**Position Purpose:** Supervises the care coordination team that serves long-term care members with physical/medical health needs and/or behavioral/mental health needs to promote quality and efficacy of care management delivery. Supervises day-to-day escalations and care management issues related to members or providers.
+ Monitors and reviews long-term care management required documentation to maintain and ensure compliance with federal and state regulations and contractual agreements
+ Assigns caseloads and work assignments to long-term care management team based on state requirements, care management staff experience, and member needs
+ Works with long-term care senior management on escalated and complex care cases related to physical and/or behavioral health, and provides guidance to junior team members to address member concerns
+ Educates and provides resources for long-term care management team on key initiatives and member outreach to facilitate on-going communication between care management team, members, and providers
+ Evaluates long-term care management team performance and provides feedback regarding performance, goals, and career milestones
+ Provides coaching and guidance to long-term care management team and providers to ensure members are receiving high quality care and information regarding service / care plan options, procedures, referrals, and healthcare benefits
+ Monitors and review reports on a regular basis ensuring quality and productivity metrics are met and for case assignments, and may perform audits of staff on a regular basis
+ May perform telephonic, digital, home and/or other site outreach to assess member needs and collaborate with resources
+ Assists with onboarding, hiring, and training long-term care management team members
+ Leads and champions change within scope of responsibility

**Education/Experience:** Positions overseeing RN team members: Requires Graduate from an Accredited School of Nursing or a Bachelor's degree and 4+ years of related experience Or,
Positions overseeing BH team members: Requires a Master's degree or Graduate from an Accredited School of Nursing and 4+ years of related experience.
Highly advanced clinical knowledge and ability to assess member needs in context of relevant diagnoses, treatment plans and goals, and identify potential gaps in care or risks for readmission or complications preferred.
Strong knowledge of healthcare managed care principles preferred.
Experience working with providers and healthcare teams to develop appropriate long-term care plans/service plans preferred.
Strong knowledge of medication indications and side effects and/or knowledge of BH care management principles preferred preferred.

Pay Range: $85,300.00 - $158,100.00 per year

Under the direction of the Supervisor of Field Service the responsibilities will include managing all image uploads and imaging equipment, perform equipment maintenance and management, assist with specimen collection and Coggins reporting. Clerical and technical tasks; including working in tracking and stocking of the Field Service pharmacy. Loading and unloading trucks as needed for the day.
+ Stock medication and central supplies in the Field Service Pharmacy and monitor instruments and laundry between field service and central supply, label bovine medications, empty bio-hazard container and order/draw up vaccines.
+ Assist clinicians with handling large animals at farm calls .
+ Stock all Field Service trucks daily in accordance with clinician's needs .
+ Assist with specimen submission and Coggins reporting, all information must be verified and added to the GVL database.
+ Manage all imaging equipment uploads from all modalities, for example radiology, ultrasound, gastroscope, etc. Create a RIS order when necessary.
+ Required to work in the Field Service EMR, HVMS. Utilizing HVMS to maintain and create codes to include package creation for all standard clinical procedures and create worksheets.
+ Maintain and manage all equipment to include floats, reamers, henderson etc.
+ Able to create purchase orders in Penn Marketplace, receive and check in these orders.
+ Perform additional duties as assigned
This position is located in Kennett Square, PA, which is approximately 33 miles outside of Philadelphia. No public transportation is available at this location. You must have your own transportation.

General Summary:
The Registered Nurse provides professional nursing care to patients within a assigned unit with a commitment to quality and safety. Professional nursing practices, requiring specialized knowledge judgment and skill are performed using the process in accordance with hospital policy, procedures and nursing philosophy.
Responsibilities:
1. Provide direct, professional nursing care to specified population of patients in the acute care setting.
2. Develops a patient centered plan of care based on individualized needs.
3. Maintains confidentiality of the patient medical record.
4. Counsel and educate patients and their care partner on treatment plans.
5. Administer medication following the five rights of medication administration.
6. Provide wound care and dressing changes, as applicable.
7. Read and interpret patient information, making treatment decisions, when appropriate.
8. Consult with physicians to decide on best treatment decisions when appropriate
9. Performs nursing interventions to achieve optimal patient outcomes.
10. Observe and record patient behavior and response to nursing interventions.
11. Demonstrates quality, safe and effective patient care.
12. Provides delegation to unlicensed assistive staff and Licensed Practical Nurse (LPN) staff
13. Demonstrates standards of excellence which includes practicing nursing with core values of compassion, accountability, respect, teamwork and integrity.
14. Demonstrates understanding and executes quality and performance improvement plans. (only for RN evaluation)
15. Participates in decision making and takes actions based on ethical principles. (only for RN evaluation)
16. Provides timely and accurate documentation of patient care.
17. All other duties as assigned within the scope and range of job responsibilities.
Required Education, Credential(s) and Experience:
- Education: Diploma or Associate Degree in Nursing
- Credential(s): Registered Nurse RN
Basic Life Support BLS
Pediatric Advanced Life Support PALS
*ACLS may be required per manager's direction and timeframe*Commit to seeking specialty certification within time frame as indicated during interview - Experience: Acute care RN experience, ideally in a Pediatric setting, is strongly preferred, but is not required.
Preferred Education, Credential(s) and Experience:
- Education: Bachelor Degree Nursing BSN
- Credential(s): Certification related to the position, if applicable.
- Experience: Several years experience in Pediatrics.

Position OverviewPosition responsible for performing advanced indirect and selected direct patient care duties in support of the nursing process in a competent manner under the direction of an RN and for performing related clerical functions, must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles.
Essential Job Functions- Must be able to demonstrate knowledge and skills necessary to provide care to the patient served
- Delivers appropriate advanced indirect and selected direct patient care at a competent level as directed by a physician or registered nurse within the scope of hospital-approved PCA skills inventory according to the needs/age of the patient/population served; collaborates with other members of the healthcare team to plan, expedite and promote quality patient care; employs critical thinking skills when identifying and responding to abnormal findings, reporting such to the nurse immediately; maintains simple drainage devices; takes patient vital signs, perform blood glucose checks, perform EKGs, monitor intake and output, and maintain related records; perform skin preps and prepares patient for procedures as directed
- Answers call bells, prepares patients for meals, prepares meals and feeds patients; performs/assists with baths; assists patient in mobility; transports patients within the facility and performs assigned treatments, as directed; maintains equipment and patient rooms in orderly fashion; performs housekeeping duties for patient rooms, performs assigned data entry via computer; serves as Unit Clerk and/or monitors patients as assigned
- Ensures safe care to patients adhering to policies/procedures/standards of care, within budgetary specifications, including time management, supply management, and accuracy of practice
- Promotes individual professional growth/development by meeting requirements for mandatory all-staff meetings/continuing education/skills competency; supports department goals which contribute to the success of the organization; serves as preceptor/mentor/resource to less experienced staff
- High School Diploma or GED, required
- Certified Nursing Assistant, preferred
- Current BLS certification, required
- 2 years of work experience in an acute care hospital as a Nursing Assistant or currently enrolled in an accredited nursing program, required (this qualification is preferred for hires in Oklahoma)
- Ability to understand written instructions and perform basic mathematical calculations; demonstrates skills for assigned specialty area/function or assigned procedures
- Demonstrated ability to perform duties with increased responsibilities.
- Ability to perform duties requiring prolonged standing, walking, stooping, bending, and lifting
- Basic knowledge and understanding of PPE protocols and universal precautions
- Position requires fluency in English; written and verbal communication
