Scope of Responsibilities
The Home-School Support Specialist facilitates the collaboration and communication between the home and the school and assists in removing barriers to improve achievement for identified students.
Performance Responsibilities
- Facilitates the participation of parents in decisions that impact their children's academic performance.
- Assists in the development, implementation, monitoring, and evaluation of home support plans.
- Participates as a building team member to maximize services for identified students.
- Identifies resources for the provision of staff and parent training opportunities.
- Utilizes existing district and community family support programs to coordinate service delivery to identified students and their families
- Maintains records of parent participation, school-to-home, and home-to-school contacts.
- Completes and submit Time and Effort Reports (i.e., Monthly Federal Certification Forms and Personnel Activity Reports) and/or any reports as required.
- Leads attendance efforts in the school.
- Performs other related duties as assigned.
- High School Diploma or Equivalency.
- Completion of college coursework and experience in education, criminal justice, social work, and/or human services.
- Demonstrated ability to successfully interact with students, staff, and families in diverse/multicultural settings.
- Demonstrated ability to maintain and support relationships with identified students and families. Monitor and evaluate student/family plans.
- Demonstrated ability to identify common student/family problems and recommend appropriate intervention.
- Ability to advise/advocate for students and families.
- Knowledge of community agencies and human services resources.
- Ability to work with groups.
- Reliable transportation.
- Evidence of desirable work record (i.e., low absenteeism, good job performance, flexibility).

Your Role and Responsibilities
As an RBT at UNIFI, you will:
- Implement patient-specific programming as prescribed by the BCBA.
- Follow behavior intervention plans, including safety procedures.
- Collect data and complete necessary paperwork and billing.
- Administer patient medications when needed.
- Attend and participate in scheduled meetings and training.
- Work professionally and collaboratively with the patient's entire UNIFI team.

Purpose Statement
The job of Home-School Liaison is done for the purpose/s of providing support to the
instructional program with specific responsibilities for conveying information regarding school
and/or district activities and procedures; monitoring and providing support regarding student
attendance concerns; providing information on services available to eligible students and
families; and referring families to other agencies.

Essential Functions
• Acts as a liaison between school district and parents/students for the purpose of facilitating access to
resources.
• Assists assigned administrators/supervisors in developing program resources for the purpose of
fostering community resources and developing reference lists of available community resources.
• Collaborates with internal and external personnel (e.g. school staff, parents, students, principal,
counselors, mentors, nurses, CPS, law enforcement, etc.) for the purpose of implementing and/or
maintaining services and programs.
• Conducts home visits for the purpose of assessing family needs.
• Coordinates assigned projects and/or activities (e.g. presentations, parent nights, volunteers, etc.) for
the purpose of completing projects and/or activities and/or directing services in a timely manner.
• Assists in reviewing and analyzing attendance records for all students for the purpose of identifying
trends and to assist in improving student participation school-wide.
• Assists in developing and monitoring student attendance reports for the purpose of providing support
for student groups identified with high absenteeism.
• Works with administrators, students, parents and other stakeholders to enhance student attendance at
the assigned site.
• Maintains a variety of confidential and non-confidential manual and electronic lists and records (e.g.
daily log of activities, program participation, progress, referrals, etc.) for the purpose of providing
required information and/or documentation.
• Orients students, families and volunteers for the purpose of establishing familiarity with program,
services, and required processes.
• Participates in a variety of meetings, workshops and committees (e.g. mediations, truancy program,
parent meetings, etc.) for the purpose of conveying and/or gathering information required to perform
functions.
• Prepares a variety of documents, reports and written materials (e.g. reports, memos, letters, etc.) for
the purpose of communicating information to students, parents, and/or staff.
• Responds to inquiries from a variety of internal and external sources for the purpose of providing
information and/or direction as may be required.
• Translates verbal and written communication(s) (e.g. documents, IEPs, conferences, etc.) for the
purpose of assisting students, teachers and parents in communicating.
• Assists the school front office team when needed in order to meet the operational needs of the site.
