Treatment Adherence Nurse Needed
Full Time Temp Position
Position Summary
We are seeking a dedicated Treatment Adherence Nurse to join our outpatient clinical team. This role involves training clinical staff on the Partnership for Health and medication adherence interventions , supporting integration into clinical sessions, monitoring fidelity, tracking service delivery, managing intervention data, and coordinating booster trainings. The ideal candidate will be passionate about patient engagement and treatment adherence.
Essential Functions
- Engage patients in meaningful dialogue to promote adherence to medical treatment.
- Coordinate and train clinical staff on effective Partnership for Health (PFH) interventions and ensure proper integration into clinical sessions.
- Deliver evidence-based interventions focused on prevention, risk reduction education, treatment adherence, sexual health, and substance use prevention .
- Provide ongoing treatment adherence support , emphasizing medical appointment compliance and medication adherence.
- Work collaboratively with medical providers and case management staff to facilitate access to medical and social support services.
- Conduct patient follow-ups and document outcomes in electronic medical records (EMR).
- Track and manage patient data for adherence interventions and support reporting processes.
- Assist in managing daily patient flow within the clinic to optimize care delivery.
- Lead daily huddles with healthcare teams and assist in identifying patients requiring follow-ups.
- Support front-end clinic infrastructure, including scheduling.
- Promote a culture of safety through communication, documentation, and teamwork.
- Attend ongoing medical education and provide Health Literacy Assessments .
Qualifications and Requirements
-Active RN license in the state of NY .
-3+ years of job-related experience (HIV-related experience preferred).
-Bilingual English/Spanish preferred .
- Strong ability to work independently and as a team member .
- Proficiency in Microsoft Office and electronic medical records (EMR) .
- Strong organizational skills with the ability to prioritize workload and manage caseload .
- Ability to provide patient-centered, holistic care to improve treatment adherence.
- Experience with coordinating training programs and integrating interventions into clinical practice.
Working Conditions and Physical Requirements
- Some manual skills required, including fine motor coordination.
- May involve prolonged standing, transporting patients, or lifting (up to 30 lbs.) .
- Work may occasionally require evening, night, holiday, or weekend shifts .

Job Description
Hospital Care at Home is an innovative care model that allows us to provide inpatient level care to patients in their homes. Potential patients are assessed by a Home Hospital provider to ensure their diagnosis can be treated in the home, that the patients home environment is a safe place to receive care, and that the patient has consented to participate. We have successfully cared for more than 500 patients in Hospital Care at Home and are excited to continue to grow!
The Home Hospital RN will partner closely with the attending physician and other members of the care team to provide care for patients, both virtually and through in-home visits. Primary responsibilities include:
- Participate in the initial and ongoing assessment of patients determining care and services needed to stay in the home.
- Collaborate with medical providers, patient care staff and skilled services in the planning and implementation of patient care plan.
- Direct and coordinate services (i.e., nursing, therapy, care management, etc.) provided in the home during the inpatient admissions.
- Collaborate with the clinical team for initiation of remote monitoring services. Monitor and manage alerts, provide education and assess patient understanding for use.
- Act as a central point of contact for all patients, including interaction with medical, nursing, and ancillary services
- Develop and coordinate individual nursing plan-of-care including patient/family education and discharge planning.
- Drive to patients homes within Franklin and the six contiguous counties in a university-provided vehicle to provide in-home care.
- May support provider with patient identification and enrollment activities.
- Participate in ongoing improvement initiatives to foster a culture of high quality and patient safety

Minimum Qualifications
For Hire:
- Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program required.
- Current Ohio RN license or interim permit.
- Minimum of 3 years clinical experience in the acute care setting, preferably in a medical-surgical, progressive care environment or emergency department. *Experience in relevant care setting preferred.
- Valid United States or Canadian drivers license.
- Ability to work independently, be self-directed and adapt to unpredictable circumstances.
- Effective interpersonal, verbal and written communication skills.
- Ability to problem-solve and iterate process improvements in delivery of home-based hospital level care
- Computer savvy and effectively utilizes technology, including Remote Patient Monitoring (RPM) to gather, report and analyze clinical data.

+ Demonstrates an understanding of and ability to use the nursing process. Participates in the assessment, planning, intervention & evaluation of physical, emotional, psychosocial, spiritual, & learning needs of the patient & significant other(s).
1. Responsible for assessment of patient care needs.
1. Assess each patients condition at time of admission.
2. Completes thorough admission assessment for each patient admitted within 8 hours of admission and completes care plan within 24 hours of admission.
3. Assess each patients condition at start of shift and thereafter as patients condition requires and document.
+ Responsible for planning to meet patient care needs.
1. Initiates an individual care plan or individualizes standard care plan for patient, reviews and updates this plan of care as needed.
2. Obtains input for care plan from patient and family.
3. Individualized teaching plan is reflected in the plan of care.
4. Initiates discharge plan including the desired goals for each patient.
5. Prepares patient assignments giving consideration to the needs of the patients.
6. Calls surgical patients the night before surgery.
+ Responsible for implementing quality nursing care. Performs direct nursing care & patient treatments in compliance with standards, policies, procedure, & protocols of CVH & regulatory governing agencies.
1. Performs and documents daily patient care in the EMR.
2. Provides individualized patient teaching and records patient understanding.
3. Explains procedures to family and patient before implementation.
4. Carries out physicians orders promptly and accurately.
5. Questions physician when orders need clarification and documents result.
6. Questions physicians orders when conflict with hospital policy and/or nursing judgment.
7. Discusses conflicting orders with attending physician and if necessary thereafter follows chain of command then documents result.
8. Prepares and administers medications according to established policy and physician orders.
1. Directly administers care in complex and acute care situations.
2. Directs the implementation of care of team members according to their skill level.
3. Identifies and prioritizes nursing actions based on patient condition.
4. Makes patient rounds with physicians.
+ Responsible for evaluation of nursing care.
1. Evaluates and records observations of patient response to medication and other nursing care measures.
2. Recognizes changes in patient condition and notifies physician as appropriate.
3. Completes follow-up calls on discharged patients.

2. Maintains strict confidentiality of all hospital and patient related information.

4. Respects and supports co-worker, & families, as well as the patient.

+ Functions as an active member of the health care team.
1. Utilizes interactive communication with other staff, physicians, & other health care disciplines, to provide & implement a comprehensive & integrated plan of care, including discharge planning.
2. Answers nurse call system promptly and courteously.
3. Assists in orientation of new employees as directed.
4. Assists other staff members in completion of their duties as time and assignments allow.
5. Assists in keeping med-room and all utility rooms orderly and well stocked.
6. Demonstrates ability to work with other members of the healthcare team.
7. Responds to all code blue and emergencies taking charge of activities when appropriate.
8. Utilizes appropriate chain of command.
9. Notifies surgery crew and nursing staff of assignments as necessary.
10. Completes patient registration after business office hours.
11. Assists in general ward clerk duties including maintaining charts and general clerical duties as assigned. Refer to unit clerk checklist.
12. Answers and documents ambulance calls.
13. Uses present paging system effectively.
14. Assists in monthly crash-cart checks.
+ Contributes to a safe patient environment.
1. Knows and observes departmental and hospital safety, fire, and disaster policies and procedures.
2. Reports and documents incidents involving patients, staff, and visitors.
3. Assist in keeping patients' rooms orderly and hazard free.
4. Demonstrates proper technique when transferring and repositioning patients.
5. Insures nurse call bell is within patient reach at all times.
6. Reports broken and non-functional equipment to appropriate persons.
7. Is aware of and promotes the Fall Prevention Program.
+ Recognizes job relationships and is willing to function effectively within organizational structure.
1. Performs in whichever capacity patient census, condition, or present staffing level necessitates. Performs the duties of those supervised in daily functions, as needed.
2. Recognizes legal responsibilities and functions within these limitations as specified by state law and hospital policy.
3. Performs daily duties utilizing the AHS SHARE principles.

1. Attends staff meetings, (at least half of them within past year) and inservices to maintain a current knowledge of nursing practice.

5. Renews BLS / ACLS certifications as required.

Qualifications
The expertise and experiences you'll need to succeed:
+ Wisconsin Registered Nurse License
+ BCLS obtained through the American Heart Association
+ Associate's Degree in Nursing - Diploma Nurse or Bachelor's Degree Nurse
+ No Previous Experience Required
