Pharmacy Consultant, Clinical Services

The Pharmacy Consultant, Clinical Services, is responsible for building and maintaining relationships internally and externally with customers and related partnership groups and organizations. In addition, the Pharmacy Consultant provides consultation, coaching, training, and education to customers who partner in clinical program execution, helping to ensure greater adoption and utilization, success, and outcomes for their patients and pharmacies. The Pharmacy Consultant will report to the Vice President of Clinical Services and work cross-functionally with Sales, Operations, Marketing, Product Development, and Training.

Key Duties

  • Actively participate in national and regional meetings of professional and industry pharmacy organizations, communicating issues, trends, and the future vision of the profession to the appropriate members of the RedSail Technologies team.
  • Liaison and contact for pharmacy organization and industry meetings.
  • Serve as a pharmacy and clinical services expert/liaison and articulate the company's solutions, services, and program offerings to customers.
  • Provide knowledge and expertise on pharmacy industry best practices, workflow, and service offerings.
  • Provide Demonstrations of solutions and workflow processes to customers and partners
  • Provide RedSail's software and product development teams with clinical input into the development and enhancement of features/functionality
  • Continuous Improvement efforts for both customers and internal teams
  • Lead Customer Coaching and Consultative Clinical Calls
  • Mentoring and Leadership for pharmacy-related interns/residency programs
  • Assist in training and development (internally and externally)
  • Identify opportunities for workflow and software utilization improvement and provide insights to customers.
  • Serve as a source of industry and professional information and offer guidance as requested to the executive committee, product management teams, sales, account managers, and others.
  • Serve as a communication resource when the peer-to-peer relationship is critical to customers.
  • Maintain open communication regarding industry issues, news, and trends with peers across business units of RedSail Technologies.
  • Works closely with the Marketing & Sales Department to ensure appropriate communication with RedSail Technology customers.
  • Attends and participates in relevant industry events and conferences to provide sales support, discover market trends, and be available to existing customers.

  • Graduate of Accredited College of Pharmacy
  • Pharmacist License in good standing
  • Minimum 2 years of applicable experience
  • Experience in both community/retail and long-term care pharmacy preferred

**PURPOSE AND SCOPE:**

Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. **DUTIES / ACTIVITIES:**

+ Welcome assigned patients and inquire as to their well-being since their last treatment. Report any complaints or observations to the nurse supervisor.
+ Evaluate vascular access for patency and report any unusual findings to nurse supervisor.
+ Obtain necessary pre and post treatment vital signs and weight. Report unusual findings to nurse supervisor.

+ Perform vascular access cannulation and report any difficulties in cannulation or unusual findings to nurse supervisor.
+ Perform administration of Heparin as delegated or as allowed by state law.
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy and report any unusual findings to nurse supervisor.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures,

  • Evaluate patients’ vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.

  • Obtain all prescribed laboratory testing and prepare specimens for collection.   

  • Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.   

  • Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.   

  • Monitors patients performing self-care under the supervision of RN.

Staff Related

  • Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.

Education/Communication:

  • Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices

  • Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.

  • Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.

  • Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).

  • Initiate Solution Delivery System (SDS) system.

RECORD KEEPING:

  • Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.  

  • Enters all treatment data into the designated clinical application in an accurate and timely manner.   

  • Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure

omitted entries are completed or corrected by appropriate staff.

  • Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.

  • Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.   

  • Collect, label, appropriately prepare and store lab samples according required laboratory specifications.   

  • Ensure collection of lab specimens by appropriate lab courier.

   

INFECTION CONTROL:

  • Assists in collecting  information for infection control audits.   

  • Supports staff and patient Adherence to infection control practices.   

  • Follows infection control Policies and Procedures   

  • Participates in and reinforces infection control education of patients and families under the direction of the CM or designee as allowed by state law.

   

TRANSITIONAL CARE UNIT:

  • Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care 

  • Reinforces all education and care related matters as it relates to the new patient     as allowed by state law

  • Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.   

  • Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.

  • Sets and preforms a treatment on all machines used in the transitional care unit.

  • Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.

  • Assists with coordination of transition to patient’s modality choice.   

  • Assists with supply ordering and inventory for TCU

    GENERAL OVERVIEW:

    This job provides direct care to patients and assists with non-direct patient care unit coordination. The incumbent in this position is expected to perform basic nursing procedures related to care and comfort of patient's in stable condition, and have knowledge of patient population needs in the assigned work area. In addition, the incumbent is expected to assist the RN in the care of acutely ill patients. He/she is expected to practice medical asepsis, respect the dignity and privacy with every patient and family, and to maintain objectivity during patient care. He/she is expected to know and adhere to all basic hospital policies and procedures and respond appropriately during emergency situations. He/she is expected to maintain the patient/family environment and perform general unit upkeep. He/she is expected to use accurate verbal and written communication, and to document patient care appropriately in the electronic medical record. The incumbent works under the general supervision of the Registered Nurse for patients in stable condition. Will work under the direct supervision of the Registered Nurse in the care of the acutely ill and/or complex patient. The incumbent receives specific work assignments from the Charge Nurse and/or Care Team Leader which are both written & verbal and is expected to demonstrate evidence of efficient time management by completing the work within the scheduled time. In addition to clinical responsibilities, the incumbent is expected to perform a variety of duties related to the coordination of activities on the inpatient unit. This includes activities related to patient admissions, discharges, and transfers.

    ESSENTIAL RESPONSIBILITIES

    • Provides, obtains or performs and documents direct patient care and activities such as: activities of daily livings, encouraging and observing patient on incentive spirometer and/or cough and deep breath, skin survey, vital signs, specimen collection, phlebotomy, intake and output, meal consumption, height and weight, 12 lead EKG testing, blood glucose testing/monitoring, point of care testing, bladder scanning, transporting of patient and their medical record, documentation of O2, early mobility/ambulation protocols, clean catheterization technique (per hospital protocol), simple dressing change, dry dressing change, removal of Indwelling urinary catheter, perform straight catheterizations, catheter, collection of urine specimen from Foley catheter (per hospital protocol),IV access insertions and removal with documentation, assistance with exams, treatments and procedures. Assist in cardiac monitoring and transporting of low-level monitoring patients. (50%)

    • Participates in quality and patient satisfaction initiatives which may include bedside shift report, purposeful hourly rounding promoting safety such as falls prevention, hand washing, isolation protocols and maintaining a clean and orderly environment, precepts newly hired PCTs. (20%)

    • Accurately documents vital signs, weights, I&O, restraints, specimen collection, EKG completion and all other treatments. Report changes in patient's physical, mental and emotional conditions, as well as any issues or concerns identified to nursing staff. (15%)

    • Participates in unit activity such as supply stocking and equipment/appliance cleaning, checks and retrieval. (15%)

    • Performs other duties as assigned or required.

    QUALIFICATIONS:

    Minimum

    • High School Diploma or GED

    • Experience as a PCA, PCT, NA, LPN, EMT, Military experience as a Hospital Corpsman (HM), Combat medic (68W), or Medical Service Technician (4N0X1)

    • 1 year in an acute or long-term care facility OR enrolled in an Associate's in Nursing (halfway through program) or Bachelor's in Nursing (Junior year) program

    • Basic computer skills

    • Proficient in: IV insertion, phlebotomy, straight catheterization, and basic cardiac dysrhythmia training or successful completion of a cardiac dysrhythmia training course upon hire.
