DUTIES AND RESPONSIBILITIES :

  • Assumes a leadership role relative to the clinical aspects of the LHCSA, overseeing and directing all clinical issues, HHA training compliance, and successfully meets all NYSDOH clinical requirements.
  • Able to competently address family and patient complaints and issues.  Works effectively with referral sources.
  • Assumes total clinical responsibility for active census, ensuring all visits are conducted, timely and handling emergencies as needed. Full responsibility for clinical on call.
  • Conducts quarterly Professional Advisory Committee meetings creating and trending required outcomes and data.
  • Ensures that a complete assessment and coordinated plan of care is developed and adhered to for all patients referred to the LHCSA.      Works effectively with referring facilities to insure cohesive transitional care.
  • Participates in formulating, evaluating and revising agency policies and procedures.
  • Oversees employee inservice and orientation to ensure staff compliance with NYS DOH requirements. Full responsibility for the oversight of the HHATP.
  • Supports efforts of office staff to ensure compliance with all NYSDOH requirements i.e., completing aide supervision visits and aide competencies as required, reviewing and approving HHA medicals,etc.
  • Assumes a leadership role in Emergency Preparedness for the Agency through the participation in DOH drills and actual activations.
  • Successfully meets targeted performance indicators related to the oversight and management of the clinical staff.
  • Overall responsibility for the maintenance of EMR patient files; current and discharged.
  • Demonstrates an attitude of cooperation and professionalism when dealing with peers and supervisors.
  • Performs job function with maximum efficiency and effectiveness.
  • Accepts constructive feedback, comprehends overall concepts communicated and makes necessary changes to improve results.
  • Consistently performs responsibilities with focus to customer service.  Knows who the customer is, addresses issues, and resolves through to full recovery.
  • Maintains appropriate level of confidentiality consistent with HIPAA regulations.  Adheres to the Compliance policy and procedures.
  • Performs related tasks as assigned.

QUALIFICATIONS:

  • A current New York State Nursing license and BSN, or a bachelor’s degree in a related health field or equivalent experience .
  • A minimum of four years’ experience is necessary, two of which was in a supervisory capacity overseeing direct nursing services preferred.
  • Excellent educator, home health aide assessment and patient assessment skills and the ability to set priorities is needed.

+ Draw and process blood, as appropriate, within State and County guidelines.
+ Obtains specimens, such as urine, stool and sputum.
+ Administers enemas, as directed.
+ Weighs and records patient weight on admission and as indicated.
+ Shave patients as instructed in preparation for procedures.
+ Performs simple procedures utilizing aseptic technique (i.e. - applying and changing simple dressings).
+ Performs simple respiratory treatments, as required (i.e. oral suctioning).
1. Performs a variety of related duties:
+ Assists patients in and out of bed, ambulates patients, lifts, turns and positions patients.

**PURPOSE AND SCOPE:**

+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures,
+ Evaluate patient prior to termination of venous access - standing & sitting blood pressure.
+ Obtain Hemostasis and apply appropriate dressings.

+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the nurse supervisor.
+ Observe patient, and conduct machine safety checks according to facility policy. Report any change or unusual findings to the nurse supervisor.

Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies.   • Obtain necessary pre and post treatment vital signs and weight.  Report unusual findings to nurse supervisor.

  • Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
  • Discontinue dialysis treatment according to established procedures,
  • Evaluate patient prior to termination of venous access - standing & sitting blood pressure.
  • Obtain Hemostasis and apply appropriate dressings.  
  • Evaluate the patient for prior to discharge and report any unusual findings to nurse supervisor.
  • Perform and record Pre and Post dialysis evaluation, weight and vital signs with initial identification.
  • Responsible for calculating and entry of individual patients’ dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed.
  • Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits (NVL) 160.
  • Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
  • Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
  • Report any significant information and/or change in patient condition directly to the nurse supervisor. 
  • Observe patient, and conduct machine safety checks according to facility policy.  Report any change or unusual findings to the nurse supervisor.
  • Perform and document any intervention for unusual patient status and document patients' response to intervention.
  • Evaluate patients’ vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the nurse supervisor.
  • Obtain all prescribed laboratory testing and prepare specimens for collection.

MAINTENANCE/TECHNICAL:

  • Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
  • Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
  • Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate.
  • Perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
  • Initiate Solution Delivery System (SDS) system. 
  • Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
  • Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.

CLERICAL & ADMINISTRATIVE:

  • Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities. 
  • Enters all treatment data into the designated clinical application accurately and in a timely manner.
  • Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure omitted entries are completed or corrected where appropriate.

**Description**
The Department of Pediatrics in Stony Brook Children's Hospital seeks faculty for the Division of Pediatric Critical Care Medicine at the Clinical Instructor level.
The Nurse Practitioner/Physician Assistant will join physician faculty of the Division of Pediatric Critical Care Medicine at Stony Brook Children's Hospital to provide direct care to patients in the Pediatric Intensive Care Unit (PICU) in collaboration with an Attending Intensivist. The NP/PA will also be responsible for organizing the Pediatric Special Procedures (PSP) intake and recovery area and managing patients undergoing procedural sedation or general anesthesia and IV infusions of biologics and other medications. The NP/PA will be responsible for the following:
+ Provide coverage with most shifts being weekday plus, on average, one weekend shift and one-week night shift a month.
+ Provide comprehensive care to patients on the Pediatric ICU at Stony Brook Children's in collaboration with an Attending Intensivist.
+ Comprehensive PICU care will include obtaining patient admission medical histories, performing physical examinations, ordering medications, ordering and interpreting laboratory tests and diagnostic procedures, and discharging patients with complete, comprehensive discharge plans.
+ Provide care to patients on the PSP at Stony Brook Children's in collaboration with Attending Physicians including Intensivists, Anesthesiologists, and Pediatric Subspecialists.
+ Comprehensive care of patients in the PSP will include obtaining procedural consent for sedation, prescribing medications for relief of pain and motionless procedures, recovery of patients from sedation and anesthesia, monitoring of patients during intravenous infusions, managing patients during IV infusions and responding to adverse events, and communicating with ordering physicians
+ Other responsibilities will include: providing education to staff PICU and PSP nurses; conferring with patients to obtain accurate information for records, explain procedures, ally fears and elicit cooperation, and engagement in plan of care; ensuring execution of provider established plan of care including but not limited to medicine reconciliations, diagnostic follow up visits and specialist consultations, lab, imaging studies and/or procedures; contributing to patient care through patient and family education, population health management, encouraging patient self-management, distributing resource literature and community service recommendations and referrals as needed; maintaining liaison with physicians and practice administration to ensure efficient and effective patient care.
+ For Nurse Practitioners - Assistance in the annual review Nurse Practitioner protocols.
This is a faculty academic position at the Clinical Instructor level. Individuals will be selected on the basis of their training, clinical experience, research potential, and their commitment to teaching. Faculty appointment will be commensurate with qualifications.
