advocacy model, the Health Systems Navigator will provide support for folks with intersecting marginalized identities who are more likely to face barriers while navigating complex healthcare systems and medical institutions. The Navigator will help participants access healthcare and mental health support through referrals and will provide whole-patient care through one-on-one meetings with program participants. The Navigator will be a subject matter expert on gender affirming care services such as medical transitioning, legal name changes, insurance navigation, and PrEP/HIV care. The Navigator will be responsible for providing a warm hand off to partner organizations.

Duties and Responsibilities

  • Collaborate with Mental Health Program Coordinator and Supervisor to provide patient centered referrals and connection to care according to individual care plan
  • Engage and connect members served to mental health, primary care, substance use, and specialty health care services
  • Helps individuals develop personal health and wellness goals; educates them on how to advocate for themselves and effectively communicate their needs; and supports them in overcoming barriers and building resilience
  • This includes conducting one-on-one sessions, communicating with clients via texts and phone calls, and providing information on community-based services
  • The Peer Navigator will have access to electronic health records and scheduling software to facilitate clear communication between the provider team and front office, supporting appointment scheduling and follow-ups
  • The Peer Navigator will accompany clients to appointments as needed and must adhere to HIPAA compliance policies and confidentiality rules
  • Provide peer support to individuals receiving gender affirming care at Kelley Ross Pharmacy
  • Provide peer support to individuals receiving mental health counseling through the Mental Health Program at Seattle’s LGBTQ+ Program

Qualifications (required)

The Role: Healthcare Navigator, SSVF

What You'll Do: You'll work with Veterans with complex needs who require assistance accessing health care services and adhering to health care plans. The main goal of this position is to improve access to needed health care. You'll provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation to Veterans. You'll connect Veterans to health care benefits, working closely with their primary care provider and members of their interdisciplinary treatment team.   • Conduct NON-CLINICAL assessments to assess strengths, limitations, risk factors, and internal/external supports and service needs to optimize the Veteran's ability to access and maintain health care services

  • Advocate and work closely with Veterans in communicating preferences in care and personal health-related goals

What You'll Bring to SupportWorks:

  • Bachelor of Social Work or equivalent educational background in human/social services/healthcare preferred

Job Summary

The Health Care Navigators are responsible for connecting Veterans to VA healthcare benefits and/or community healthcare benefits, providing case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and other administrative duties as assigned. The healthcare navigator will act as a liaison between the SSVF grantee and the VA or community medical clinic and works with a population of Veterans with complex needs who require assistance accessing healthcare services or adhering to healthcare plans.

This position works in conjunction with the SSVF case management team, the Veteran’s assigned interdisciplinary team, which includes medical, nursing, administrative specialists, and case management personnel. The SSVF healthcare navigator works collaboratively with the team and the Veteran to identify and address systems challenges for enhanced care coordination as needed.

This position also includes outreach to Veteran households throughout the program’s service area as needed.

The duties and responsibilities of the Health Care Navigator are subject to change depending on the needs of the SSVF grantee and the Veteran population experiencing homelessness in a geographic catchment area.

Essential Duties and Responsibilities

  • Non-Clinical Assessments: conducts assessments of the Veteran in collaboration with interdisciplinary health teams, the Veterans family members, and significant others to assess the Veteran’s situation, potential barriers to care, and the impact of such barriers on the Veteran’s ability to access and maintain health care services.

  • Assists team in conducting outreach to inform eligible clients of services provided and to develop connections with outside agencies.

  • Receives and assesses calls for service and conducts intake process as needed.

  • Documents all calls received and disposition each in the format provided.

  • Acts as a community liaison for Volunteers of America, maintaining cooperative working relations throughout the community and providing accurate program information through presentation and literature.

  • Serves as a liaison to VA and community health care programs and represents the SSVF program in contacts with other agencies and the public.

  • Helps coordinate supportive and additional services with the Veteran, including but not limited to housing, financial benefits, and transportation, and provides referrals upon the Veteran’s preference

  • Assists in identifying the Veteran and Veteran family’s health education needs and provides education services and materials that match the health literacy level of the Veteran

Summary: Under direct supervision of the Program Director provides services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. ESSENTIAL DUTIES AND RESPONSIBILITIES

* Coordinate pre-admission and admission processes by obtaining pertinent information from patients and/or family members, referral sources, and centralized intake for admission and complete admission paperwork with family and patient.
* Verify that the patient room, etc. is ready prior to admission.
* Sustain contact and provide support to patient/families to include help in dealing with the patient's transition.
* Provide frequent visits to new admissions to provide a consistent and well received patient experience.
* Orient the patient to the center environment.
* Participate in center's IDT (Interdisciplinary Team) to assist in healthcare navigation needs.
* Maintain frequent contact with center's central intake coordinator(s) within designated response times.
* Coordinate back up plans for outreach, tours and admissions processes for nights and weekends.
* Oversee development of center's strategic outreach plans and activities.
* Develop partnerships and consistent communication flows with affiliated healthcare partners/navigators/advocates related to delivering the appropriate level of care, in the most appropriate setting and at the most appropriate time based on patient's needs.
* Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
* Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
* Performs other duties as necessary to ensure the success of the System.

SKILLS AND ABILITIES

* Maintain up-to-date information about agencies to which referrals may be made.
* Communicate admission information to other departments.
* Complete understanding of Healthcare Navigation Statement and Support functionality.
* Complete understanding of admission packet and ability to explain to family/patient.
* Knowledge of long-term care admission requirements.
* Knowledge of advanced directives.
* Knowledge and understanding of Medicare, Medicaid, Private Pay and Third Party reimbursement.

MINIMUM QUALIFICATIONS

* Bachelor's degree in Business Administration, Social Work, Marketing or related field
* Associate's degree in related field with three years experience
