**Job Summary and Qualifications**
This position supports the mission of Presbyterian/St Luke's Medical Center of optimizing quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. Within scope, the nurse navigator oversees and directs the care of the BMT or Hematology-Oncology patient and is responsible for collaboration with all disciplines to provide well-coordinated patient care. This position functions to navigate blood cancer patients through the continuum of care including diagnosis, treatment, follow-up and long-term care needs. This includes coordination of chemotherapy orders, prescriptions and individualized follow-up. The hematology nurse coordinator demonstrates competence in the ability to triage patients over the phone and in person. The hematology nurse coordinator demonstrates the ability to interpret laboratory and radiologic test results and communicates results to physician as necessary. They provide patient and family education relative to the patient's diagnosis and treatment process. The hematology nurse coordinator demonstrates collaboration, professionalism, and effective communication in all interactions with physicians, the administrative team, PSLMC employees, patients, caregivers and referral sources. The Hematology / Oncologist will be responsible for managing the care and treatment of cancer patients. The Medical Oncologist/Hematologist will work closely with center team members to implement treatment plans and monitor progress.

The primary function is to establish a relationship with patients, family members/caregivers, and physicians, and facilitate effective communication with all team members. The Hematology Nurse Coordinator is responsible for coordinating the plan of care which includes, appointments, patient and family/caregiver education, provision and/or enablement of support services, and follow-up. S/he will also serve as a liaison between the patient and family/caregiver, the primary care physician, internal and external care providers, specialists, referring providers, supportive care members (e.g., social workers, hospital/clinic staff, and the wider healthcare community). The Hematology Nurse Coordinator will also assume responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes (e.g., the Oncology Alert Program) for each blood cancer patient, provide support for interdisciplinary and cross-facility collaboration (e.g., tumor boards, weekly intake meetings, referring provider communication), and participate in care coordination between the inpatient and outpatient setting, as well as provider visits as appropriate. The Hematology Nurse Coordinator is expected to demonstrate professionalism and collaboration in all interactions, representing the Transplant and Cellular Therapy Program in internal and external outreach markets.

- Assess patient’s medical, social, psychosocial, and other care needs.
- Provides appropriate teaching and education to patients and families/caregivers. The goal of this education is to ensure the patient feels informed and empowered to manager his/her health.
- Streamline appointments and paperwork by assisting patients/families with scheduling appointments and preparation of treatments.
- Initiate referrals to hospital and community resources to connect patients with resources and supporting systems.
- Track and document interventions and outcomes.
- Drives process improvement.
- Conduct outreach to referring providers, care team and other medical professionals, as well as the corporate ‘customer’ community.
- Ensure service continuity.
- Basic Cardiac Life Support must be obtained within 30 days of employment start date
- Florida RN license and/or approved multistate RN license required. Compact license holders have 60 days to obtain the Florida license after relocation. BLS-AHA Provider issued card required within 30 days of start
- (BMTCN) Blood & Marrow Transplant Certified Nurse, or (OCN) Oncology Certified Nurse, or Bachelors Degree, or Masters Degree must be obtained within 1 year of employment start date
