DUTIES AND RESPONSIBILITIES :
- Assumes a leadership role relative to the clinical aspects of the LHCSA, overseeing and directing all clinical issues, HHA training compliance, and successfully meets all NYSDOH clinical requirements.
- Able to competently address family and patient complaints and issues. Works effectively with referral sources.
- Assumes total clinical responsibility for active census, ensuring all visits are conducted, timely and handling emergencies as needed. Full responsibility for clinical on call.
- Conducts quarterly Professional Advisory Committee meetings creating and trending required outcomes and data.
- Ensures that a complete assessment and coordinated plan of care is developed and adhered to for all patients referred to the LHCSA. Works effectively with referring facilities to insure cohesive transitional care.
- Participates in formulating, evaluating and revising agency policies and procedures.
- Oversees employee inservice and orientation to ensure staff compliance with NYS DOH requirements. Full responsibility for the oversight of the HHATP.
- Supports efforts of office staff to ensure compliance with all NYSDOH requirements i.e., completing aide supervision visits and aide competencies as required, reviewing and approving HHA medicals,etc.
- Assumes a leadership role in Emergency Preparedness for the Agency through the participation in DOH drills and actual activations.
- Successfully meets targeted performance indicators related to the oversight and management of the clinical staff.
- Overall responsibility for the maintenance of EMR patient files; current and discharged.
- Demonstrates an attitude of cooperation and professionalism when dealing with peers and supervisors.
- Performs job function with maximum efficiency and effectiveness.
- Accepts constructive feedback, comprehends overall concepts communicated and makes necessary changes to improve results.
- Consistently performs responsibilities with focus to customer service. Knows who the customer is, addresses issues, and resolves through to full recovery.
- Maintains appropriate level of confidentiality consistent with HIPAA regulations. Adheres to the Compliance policy and procedures.

QUALIFICATIONS:
- A current New York State Nursing license and BSN, or a bachelor’s degree in a related health field or equivalent experience .
- A minimum of four years’ experience is necessary, two of which was in a supervisory capacity overseeing direct nursing services preferred.
- Excellent educator, home health aide assessment and patient assessment skills and the ability to set priorities is needed.

Patient Care Assistant I Responsibilities:
- Performs a variety of technical, para-professional, rehabilitative or therapeutic procedures and activities relating to patient care such as, but not limited to, providing skincare maintenance, performing procedural and diagnostic preps, maintains airway clearance, takes vital signs.
- Prepares patient and equipment for procedure, performs post-mortem care duties, performs phlebotomy services, performs EKGs.
- High school diploma or GED equivalent required.
- Completion of a CNA training class and/or Fundamentals of Nursing in a nursing program preferred.
- 1 year of recent experience as a nursing assistant in an acute care emergency department preferred.
- Certification in phlebotomy, EKG, and BLS preferred.
