**PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**
**Patient Related**
Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.

+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.
+ Monitors patients performing self-care under the supervision of RN.
**Staff Related**
+ Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.
**Education/Communication:**
+ Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).

**RECORD KEEPING:**
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.

+ Sets and preforms a treatment on all machines used in the transitional care unit.
+ Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.
+ Assists with coordination of transition to patient's modality choice.

  • Monitor patients’ response to dialysis therapy. 

  • Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.   

  • Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure

  • Obtain Hemostasis and apply appropriate dressings.   

  • Evaluate the patient prior to discharge.

  • Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification   

  • Responsible for calculating and entry of individual patients’ dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.   

  • Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.   

  • Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification   

  • Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.   

  • Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.

  • Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.

  • Perform and document any intervention for unusual patient status and document patients' response to intervention.

  • Evaluate patients’ vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.

  • Obtain all prescribed laboratory testing and prepare specimens for collection.   

  • Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.   

  • Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.   

  • Monitors patients performing self-care under the supervision of RN.

Staff Related

  • Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.

Education/Communication:

  • Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices

  • Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.

  • Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.

  • Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).

  • Initiate Solution Delivery System (SDS) system.

RECORD KEEPING:

  • Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.  

  • Enters all treatment data into the designated clinical application in an accurate and timely manner.   

  • Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure

omitted entries are completed or corrected by appropriate staff.

  • Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.

  • Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.   

  • Collect, label, appropriately prepare and store lab samples according required laboratory specifications.   

  • Ensure collection of lab specimens by appropriate lab courier.

   

INFECTION CONTROL:

  • Assists in collecting  information for infection control audits.   

  • Supports staff and patient Adherence to infection control practices.   

  • Follows infection control Policies and Procedures   

  • Participates in and reinforces infection control education of patients and families under the direction of the CM or designee as allowed by state law.

   

TRANSITIONAL CARE UNIT:

  • Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care 

  • Reinforces all education and care related matters as it relates to the new patient     as allowed by state law

  • Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.   

  • Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.

  • Sets and preforms a treatment on all machines used in the transitional care unit.

  • Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.

  • Assists with coordination of transition to patient’s modality choice.   

  • Assists with supply ordering and inventory for TCU

    Responsibilities

    The Home Health Patient Care Manager is responsible for the supervision and coordination of clinical services and provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.

    * Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.

    Maximus is currently hiring for a Mobile Medical Unit Driver to join our Veterans Evaluation Services (VES) team. This is a remote-travel opportunity. The Mobile Medical Unit Driver is responsible for maintaining, stocking, and transporting the mobile unit to scheduled clinic locations safely, on time and in accordance with company policies. Additionally, the driver will function as a Clinic Intake Specialist for the mobile unit appointments. The Mobile Unit Driver is a public-facing role providing punctual, solutions-oriented customer experience to Veterans and VES providers with a caring and positive attitude.

    - Due to contract requirements, only a US Citizen or a Green Card holder can be considered for this opportunity.

    Essential Duties and Responsibilities:
    - Travel 100% of the time to mobile unit event sites; site locations could be in any of the 50 states.
    - Transport the mobile unit to clinic locations safely, on time, and in accordance with applicable traffic laws and company policies.
    - Adhere to DOT hours of service rules and regulations.
    - Assist with the loading, unloading, set-up, and tear down of equipment at mobile sites, including displaying snacks/water/flags/veteran material in the waiting area and post VES signs in the vicinity.
    - Perform or facilitate basic routine maintenance and interior cleaning services for mobile unit and its associated equipment, including inspecting mobile unit before and after each trip, maintaining a log of vehicle mileage and condition, completing regular medical supply inventory, reporting items to be ordered to Clinic Director/Manager to order. and replenishing stock of clinic/medical supplies.
    - Facilitate the operation of the clinic day, including check-in/check-out process and managing sign in sheet, greeting each Veteran, and gathering any medical records or diagnostic results brought in by the Veteran.
    - Provide information to the providers (factsheets, medication list, medical records), and assist with vital signs (weight, height, blood pressure) as needed.
    - Document and update system comments regarding the Veteran appointment.
    - Perform all job functions in compliance with HIPAA policies and adhere to local and externally relevant health and safety laws and policies.
    - Work with Domestic Travel Team on route creation and trip timelines.

    - Must have an excellent driving record without any serious driving violations for the last five (5) years.

    - Due to insurance requirements, driver must be a minimum of 25 years to operate company vehicles

    - Must be willing and able to travel out of state with overnight stays along clinic routes

    - Must be willing and able to work weekends as required

    - Valid driver's license is required

    - Must be willing and able to lift, move or carry materials weighing up to 50lbs

    - Residence near an airport for travel purposes preferred

    - Proficiency with software such as Microsoft Word, Outlook, Adobe Acrobat, & general internet search

    - Keeps current on weather forecast, routing, roadwork, or any other event likely to impact the punctuality or safety of transporting the mobile unit and appropriately reports concerns in a timely way to stakeholders

    - Follows all company policies regarding safety and upkeep of conditions/equipment and adheres to local and externally relevant health and safety laws and policies

    - Facilitates the check-in/check-out process and manages sign in sheet

    - Greets each Veteran immediately and gathers medical records or diagnostic results

    - Documents and updates system comments for services pertaining to Veterans and providers

    - Keeps all Veterans' information private and confidential

    - Communicates in a courteous and respectful manner while assisting in any way needed

    - Communicates and assists other departments in a collaborative effort to expedite cases

    - Adapts to new instructions, requests or procedures maintaining a high sense of urgency
