Job Title: Experienced Nurse Specialist
We have a rewarding opportunity for an experienced Licensed Practical/Vocational Nurse to join our mission-driven team. As a Nurse Specialist, you will play a vital role in providing high-quality nursing services and supports to individuals with diverse needs.
Your primary responsibilities will include:
- Monitoring the health conditions of individuals served and ensuring quality services are provided.
- Administering medications and providing documentation, as well as overseeing and monitoring staff members who administer medications.
- Providing prescribed medical treatment by physicians' orders and personal care services.
- Conducting assessments, implementing healthcare services to be included in each Individual Service Plan (ISP), and responding to changes of condition.
- Providing training to staff members on universal precautions, infection control, and medication administration.
- Serving as liaison and advocate with other medical and professional service providers.
- Providing training for individuals served on topics including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation.
- Associates Degree in Nursing or related field, or a graduate of an accredited LPN program.
- One year of experience working with the population served in a nursing capacity.
- Current state LPN or LVN License.
- Current driver's license, car registration, and auto insurance.
- Additional certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training.
- Ability to effectively communicate with staff, individuals served, and families.
