This is an academic position with an academic faculty appointment at Case Western Reserve University (CWRU) School of Medicine. In addition to clinical efforts, candidates will have protected time for teaching and research.**
**The HFCC at MetroHealth is run by a collaborative team of providers including HF-trained, board-certified, cardiologists, HF trained nurse practitioners, HF-certified nurses and nurse coordinator, and a clinical pharmacist. This team has established multiple clinical programs for the management of patients with various types of HF, pulmonary arterial hypertension, cardiac amyloidosis, cardio-oncology, muscular dystrophy. The HFCC also works collaboratively with the heart transplantation centers in the region to identify and evaluate patients who may be candidates for advanced HF therapy, and to comanage patients after receiving left ventricular assist devices through the Shared Care LVAD program. The HFCC is an active participant in several multicenter randomized clinical trials. HFCC has been the recipient of multiple get-with-the-guidelines (GWTG) Gold-plus certification for implementing guideline-directed medical therapy for patients with HF.**

We are seeking candidates interested in creating a program for heart failure at three Cleveland Clinic Regional Hospitals - Cleveland Clinic Akron Hospital, Cleveland Clinic Mercy Hospital and Cleveland Clinic Hillcrest Hospital. The focus of the position is three-fold: (1) creation of systems of care for high quality and efficient hospital care of heart failure patients, (2) population health management of outpatients with heart failure, and (3) appropriate identification and follow-up of patients needing advanced procedural treatments. Multidisciplinary coordination of care with primary care, cardiovascular specialists, and procedural teams will be a core component of the position. Electronic medical record population health analytics will also be an important aspect of the program.
Cleveland Clinic Akron, Cleveland Clinic Mercy Canton and Cleveland Clinic Hillcrest Hospital are premier cardiovascular programs with high patient volumes and full cardiovascular services included interventional cardiology, electrophysiology, and cardiac surgery. The position will also coordinate care closely with Cleveland Clinic Main Campus which can provide patients access to mechanical support options and cardiac transplantation. Opportunities to round on Main Campus advanced heart failure services can be included in the position.
Candidates should be BC/BE in Cardiovascular Medicine. Advanced subspecialty training in heart failure is strongly encouraged. A faculty appointment at a rank commensurate with experience is available at the Cleveland Clinic Lerner College of Medicine of Case Western Reserve University.

  • 1. STANDARDS OF PRACTICE RN practice is guided by the ANA Scope and Standards of Practice (2015). The Standards of Practice describe a competent level of nursing care as demonstrated by utilizing the nursing process components of assessment, diagnosis, outcomes identification, planning, implementation and evaluation. Assessment: The registered nurse collects pertinent data and information relative to the healthcare consumer's health or situation Diagnosis: The RN analyzes the assessment data to determine the actual and potential diagnoses or the issues. Outcomes Identification: The RN identifies expected outcomes for a plan individualized to the healthcare consumer or the situation. Planning: The RN develops and modifies a plan of care that prescribes strategies and alternatives to attain expected outcomes. Implementation: The RN implements the identified plan. Coordination of Care: The RN coordinates care delivery and transitions in care. Health Teaching and Health Promotion: The RN employs strategies to promote health and a safe environment. Evaluation: The RN evaluates progress toward attainment of outcomes.

STANDARDS OF PROFESSIONAL PERFORMANCE The ANA Standards of Professional Performance (2015) describe a competent level of behavior in the professional role, including activities related to: ethics, culturally congruent practice, communication, collaboration, leadership, education, evidence-based practice and research, quality of practice professional practice evaluation, resource utilization and environmental health. The RN is expected to engage in professional role activities, including leadership, appropriate to their education and position. The RN is accountable to for their professional actions to themselves, their healthcare consumers, their peers and ultimately to society. Ethics: The RN practices ethically. Culturally congruent practice: The RN practices in a manner that is congruent with cultural diversity and inclusion principles. Communication: The RN communicates effectively in all areas of practice Collaboration: The RN collaborates with healthcare consumer and other key stakeholders in the conduct of nursing practice. Qualifications

EDUCATION

Graduation from an accredited school of nursing. BSN preferred. All newly hired registered nurses must have current BCLS certification or must obtain within one month of hire.

LICENSURE
