**The role you'll contribute:**
The Spine Clinical Care Coordinator facilitates an evidence-based, multidisciplinary, multi-specialty approach to care throughout the care continuum. Oversees the development and redesign, as needed, of patient education, of evidence-based standards of clinical care and the post discharge rehabilitation care. Gathers, records, and evaluates program data and uses this data in establishing program goals with the oversight of the Medical Director and the Program Manager. Accountable for the ongoing monitoring of program quality and assists with program marketing needs. The overall goal of the Spine Navigator is to improve organizational clinical/operational outcomes for spine-related conditions and procedures. The Advent Health Neurosciences Institute Spine Center model is structured to achieve established objectives using a team approach supported by a system-wide Spine Center Manager. The Spine Navigator guides interdisciplinary team members in performance improvement and the incorporation of evidence-based practice protocols into the daily operations of the Advent Health Spine Center Program. The Spine Navigator works with community and professional organizations to further community education about spinal care and treatment, and actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Qualifications

**About the Job**
In this role, the Point of Care Coordinator (POCC) provides technical oversight for all Point of Care Testing (POCT) across the region. Physical Requirements and Working Conditions:
Ability to demonstrate a commitment to patient-focused care. Ability to work in a fast-paced environment with established time constraints and in difficult situations. May be exposed to hazardous materials and life-threatening diseases. Ability to perform lifting/transfer activities related to patients (up to 50 lbs. may be required). Ability to work designated shifts including weekends and holidays. Ability to transport objects from one location to another. This position needs to recognize needs and behaviors of specific age groups of patients treated.
If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
