**This is a full time, day shift, Clinical Care Coordinator -Stroke Program (RN) position working onsite supporting Northwest Medical Center.**

+ Develops and maintains an organizational structure to facilitate the safe, effective, and efficient delivery of patient care.
+ Prepares weekly and monthly nursing schedules and implements cost-control measures to ensure budgetary compliance.
+ Participates in infection control surveillance, environmental hazard monitoring, and emergency preparedness drills to ensure safety and compliance.
+ Serves as a resource for nursing staff, addressing clinical and operational questions, and providing mentorship and guidance.

**Qualifications**
+ **2-4 years of clinical nursing experience required**
+ **1-2 years of experience as a charge nurse or clinical care coordination required**
+ **Three (3) years experience in clinical setting specific to defined patient population strongly preferred.**
+ **Experience with program development, third party payers and marketing preferred.**

POSITION SUMMARY
This position actively participates on the healthcare team as a pharmacotherapy expert to provide quality patient care in area of specialty. 4. Applies the principles of continuous quality improvement consistent with job expectations. Incorporates quality improvement principles into other activities and facility/system-based projects (i.e. data collection, documentation, med error and adverse drug event trending). Participates in drug use evaluations and facility level medication safety oversight.
5. Participates in local and system-based committees (e.g. area of specialty or other related area) and effectively communicates to stakeholders. Participates in the development of innovative drug-related patient care practices in their specialty at both institutional and system level. Serves as a resource and mentor to other pharmacists, students, residents, and other health-care providers at both the facility and system level.
6. Meets ASHP preceptor qualifications. Participates as primary preceptor to residents where applicable. When needed, serves as the Program Director for the institution's Pharmacy Practice Residency Program. Serves as an educator, resource expert, and mentor to other pharmacists, students, residents, and/or other healthcare providers.
7. Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. MINIMUM QUALIFICATIONS
A Doctorate in Pharmacy degree and at minimum completion of a PGY1 residency program. Without residency training a minimum of 3 years of health-system progressive clinical pharmacy experience (evaluating disease state & medication management e.g. thromboembolic, diabetes, oncology, pharmacokinetics, pain management & nutrition, investigational studies, etc).
Additional minimum qualifications include board certification through the Board of Pharmaceutical Specialties (e.g. BCPS, BCOP). Board certification must be active and maintained.
For pharmacists providing care in Investigational Drug Services, the ASHP Investigational Drug Service Certificate will be allowed. Candidates who are not Board Certified will be given 24 months to achieve certification.
Requires pharmacist licensure in the state of practice and a degree in Pharmacy from an accredited College of Pharmacy. If not licensed in the state of practice, must have completed the Board of Pharmacy application process within 30 days of job offer. Must be licensed within 90 days of hire unless the delay is due to the State Board of Pharmacy. May be required to possess a valid driver's license and be eligible for coverage under the company's auto insurance policy.

**Primary Responsibilities:**
+ Perform clinical documentation review of inpatient and outpatient care delivered to adults and children with Medicaid, Medicare health care benefits through Arizona United Health Care Community Plans for peer review and internal investigations of Quality of Care concerns
+ Create professionally written case investigation summaries for Peer Review, present case summaries at the Provider Advisory Committee as applicable
+ Review plans of correction from providers in response to a substantiated QOC occurrence
+ Effectively interface with external customers, facilities and providers to resolve quality of care concerns, obtain medical records and other information
+ Conduct delegated oversight reviews of contractors that perform work on behalf of UnitedHealthcare. This includes reviewing samples of contractor work against an audit tool or information found in medical charts
+ Collaborate internally with Medical Directors and staff in other UHCCP departments
+ Manage multiple tasks and projects and changing priorities; prioritize work products effectively
+ Maintain timeliness for deliverables and regulator requests
+ Work independently while utilizing good critical thinking skills, as well as excellent verbal and written communication skills
+ Occasional on-site provider visits may be required for potential Quality of Care concerns
