Care Management Director

This position is responsible for providing management and oversight of daily care management activities provided in an acute care hospital. He/she will work in partnership with the medical staff, nursing, clinical, and revenue cycle leaders to ensure the provision of optimal patient care, attainment of financial goals, and development of leaders. This position has departmental accountability for hospital care management services (may include all or a combination of the following: medical social work (including trauma), transition planning, and care coordination. This position typically reports to the Hospital Care Management Director. Typical distribution of job duties: 50% tactical, 40% operational, 10% strategic.

Supervises the daily activities of the Facilities Management Department and other Support Service departments that report to the Facilities Management Director.

**Job Summary and Qualifications**
A Division Director of Facility Management (DDFM) will provide technical and project support for all matters relating to facility management within an assigned division. The DDFM will be required to live within the geographical region that they support. + Ability to evaluate infrastructure systems and components to determine operability problems, or maintenance and repairs that need to be performed, and provide technical guidance to remediate the problem or steps to move toward correction.
+ Continuously prioritize all infrastructure capital requests within the division in order to assist FacilitiGroup to address the most urgent needs
+ Review all facility and RCAP funded projects for scope and ensure all MEP issues are included and that they adhere to the HCA MEP design guidelines
+ Maintain a condition assessment database for all assets in the CMMS system
+ Ability to provide oversight for facility and RCAP funded projects to ensure that they are constructed per design. Assist the Corporate Cx engineer with design review, contractor commissioning process coordination and functional performance testing for corporate funded projects
+ Evaluate vendor performance at each facility

+ Provide interim support as schedule allows, in the event a facility is temporarily without a Director of Facility Management.
+ Provide onsite assistance to facilities that are experiencing an emergency such as a hurricane, earthquake, etc.
+ Provide assistance to HCA Planning and engineering consultants performing due diligence for corporately funded Construction and Equipment projects
+ Support facilities with technical expertise during projects and supervise major shutdowns when needed
+ Support facilities during infrastructure utilities losses
+ Support facilities during emergency remediation efforts, through vendor/contractor acquisition, moisture mapping, plan an approach to remediate, and support of build back efforts.
**People**
+ Work with each facility's administrative team to address any plant operation team efficiency issues.
+ Assist facilities in the hiring, training and evaluation of Facility Management staff.
**Growth**
+ Works with Regional Director of Facility Management and Director of Facility Management to develop and determine department work habits and practices for optimal upkeep of the building and its components.
+ At least annually provide each facility senior management team with an evaluation of their Facility Management department performance
+ Continual development of both staff and one's self through training and learning on the job, to provide upward mobility and growth in the future.
**Finance**
+ Oversight of critical infrastructure equipment to ensure efficient operation.

Responsible for the planning, organizing, directing, and controlling of engineering, maintenance, security, construction, and safety departments. Basic Qualifications:

Experience

+ Minimum one (1) year of Social Work experience in a hospital, medical outpatient clinic, home health/hospice, or long-term care setting -OR- successful completion of the Kaiser Permanente NW Social Worker Internship program.

+ Minimum three (3) years of additional experience in a related environment.

Education

+ Masters degree in social work from an accredited university.

License, Certification, Registration

+ This job requires credentials from multiple states. Credentials from the primary work state are required at hire. Additional Credentials from the secondary work state(s) are required post hire. required at hire

+ Clinical Social Work Associate Certificate (Oregon) within 6 months of hire OR Licensed Clinical Social Worker (Oregon) within 6 months of hire OR Masters Social Worker License (Oregon) within 6 months of hire

+ Licensed Social Worker Associate-Independent Clinical (Washington) within 6 months of hire OR Certified Social Worker Associate License (Washington) within 6 months of hire OR Licensed Advanced Social Worker License (Washington) within 6 months of hire OR Licensed Independent Clinical Social Worker (Washington) within 6 months of hire

+ Basic Life Support within 3 months of hire from American Heart Association

+ National Provider Identifier required at hire

Additional Requirements:

+ Ability to work effectively on a multi-disciplinary team.

Preferred Qualifications:

+ Minimum three (3) years of professional Social Work experience in a hospital, medical outpatient clinic, home health/hospice, or long-term care setting.

+ LCSW

+ Working knowledge of KPNW services and benefits

+ Expertise with short term counseling interventions.

+ Demonstrated ability to provide leadership to a multi-disciplinary team.

+ Advanced knowledge of local and national resources related to health care.
