Reach to Win Youth Home & Family Services is a Short Term Residential Therapeutic Program (STRTP). The program provides daily assistance, mental healthcare support, and living accommodations to youth in need. We provide a home environment. The Facility Manager reports directly to the Program Specialist (Facility Administrator).
We are seeking a qualified candidate to join our team. The essence of the Short-Term Residential Treatment Program (STRTP) Mental Health Care Worker is best stated to be accountable for meeting the needs of the teen and non-minor dependent girls and to provide adequate care and supervision of youth residents that are always in our care individually and in groups, in accordance with applicable law, licensing regulations, our clinical team and facility policies.
The Mental Health Care Worker's goals will be to improve the quality of life and make a difference in the lives of the youth under their direct care by providing direct supervision, support and behavior management in a safe, therapeutic, and nurturing environment residing in residential care.
ADDITIONAL QUALIFICATIONS: Mental Health Care/Direct care staff shall meet at least ONE of the following qualifications:
- Have obtained an Associate's Degree or Bachelor of Arts or Sciences degree Or • 2. Have obtained a valid Child Development Teaching Permit Or. • 3. Have completed 12 semester units of Early Childhood Education, Adolescent Development, Foster and Kinship Care Education, or Psychology and have completed at least 100 hours of on-the-job experience working with children. Or • 4. Have a valid certificate as an Alcohol Counselor, Drug Counselor or Alcohol and Drug Counselor, and have completed at least 100 hours of on-the-job experience working with youth children. • 1. The 100 hours of on-the-job experience may be completed after starting employment at the facility. As an LPN in Home Health, you will provide skilled nursing care to patients in their homes, supporting recovery, chronic disease management, and overall health maintenance. You'll work independently and collaboratively with the interdisciplinary care team to promote patient comfort and wellness.
As an LPN, you should be prepared to perform the following tasks:
- Assess patient health status and monitor vital signs during home visits
- Administer medications, injections, wound care, and other treatments as ordered
- Provide patient and family education on disease management, medication administration, and safety
- Coordinate care with physicians, therapists, and other healthcare providers
- Assist patients with activities of daily living as needed
- Document nursing care, patient progress, and any changes accurately and timely
- Identify and report any concerns or complications promptly
- Ensure compliance with infection control, safety, and regulatory guidelines
- Maintain professional boundaries and provide compassionate care in the home environment
- Travel to patient homes according to the schedule and maintain reliable communication
The ideal candidate must have at least one year of experience as an LPN in a Home Health setting. Additionally, they should possess a current BLS certification and be eligible for State licensure. Additional certifications may be required.
