Job Description
This is a great opportunity for a highly motivated Clinical Pharmacist to work within the Primary Care team to provide high quality patient care. This is a full-time position with variable hours Monday - Friday. No weekends or call.
**Primary Responsibilities:**
+ Embedded within the primary care team with a focus on management hypertension, hyperlipidemia, diabetes, as well as other chronic disease states
+ Provide direct patient care based on evidence-based guidelines and protocols
+ Implement drug therapy changes and disease state management utilizing a Collaborative Drug Therapy Agreement (CDTA)
+ Perform patient medication reconciliation, medication optimization and 5-star quality reviews
+ Work directly with providers and clinical care teams to implement cost-effective medication interventions
+ Provide clinical medication education to both patients and providers

**Required Qualifications:**
+ Bachelor of Pharmacy
+ Residency trained or 3+ years of experience

+ Direct patient care experience

Impact Recruiting Solutions is currently seeking a Clinical Social Worker Health Care Facility Surveyor to fill an opening with a Quality Improvement Consulting Company and will work in a technically exciting environment supporting internal and external customers nationwide.
Requirements of the Clinical Social Worker Health Care Facility Surveyor :
- Must have a Master's degree in Social Work
- Must have successfully completed CMS’s Long-Term Care Basic Training and passed the Surveyor Minimum Qualifications Test (SMQT).
- Must have at least two (2) years of recent experience working as a surveyor accrediting or certifying facilities that serve the residents of long-term care facilities.
- Must maintain current licensure to practice as an LCSW
- Demonstrated history of independent decision-making skills to direct and effectively manage the survey process.
- Ability to set priorities independently and collectively in performing survey tasks.
- Ability to openly discuss conflicts/controversy, and to seek assistance when appropriate to make decisions and resolve conflicts.
- U.S. Citizenship is required for this position
- Ability to travel up to 75% of the time on a regular basis is required

The Senior Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service, and adhere to Lean processes. Supports the teams in meeting financial, clinical, and service goals. Provides float support for 6 surgeons, including front and back-office duties, as needed.
Schedule: Monday through Friday, 8:30am-5pm or 8am-4:30pm PST

Primary Responsibilities:
- Provide front and back-office support in a float Patient Care Coordinator/Scheduler role
- Answers all phone calls from 3 phone queues (scheduling, high priority, and overflow)
- Provide high level of customer service while communicating with internal departments and external provider office staff as pertains to performance of job duties
- Review and process prior authorization requests to meet departmental key indicator productivity
- Verifies patient insurance eligibility & benefits to include applicable co-pays, co-insurances, deductibles, Out of Pocket Maximum, appropriate service facilities, services covered and which services if any require prior authorization
- Intakes all demographic information for new patients
- Updates demographic information for existing patients
- Collects insurance information
- Requests medical records from the PCP/hospitals/referring physician for new patients
- Schedules/Reschedules/Cancels patient's appointments for multi-location multi-providers for an Internal Medicine practice
- Routes calls throughout the office to the appropriate department
- Submit Specialists Referrals with appropriate clinical information using established protocols & procedures
- Resolve customer service inquiries which could include Benefit and Eligibility information, Authorization for treatment, Specialists Orders/Referrals
- Relays messages from patients/specialist/pharmacies to the appropriate individual
- Willing to work with others as well as an individual to meet company goals
- Handle 90+ incoming calls daily
- Accepting all incoming HMO and FFS referrals 100+ daily via portals, faxes, messages
- Handle all hospital face sheets/service messages/voicemails/faxes/direct messages and add-ons
- Exceed attendance standards set by company policy
- Responsible for contacting patient leads for cosmetic consultations
- Monitoring workflow
- Training new hires
- Creating and managing team schedules
- Assist with onboarding new employees
- Ability to support all staff and leadership in their roles
- Ability to de-escalate and resolve patient complaints
- Any other duties that may arise
