The role you'll contribute:
The Patient Care Advocate position is an outreach position for AdventHealth Medical Group to assess patients who need care for a variety of medical conditions that are identified by data analysis. Chart reviews will be undertaken to determine if patient meets criteria for outreach. Patients will be contacted and appropriate appointments made, lab or imaging studies ordered to facilitate the care of the patient. Contact with patient's primary care provider and staff will be required to ensure coordination of care. The Patient Care Advocate position will coordinate, assist, and implement quality improvement initiatives within Physician Enterprise. This may involve researching data, developing project plans, monitoring quality improvement activities, individual case review, serving as a resource and providing education to team members, providers, and practitioners. The clinical Patient Care Advocate will be involved in educating clinic staff and providers on Quality Measures and processes for satisfying the measures and provide support for ongoing development and maintenance of standards for performance and clinical outcomes. This role will help coordinate and implement a clinical competency program for all "non-provider" clinical staff in the ambulatory clinic settings.

+ Communicates effectively and courteously on the telephone and in person with patients and colleagues. Incorporates excellent verbal, written and listening communication skills; and critical analysis skills.
+ Fulfills responsibilities such as: checking on patients and making sure they come to appointments and follow-ups.
+ Possess a strong attention to detail, a personal commitment to accuracy and excellence, excellent communication and interpersonal skills and demonstrated organizational skills.

Minimum qualifications:
+ 3 years clinical experience
+ 3 years EMR
+ Licensed Practical Nurse - State Licensure

**Overview**
We're looking for a **Patient Care Manager-Registered Nurse** to join our team.You will report directly to the Executive Director, Administrator, or Senior Patient Care Manager. You will be responsible for maintaining a high performing hospice program including managing, overseeing, and directing patient care activities to ensure the delivery of quality patient care. + Preparing for, serving on, and facilitating the Interdisciplinary Group (IDG), as assigned, including presiding over IDG, presentation of patients for review, coordination of minutes, review of patient charts to note and address any changes to patient condition and/or the plan of care, and soliciting input from various care team members
**About You**
+ At least three years' experience of direct patient care and two years' experience in hospice or home-based care setting
+ Graduate of accredited school of nursing; bachelor's degree in nursing preferred
+ Current Registered Nurse licensure in state of residence and requested employment
+ Demonstrates knowledge and compliance with accepted hospice principles and practice, including industry standards, regulations, and best practices (i.e., Medicare, Medicaid, JCAHO, ACHC), company policies/procedures, and understanding ofterminally ill patients and their families
+ Ability to apply knowledge of the unique needs of hospice patient and families from various socioeconomic backgrounds to provide appropriate advocacy and oversight
