**JOB PURPOSE:**
Responsible for assisting the patient and patient caregiver with personal hygiene needs to increase the level of comfort of the patient.
**KEY RESPONSIBILITIES:**
1. Provides optimal comfort care, dignity and privacy for the patient.
2. Follows procedures for handling medical emergencies or other incidents that affect the delivery of service in accordance with the Care Plan.
3. Reports patient progress and problems to supervisory personnel.
4. Assists with home management to include light housekeeping, laundry and changing linens.
5. Maintains a safe, clean and comfortable environment for the patient.
6. Assists with meal preparation and related tasks as indicated by the Care Plan.
7. Assists with patient transfer, ambulation and exercise of patients as directed by the Care Plan.
8. Provides personal hygiene to patients including bathing, shaving, oral hygiene, skin care, dressing, toileting, and hair care.
9. Provides patient care services in accordance with PruittHealth Family First policies and
procedures and as directed by the Care Plan.
10. Coordinates care with the Registered Nurse (RN) and/or Executive Director.
11. Maintains accurate and current records of visits.
**KNOWLEDGE, SKILLS, ABILITIES:**
- Attends a minimum of 8 hours of annual in-service education and provide documentation to
the agency.
- Attends team meetings and staff development in-services.
- Communicates well with patients/clients and family members providing warm and friendly greeting and an approachable attitude to families, visitors, patients/residents/clients and responds to expressed concerns while displaying a helpful, caring demeanor. Answers questions when appropriate in a professional manner.
- Able to respond to change productively and handle additional projects as assigned.
- Participates in center/agency surveys (Licensure/JCAHO) and any subsequently required reports.
- Attends and participates in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification as required.
- Attends and participates in mandatory in-services.
- Honors patients/residents' rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.
- Complies with corporate compliance program.
- Reports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.
- Follows established safety regulations, to include fire protection & prevention, smoking regulations, infection control, etc.
- Follows established safety procedures when performing tasks and/or working with equipment.
**MINIMUM EDUCATION REQUIRED:**
Certificate of completion of a state recognized Home Care Aide Training Program (20 hours) and supervised clinical practice (20 hours). Successful completion of nurse aide credentialing or competency evaluation recognized by the state of Georgia is also acceptable.
**MINIMUM EXPERIENCE REQUIRED:**
One (1) years of experience working with adults in a healthcare setting.
**MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:**
Proof of basic first aid during training and current CPR certification. Valid driver's license and automobile liability insurance.

+ Performs assigned patient care and observation duties under the direction of a licensed provider, RN, or LPN.

Are you an ATL Metro Area Critical Care Advanced Practice Provider seeking a new rewarding position! Palm Health Resources is hiring an experienced Critical Care Nurse Practitioner or Physician Assistan t for our ICU! Unique every other week schedule, please see below!

The Critical Care Nurse Practitioner : Works under the direct supervision of an Intensivist and is responsible for providing clinical care for oncology patients and coordination of services provided by nurse practitioners in his/her section of their department.

Schedule:

This is a Full-time Nights position where you will work 12-hr shifts Tuesday - Sunday every other week.

Six Nights shifts required while the seventh night (Monday) is an optional overt time shift available for call but is not mandatory.

Overall responsibilities include but are not limited to :

Provides appropriate patient care for patients with hematologic and oncologic malignancies in the ICU or step-down unit to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, discharge planning and providing accurate medical record documentation.

Educate patients in areas of procedures, pharmacology, self-management of side effects and available community resources.

Provides symptom management for treatment related toxicities.

Facilitates communication regarding goals of care including end of life decision making.

Collaborates and effectively communicates with referring physicians/clinicians and stakeholders.

Complying with all hospital by-laws, rules, and regulations; has all entries in the medical record co-signed by the attending physician

Prescribes Category III and above drugs as indicated by state/federal licensing, provides orders under supervision of physician during assigned on call duty.

Performs rounds in Infusion Center or Inpatient Unit and writes progress notes as appropriate or instructed by MD and manager.

Orders medication and develops treatment and evaluations with supervising physicians, as well as acts as liaison between patients, RNs, and physicians

Minimum Requirements

Masters degree in nursing, specialty in related field

Current Georgia RN license and Georgia Advance Practice-NP licenses required.

National Board certification by the American Nursing Credentialing Center (ANCC), or the American Academy of Nurse Practitioners (AANP)

ACLS (If not AHA must obtain AHA certification within 30 days of hire)

BLS (If not AHA must obtain AHA certification within 30 days of hire)

ONS certification (optional)

AOCNP (optional)


Minimum Experience Required

Strong Medical skills within critical situations (triage)

Perform dutiesand manages time and tasks to enable completion of daily work duties

Discerns when the situation necessitates advice and obtains information from Manager

Prioritizes goals and manages time and strong attention to details

Responsibilities

In this role, you will provide vital support to ourresidents, helping them maintain their independence while ensuring their safetyand well-being. Your contributions will be essential in creating a nurturingenvironment where seniors can thrive.

  • Assist residents with activities of daily living such as grooming, dressing, and continence program as needed
  • Provide companionship and emotional support to enhance residents' quality of life.
  • Help coordinate activities that promote social engagement among residents.
  • Observe and report any changes in residents' conditions to the healthcare team.
  • Support residents with mobility needs, including lifting or transferring as necessary.

Requirements

  • Experience in caregiving or personal assistance is preferred but not required
  • Strong interpersonal skills with a compassionate approach to senior care.
  • Ability to observe and monitor resident conditions effectively.
  • Knowledge of dementia care practices is a plus.
  • Basic understanding of vital sign monitoring is beneficial but not required.
  • CPR and First Aide Certified
