This job, as part of a physician team, ensures that utilization management responsibilities are performed in accordance with the highest and most current clinical standards. The incumbent reviews escalated cases electronically and using Medical Policy criteria sets to evaluate the medical necessity and appropriateness of the requested treatment of service. **ESSENTIAL RESPONSIBILITIES**
+ Conduct electronic review of escalated cases against medical policy criteria, which may include telephonic peer to peer discussions, to determine medical necessity and appropriateness. Complete initial determination of cases, review of appeals and grievances, and other reviews as assigned. Compose clear and concise rationales for member and provider determination notifications all while adhering to required compliance standards (NCQA, URAC, CMS, DOH, and DOL regulations, etc.). Ensure that all aspects of the medical management process are consistent with community standards of care.
+ Participate as a member of the CMDM multidisciplinary team. Attend huddles and grand rounds. Advise multidisciplinary team on cases that require physician expertise.

As part of a larger population health strategy, the Care Facilitator will play an integral role in the larger ambulatory care management redesign for Sparrow Medical Group (SMG) practices, acting as the liaison to their locally assigned clinic(s). Although this position will report to the Population Health Service Organization (PHSO), their responsibilities and day-to-day interactions will be under the guidance of the Office Manager and Practice Leads, supporting the care and coverage needed in the clinic.
Common Duties and Responsibilities:
As part of a larger network, the Care Facilitator will work their colleagues to set a standardized approach to patient outreach through both episodic and longitudinal care management. They will work the Epic Registries to proactively identify high-risk patients with a focus on chronic disease management. Additional strategic priorities include: advanced care planning, self-management training, and behavioral health support.

* Works with office staff as part of an Interdisciplinary Team, develops and implements a plan which addresses problems and needs identified in the assessment and ensures continuity of care by:
* Assisting patient and family with access to appropriate resources and services based on a thorough knowledge of community resources and eligibility requirements
* Advocating on behalf of patients and their families with internal/external resources and payers for needed services, including negotiating with insurance companies for non-covered benefits

Some details about this position:
No Dialysis experience is required.
Training may take place in a facility or a training clinic other than your assigned home clinic
Potential to float to various clinics during and after your training
You must have a flexible schedule and be able to work mornings, evenings, weekends, and holidays
What you can expect:
Direct Patient Care. Spend the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment.
Our PCTs care for multiple patients.
Technician Duties. To monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup. Please note you will have exposure to blood and needles.
Building long-term relationships with your patients who are suffering from end stage renal disease and are receiving treatment on average 3 times/week. You will also be responsible for educating patients on Kidney Dialysis related topics.
Team. Cross functional team of clinicians including technicians, nurses, dietitians, social workers and leadership. PCTs work under the supervision of a RN.

Desire to enter the health care field to care for other people in need
High school diploma or equivalent
Must be comfortable working around blood and needles
Must be comfortable mixing acid or bicarb
Physical and mental ability to work long hours (some shifts are 12+ hours) on your feet so wear comfortable shoes! Our work ends when it's safe for our patients.
Willingness to train and work across multiple clinics within the territory as needed.
Health care experience a plus including: BONENT Certified, Certified Hemodialysis Technician (CHT), Certified Nursing Assistant (CNA), Certified Medical Assistant (CMA), or Phlebotomy certification.
