**Position Summary:**
AllCare Plus Pharmacy, an IQVIA company, is seeking a Care Manager! Responsibilities will vary by program and its lifecycle. Care Manager's may be responsible for contacting insurance companies to obtain correct eligibility information, perform benefit investigations, copay assistance and check prior authorization and/or appeal status. Care Managers may also be responsible for directly contacting patients and/or providers to evaluate eligibility for assistance programs and/or varied adherence support. This is a **remote** position.
**Hours:** Must be available for an 8 hour shift between 8am-8pm EST
**Salary:** $23/hr

**Primary Responsibilities:**
+ Perform outbound calls to obtain appropriate information and document accurately
+ Responsible for answering in-bound calls and assisting customers with pharmacy related services
+ Maintain strict professionalism in all communication methods while providing efficient, courteous, and friendly service
+ Contact insurance companies for benefit investigation and coverage eligibility
+ Provide prior authorizations and appeals support
+ Assist patients with the enrollment process for manufacturer and non-profit organization copay assistance programs
+ Update job knowledge by participating in educational opportunities and training activities. Work efficiently both individually and within a team to accomplish required tasks
+ Maintain and improve quality results by adhering to standards and guidelines by meeting quality standards set forth by program KPI's
+ Report ADE's according to program policy and guidelines Adhere to all HIPAA guidelines May assist with onboarding new employees
**Required Qualifications:**
+ To be eligible for this position, you must reside in the same country where the job is located.
+ High School Diploma or equivalent, some college preferred
+ Minimum one year experience in medical billing, reimbursement, insurance verification, or similar related medical office experience
+ Previous data entry experience (minimum three months) and ability to type 30wpm+ Able to demonstrate high attention to detail in work
+ Must be computer savvy, to include navigating multiple computer tabs, monitors and applications
+ Advanced ability/knowledge of all Microsoft Suite programs (Teams, Word, Excel, Outlook, etc) and soft phone systems (WebEx, Mitel, Shoretel, etc.)
+ Exceptional communication skills, both written and verbal

Job Summary:

  • Collects urine or stool specimens as directed.
  • Gives enemas as directed.
  • Keeps patient unit/halls clean and neat.
  • Keeps bedside tables, chest of drawers, and closets clean, neat and organized. (Including clothing)
  • Cares for and cleans nursing equipment. (BP equipment, thermometers, etc.)
  • Follows restraint policy, including release.
  • Assists with post-mortem care when needed.
  • Assures residents have fresh ice water by refilling pitchers with ice and/or water at least once every shift.
  • Cleans water pitchers, cups, bedpans, urinals per schedules.
  • Rinses personal clothing, sheets, etc. if soiled before placing in appropriate laundry barrel.
  • Assists residents with bedpan or urinal.
  • Empties catheter bags at end of shift and records output on appropriate record.
  • Assists with rehabilitation programs such as bowel and bladder retraining, grooming, etc. and always charting on Rehabilitation sheet every shift.
  • Assists charge nurse with resident needs and treatments as directed.
  • Keeps a clean towel and a clean washcloth for each patient in bathrooms as all times. (Replaces immediately when used.).
  • Empties incontinent pad barrel and linen containers every shift and replaces plastic bags in containers.
  • Reports at end of shift to oncoming CNAs.
  • Understands and agree to follow policy of no transfers until Certified. Agrees to obtain assistance from a CNA assigned to assist the Care assistant with tasks they are not comfortable or trained to perform and with ALL transfers.

Job Requirements:

  • Must be 18 years of age or older and have a high school diploma or equivalent.
  • Must be a graduate of a NA1 training program. Must have passed the program and must pass state test within 4 months from completion.
  • Must be willing to care for geriatric patients and have a genuine concern for their welfare.
  • Must be willing to receive, understand, and follow orders.
  • Must be willing to work and cooperate well with other nursing staff, other departments, and families and visitors.

Provide primary-care services in accordance with Eventus WholeHealth site-specific protocols and consistent with the standard of care for the specialty of family medicine, geriatric medicine, and/or internal medicine as it pertains to long term care facility work.

Quality/Compliance
  • Review and sign MAR's as requested by facilities, sign all orders specific for Primary Care while at the facility and/or within 24 hours if faxed or emailed to include phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders as appropriate, and other orders as is relevant and appropriate to the facility patient care needs and Eventus WholeHealth. Review and address labs within 24 hours of them being sent to you. Manage all narcotic prescriptions specific to your facility's patients and as per Eventus WholeHealth Narcotic Rx Policy. Provide facility with adequate time each visit to allow for paperwork administrative duties.
  • Complete documentation and submit billing records to Eventus WholeHealth according to the published policies on charting and bill sheet submission and provide the facility with a hard copy of the electronic documentation within 48 hours of service for inclusion in the patient's facility medical record.
  • Establish and maintain open and positive communications with facility staff and administration. Provide verbal and/or written instruction or feedback regarding medications and other pertinent caregiver information. Meet with Administrator or designated facility care coordinator equivalent prior to the start of your patient care visits each day and prior to leaving facility each day.
  • Be knowledgeable of and adhere to Eventus WholeHealth standards, policies and procedures. Be aware of and adhere to all legal and regulatory agencies' rules and guidelines and professional ethical standards. Comply with all regulatory agencies governing health care delivery.
  • Maintain patient confidentiality at all times including appropriate use of Cell phone, emails text messaging, patient charts and EMR. Agrees to abide by and be knowledgeable of HIPAA rules and regulations
  • Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding the care of a resident.
Qualifications/Education:

The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The APC within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team.

**Primary Responsibilities:**
+ Conduct comprehensive assessments
+ Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
+ Manage members during a short term sub-acute rehabilitation episode
+ Ensure accurate and complete documentation, including ICD 10 conditions
+ Communicate and collaborate with the interdisciplinary care team
+ Conduct advanced illness and advanced care planning conversations
+ Provide patients and caregivers with counseling and education

**Required Qualifications:**
+ Education:
+ NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
+ PA: Graduate of an accredited Physician Assistant degree program
+ Active and unrestricted NP or PA license in the state which you reside, or ability to obtain by start date
+ Active Nurse Practitioner or Physician Assistant certification through a national board:
+ NP: Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:
+ Family Nurse Practitioner
+ Adult Nurse Practitioner
+ Gerontology Nurse Practitioner
+ Adult-Gerontology Acute Care Nurse Practitioner
+ PA: Board certified by the National Commission on Certification of Physician Assistants
+ Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
+ Ability to gain a collaborative practice agreement, if applicable in your state
+ Ability to complete physical requirements of the job including moving a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, remain in a stationary position and position self to perform physical assessment
+ Driver's license and access to reliable transportation
**Preferred Qualifications:**
+ 1+ years of clinical experience in practice (long-term care setting preferred)
+ Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
+ Understanding of Geriatrics and Chronic Illness
+ Understanding of Advanced Illness and end of life discussions
+ Proficient computer skills including the ability to document medical information with written and electronic medical records
***PLEASE NOTE*** Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.

Summary:
The Certified Medical Assistant (CMA) interviews and initiates care for patients who visit our clinics and plays key roles in the care provided, up to and including specimen collection and delivery of test results.
Responsibilities:
+ Interviews patients to collect and document history of present illness (HPI), medication reconciliation, history of allergies.
+ Performs vital sign measurement and any standard testing per clinic procedures
+ Coordinates and performs follow-on procedures as directed by a provider, including but not limited to onsite testing, specimen collection, and wound care.
+ Assists with provider-performed procedures such as suture application or splints
+ Communicates with patients in a timely and effective manner regarding their care. Notifies patients of test and lab results. Patiently and clearly explains practice policies and procedures using cultural competency and active listening.
+ Documents care into electronic medical recordkeeping system (EMR). Ensures completion of necessary documentation and forms.
+ Collects and prepares testing specimens as needed for transport to offsite laboratory.
+ Maintains and monitors consumable supplies and medications for effective ongoing clinical operations.
+ Promotes a safe, clean and secure clinical environment for all, including the application of standard precautions and principles of infection control.

+ Registered Medical Assistant (RMA) as issued by the American Medical Technologists (AMT)
+ Medical Assistant (NCMA) as issued by the National Center for Competency Testing (NCCT)
+ Certified Medical Assistant (CMA) as issued by the American Registry of Medical Assistants (ARMA)
+ Must hold current Basic Life Support (BLS) certification
+ Position will require the ability to lift supplies and equipment regularly and navigate clinical area actively throughout work.
**Preferred:**
+ Prior work experience in a clinical setting.
