As a Clinical Care Navigator, you'll be doing the important, meaningful work of managing crises, providing in-the-moment phone support to clients with complex behavioral health issues, ensuring client safety, and connecting clients to high-quality, evidence-based providers and facilities. This position does not involve providing in-person services or therapy but instead focuses on supporting clients in connecting to care, exclusively by phone and email.

Sunday - Thursday: (9:30am - 6pm PT/ 10:30 - 7pm MT/ 11:30am - 8pm CT / 12:30pm - 9pm EST)
Additionally, we are offering a Monday - Friday split shift (interrupted) option. Details include:
First Shift: 6 am - 10 am PST (7 am - 11 am MT / 8 am - 12 pm CT / 9 am - 1 pm ET) and
Second Shift: 2 pm - 6 pm PST (3 pm - 7 pm MT / 4 pm - 8 pm CT / 5 pm - 9 pm ET); there is a 4-hour break between 10 am PST - 2pm PST.
Weekends include a 10% shift pay differential!
ResponsibilitiesUsing evidence-based interventions, skillfully manage client crises, providing brief risk assessment, safety planning, and de-escalation with the goal of connecting the client to treatment
Identify roadblocks to clinical care options for our clients using Lyra's proprietary software and other methods to connect clients to the ideal treatment, based on specific needs and preferences
Independently manage a caseload of complex, multi-need clients
QualificationsMaster's degree in clinical social work or equivalent ; LCSW or equivalent independent clinical licensure is required (e.g. LMFT, LPC, LPCC, etc) or a Bachelor's degree in nursing (RN). We're unable to provide supervised hours toward licensure. Candidates without a state license to deliver mental health services, independently, will not be considered (with the exception of the RN license)
Must have two years psychiatric intake, assessment RB, other related experience
Must have two years post license experience (EAP/clinical call center experience preferred)
Significant experience managing high-risk clinical situations (e.g., suicidality, mandated reporting)
Bilingual, Spanish-speaking candidates strongly preferred (but not required)

As a full-time Clinical Care Navigator, you will be employed by Lyra Health, Inc. The anticipated base rate for this full-time position is 38.65 per hour. The base rate is determined by role and level, and the base rate will depend on a number of job-related factors, including but not limited to your skills, qualifications, experience and location. As a Clinical Care Navigator on the Organizational Services Team, you'll be doing meaningful, high-impact work supporting companies and their employees through moments of challenge, change, and crisis. You'll be the first clinical point of contact for HR, ER, and Benefits teams reaching out for expert guidance, and you'll coordinate thoughtful, timely support for employees navigating behavioral health concerns. As a Clinical Care Navigator on the Organizational Services Team, you'll be doing meaningful, high-impact work supporting companies and their employees through moments of challenge, change, and crisis. Provide clinical consultation and guidance to leaders, HR, and ER teams on employee behavioral health concerns, workplace dynamics, and appropriate support strategies.
Coordinate and facilitate responses to critical incidents, including identifying appropriate responders, organizing logistics, and conducting follow-up care outreach.
Deliver discreet, high-touch clinical support to executives and VIP employees, ensuring timely and sensitive handling of complex concerns.
Respond to calls and emails from company representatives (HR, ER, benefits leaders) with clinical expertise and Lyra resource guidance.
Reach out to employees identified as high-risk or in crisis, offering in the moment support, risk assessment, safety planning, and care navigation to ensure connection to care.
Partner with internal departments such as Customer Success, Workforce Transformation, operations, and clinical leadership to ensure seamless coordination of services and alignment with contractual commitments.
Maintain accurate, timely, and confidential records of all interactions in compliance with HIPAA and Lyra protocols.
Identify roadblocks to clinical care options for our clients using Lyra's proprietary software and other methods to connect clients to the ideal treatment, based on specific needs and preferences. Independently manage a caseload of complex, multi-need clients.
QualificationsMaster's degree in clinical social work or equivalent; LCSW or equivalent independent clinical licensure is required (e.g. LMFT, LPC, LPCC, etc). We're unable to provide supervised hours toward licensure. Candidates without a state license to deliver mental health services, independently, will not be considered.
Certified Employee Assistance Professional (CEAP) preferred.
Must have two years post license experience (EAP/clinical call center experience preferred or organizational consulting).

+ Partner with, and coordinate with the OCH sales team to understand clinical transition educational needs, and to develop improvements aimed at increasing patient transition volume.
+ Maintains confidentiality of patient and proprietary information and observes legal guidelines for safeguarding the confidentiality of patient and proprietary Option Care information.
+ Provides proper documentation of education utilizing OCH applications and technology.
+ Coordinates the transition of patients from hospital to home or alternate care settings by collaborating with facility staff, providers, and internal teams to ensure timely discharge, therapy education, and continuity of care?
