Mercy Hospital's 4M Clinical Decision Unit (CDU) is a 28-bed observation and telemetry unit. This fast-paced unit has minimal bedside procedures and in 2023, the team met their OKR goal of eliminating any central line or foley infections through consistent and thorough nursing care. As a Patient Care Nursing Assistant (PCNA) on this team, you will coordinate with fellow caregivers to create a welcoming, healing and warm environment for patients. This position offers the ability to pursue educational advancement, higher positions and skills development courses offered by Cleveland Clinic. Cleveland Clinic's Integrated Hospital Care Institute is currently seeking a qualified **Acute Care Nurse Practitioner** or **Physician Assistant** to join the **Critical Care** Advanced Practice Team at Mercy Hospital. This role offers a high degree of autonomy while working within a supportive, team-based environment caring for diverse patient populations in a mixed MICU/SICU setting, including Level II trauma patients. Additional service line includes the CCU.
**Role & Responsibilities:**
+ Provide independent, high-level critical care to acutely ill patients.
+ Perform H&Ps from the ED, floor consultations, and daily rounding.
+ Manage procedures including central line placement, arterial lines, and intubations.
+ Write admission orders, coordinate care plans, and engage in patient/family discussions.
+ Collaborate with attending physicians and consulting teams.
+ Respond to hospital-wide emergencies, including rapid responses, sepsis alerts, stroke alerts, and code blues-providing assessment, resuscitation, and appropriate disposition.
+ Participate in 24/7 coverage with in-house attending support during daytime hours and eICU resources available overnight.
**Training & Support**
New APPs receive comprehensive onboarding, structured mentorship, and access to didactic resources to ensure success in providing high-quality patient care within the critical care team.
**Schedule:**
+ 7 days on / 7 days off schedule with 12-hour shifts (7a-7p and 7p-7a).
+ All APPs share in the night rotation.
+ Straight nights option (with shift differential) available if preferred

Provides advanced nursing care to patients with complex acute, critical, and chronic health conditions, including the delivery of acute care services. Practices in any setting in which patients require complex monitoring and therapies, high intensity nursing intervention, or continuous nursing vigilance within the range of high-acuity care.
PA: Nationally certified licensed healthcare provider who provides primary and or specialty medical care in a variety of healthcare settings. Functions with the collaboration agreement of a Physician, (as per O.R.C. 4730). Develops a working diagnosis and treatment plan based on histories and physical exam, providing continuing medical care, including daily examinations, writing orders, chart notes. PA's are privileged by the Board of Governors. PA's in this role provide advanced care to patients within any setting in which patients have complex, acute, critical and chronic health conditions. Practices in any setting in which patients require complex monitoring and therapies and high intensity medical interventions and/or continuous vigilance within the range of high-acuity care.
**Job Responsibilities:**
+ Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients and hospital admissions to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan.
+ Must be able to work with collaborating physician.
+ Performs and interprets, at least to the point of recognizing deviations from normal, common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as privileged by CCF Credentialing Board
+ Autonomously takes initiative responding to and taking the lead in rapid response, codes and stroke alerts, admissions to the ICU, as well as performing specialized procedures , such as, blood draws, IV starts, intubation, arterial line placement, central venous catheter placement , ventilator management, in addition to other specific procedures as necessitated by the patient situation. Must be prepared to function independently in house as the sole provider in hospital.
+ Provides information and answers routine questions of patients and families regarding their disease, including preventive health maintenance topics such as diet, weight, smoking and topics related to medical specialty, with basic recommendations.
+ Educates patients regarding their disease, treatments, related drug and treatment side effects and hazards.
+ May participate in medical team rounds; collaborates with nursing, medical and other healthcare team members.
+ Refers patients to specialists as appropriate for consultation or for specialized health resources and treatment.
+ Ensures continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary.
+ May serve as clinical preceptor for PA/APRN students and other health professionals.

**Education:**
+ **APRN: EDUCATION** : Graduate of an accredited MSN/ACNP/AG-ACNP training program.
+ **PA: EDUCATION** : Graduate of an A.R.C.-approved Physician Assistant Program.
+ Master's degree in Physician Assistant Studies and/or applicable state license through the
+ State Medical Board for certificate to practice.
**Certifications:**
+ **APRN:** Currently licensed as a Registered Nurse and Advanced Practice Registered Nurse in applicable state.
+ National certification as Advanced Practice Registered Nurse by nationally accredited organization recognized by state Board of Nursing, current licensure by applicable state Board of Nursing, (including certificate to prescribe if applicable).

Mercy Hospital's 7M Medical Telemetry Unit cares for a diverse population of patients ranging from ages 18 to 100+ years old. This unit cares for patients who have peg tubes, foley catheters, central lines, and tracheostomies, are post-heart catheterization, require wound care, have drug and alcohol addictions, or have mental illnesses and are waiting for placement at a psychiatric facility. Union Hospital's Stepdown Unit provides an intermediate level of care between the Intensive Care Unit (ICU) and General Med/Surg Units. **A caregiver in this position works a day/night rotation from 7:00AM - 7:30PM or 7:00PM - 7:30AM, including rotating weekends and holidays.**
A caregiver who excels in this role will:
+ Provide or assist with patient care under the supervision of an RN. Patient care may include vital signs, weight, assisting patients with personal care and activities of daily living, patient transfers/transport, pulse oximetry, point of care testing, dressing changes, applying external monitoring devices and removing urinary Foley catheter.
+ Report abnormal findings or changes in physical, mental and emotional conditions to an RN.
+ Assist with keeping patients' rooms clean and orderly.
Minimum qualifications for the ideal future caregiver include:
+ High School Diploma or GED
+ Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross (If you do not hold this certification, you must obtain it during your new hire period)
+ **Nursing students:** Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross prior to hire
+ Military experience as a Hospital Corpsman (HM), Combat Medic (68W) or Medical Service Technician (4N0X1) will be considered
Preferred qualifications for the ideal future caregiver include:
+ State Tested Nursing Assistant (STNA)

Mercy Hospital's 8M Medical Oncology & Hospice Unit focuses on meeting a patient's physical, psychological, social and spiritual needs to improve their quality of life and support their loved ones. 8M caregivers provide care to oncology and hospice patients with cancer diagnoses, chest pain, shortness of breath, COPD, and abdominal pain. Additionally, this unit provides patients with end-of-life care.
**A caregiver in this position works rotating days/evenings rotation from 6:30AM - 3:00PM or 2:30PM - 11:00PM.**

Mercy Hospital's 7M Medical Telemetry Unit cares for a diverse population of patients ranging from ages 18 to 100+ years old. This unit cares for patients who have peg tubes, foley catheters, central lines, and tracheostomies, are post-heart catheterization, require wound care, have drug and alcohol addictions, or have mental illnesses and are waiting for placement at a psychiatric facility. Minimal bedside procedures are done on this unit, but caregivers occasionally see fluid aspiration from joints and incision/drainage of small skin abscesses.
