Responsibilities:  Hospital/Nurse Liaison

  • Completes daily rounds in assigned hospitals, obtaining new referrals, assessing patient care needs, making the admission decision, and forwarding documentation to the appropriate building.
  • Communicates patient information to the building Director of Admission 
  • Obtains accurate information from physicians, patient, family and referral source personnel regarding the expected discharge plan and communicates this information to the Regional Vice President or the appropriate staff at the building.
  • Develops referrals from hospitals, emergency and therapy departments, physicians, case managers, insurance companies and other referral sources.
  • Meets with potential customers prior to admission to assist in “closing” the sale as well as obtaining personal like and info that can be used by facility to enhance experience
  • May also visit with patients following admission once at the facility to support the continuum of care and ensure our representation of the facility was as expected
  • Demonstrates growth in hospital referrals as demonstrated by an increase of overall occupancy and quality mix in assigned buildings BUT most importantly will attain goals for conversion rates
  • Coordinates work with other departments in ensuring we are meeting all expectations of the patient especially clinically.
  • Participates in marketing events

**Qualifications**

Physical Abilities:
+ Must be able to observe, in real time and without slowing or otherwise interrupting the progress of all applicable types of ongoing health care treatment (e.g., including emergency treatment, treatment during weather and other extreme situations, etc.).
+ Standing for long periods of time, walking lengthy distances, lifting, climbing, stooping, pulling, and pushing to adequately inspect and observe all medical facilities, equipment, and procedures, such as emergency exit procedures, remote storage facilities, any areas where cleanliness may affect the possibility of infection, medical equipment, etc., including the following activities:
+ walking up and down stairways (e.g., to test escape routes, assess safety of emergency exits, regulatory compliance, etc.).

  • Write patient progress reports to maintain patient medical records and document unusual behavior.
  • Assess emergency situations and take appropriate action to overcome and resolve patient issues.
  • Intervenes therapeutically when patient exhibits signs of behavioral escalation to overcome situation.
  • Assesses patient's health status, plans care, or makes appropriate referrals to ensure patient needs are met.
  • Recommends changes to patient treatment plans, if applicable.

MINIMUM QUALIFICATIONS:

A Bachelor's Degree in Nursing or related field -OR- graduation from an accredited school of nursing plus one (1) year of professional nursing experience.

A license as a Registered Nurse as issued by the Maine State Board of Nursing.
