Overview
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our healthcare team. Duties
  • Perform initial evaluations, establish treatment goals, and create individualized care plans for patients with orthopedic, neurological, geriatric, pediatric, and acute conditions.
  • Provide direct patient care utilizing therapeutic exercises, manual therapy, and other evidence-based techniques to promote recovery and functional improvement.
  • Supervise and delegate tasks to Physical Therapist Assistants and other support staff, ensuring continuity and quality of care.
  • Reassess and adjust treatment plans based on patient progress and changing clinical status.
  • Educate patients and families on home exercise programs, body mechanics, and injury prevention strategies.
  • Collaborate with physicians, nurses, and other members of the interdisciplinary team to optimize patient outcomes.
  • Participate in discharge planning and ensure smooth transitions of care between inpatient and outpatient settings.
  • Maintain documentation in accordance with facility standards and regulatory requirements.

Requirements
  • Current licensure as a Physical Therapist (PT) in the state of practice.
  • Graduate of an accredited Physical Therapy program.
  • Strong understanding of anatomy, physiology, kinesiology, and rehabilitation principles.
  • Experience in skilled nursing or acute care settings preferred.
  • Excellent communication and documentation skills.
  • Demonstrated ability to work independently and as part of a multidisciplinary team.
  • Experience in geriatrics, orthopedics, or neurological rehabilitation is a plus.

**Position Summary**
The Lead Patient Care Technician (PCT) delivers high quality, patient-centered care by performing a variety of delegated basic patient care services related to common nursing functions and activities of daily living. + Assisting patients with ADLs (Activities of Daily Living), supporting mobility and comfort, checking vital signs, collecting specimens, preparing for procedures, and monitoring telemetry as needed
+ Documenting patient care accurately and communicating important updates to the nursing team to ensure coordinated, timely care
+ Helping maintain a safe, clean environment by following infection prevention practices, setting up and caring for equipment, and ensuring needed supplies are ready

Role Mission: The Health Aide is primarily responsible for providing health services to students, conducting state-mandated screenings, maintaining 100% immunization state compliance and maintaining accurate files. The school health aide is also responsible for promoting health education and preventative health practices for students. All clinic staff report directly to the campus Assistant Principal of Operations (APO). All medical and compliance for the district is directed to the Director of Health Services.

Compensation: Compensation for this role is set at an hourly rate ranging between $16.00 for 0 years of experience and $0.00. This role is also eligible for a performance bonus based on team performance and goal attainment.

Responsibilities

As a LVN, you will visit patients in their homes and provide nursing care for them under direction of RNs and in compliance with the physicians' orders.

Qualifications

1. Must possess a valid state driver's license.

2. Must possess automobile liability insurance.

3. Must wear seat belts at all times while driving.

4. Dependable transportation kept in good working condition.

5. Must be able to drive an automobile in all types of weather conditions.

If you meet these qualifications, we want to meet you!

• Minimum nine months experience as a physical therapist
• Preferred community/home health experience
Required Certifications and Licensures:
• Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
• Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations
