Job Description
Performs a variety of functions and technical procedures necessary for patient admission, assessment, diagnosis, treatment, followup, and outcome evaluation. Participates in performance improvement, research, and educational activities. ACP will open new cases to complete new patient enrollment and home care assessments. Perform home visits in the Queens/Western Nassau region. Visits take approximately 1-1.5 hours, approx. 6 visits per day. ACP must be available 1+ days per week.
Requirements:
+ Takes initial patient histories and performs physical examinations.
+ Initiates, documents, and communicates the plan of care and follows up to ensure complete screening and preparation of patients.
+ Performs daily patient rounds.
+ Writes orders for medications, laboratory work, and diagnostic tests.
+ Interprets laboratory and test results.
+ Confers with attending physicians, residents, nursing staff, and/or other care providers to ensure optimum quality of patient care.
+ Upon discharge, notes discharge summaries, prescriptions, and any referrals on patient charts.
+ Informs patients of the necessary post-discharge care instructions.
+ Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions
Qualifications:
+ Master's Degree in a Nurse Practitioner Program and National Board Certification in specialty area required.
+ Current license to practice as a Nurse Practitioner in New York State, and current license to practice as a Registered Professional Nurse in New York State, required.
+ Has a collaborative practice agreement with designated protocols filed with NYS Department of Education OR collaborative relationship attestation as per the Nurse Practitioner Modernization Act 2015.
+ Current BLS certification, required

**Primary Responsibilities:**
+ Conduct in-home assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
+ Perform evidence-based practice screenings including point of care testing (as appropriate)
+ Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
+ Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
+ Communicate findings in your assessment that will be used to inform member's PCP of potential gaps in care
+ Identify urgent and emergent situations and intervene appropriately
+ Educate members on topics such as disease process, medication, and compliance

**Required Qualifications:**
+ Active NP license or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
+ Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or the ability to obtain national certification and/or NP license in state of assignment by start date
+ Ability to complete physical requirements of the job including moving a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, remain in a stationary position and position self to perform physical assessment
+ Driver's license and access to reliable transportation to complete home visit assessments
**Preferred Qualifications:**
+ 1+ year of clinical experience in their highest level of education, clinical setting
+ Experience in gerontology, cardiology, internal medicine, or endocrinology
+ Home Health care or home visit experience
+ Ability to travel up to 5% within licensed states if requested for additional incentives (with notice, based on business needs)
+ May be requested to obtain additional licensure in other geographic areas
The salary range for this role is 104,500 to 156,000 annually based on full-time employment. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement. Role DescriptionThe Medical Director will be joining Oak Street Health at a critical time in the history of our company and of primary care. The opportunity for impact is large and growing as Oak Street is leading the transition from fee-for-service to value-based care. The Medical Director will be responsible for leading, as a dyad partner with the Mobile Practice Manager, the in-home primary care provider teams in the specified geography.
The Oak Street Health At Home division is an interdisciplinary team that focuses on primary care and transitional care support for our highest-need patients in their homes. The primary responsibility for the Medical Director is direct management, oversight, and coaching of the local team, and is part of the At Home division's leadership team which helps develop programs (i.e., complex care coordination, chronic disease management, psychosocial support, etc.) throughout the division. This entails collaboration with the clinical teams in the center, medical directors, and other partners outside of Oak Street Health. The Medical Director will see patients in their homes up to 3 days a week alongside their provider colleagues. Additionally, we see this leader being a key thought partner in providing feedback on our clinical model especially as it pertains to the At Home Program.
Core ResponsibilitiesThe Medical Director's primary duties are focused on the management of their teams. Key responsibilities include:
- Leading At Home providers and ensuring their priorities and outcomes are in line with those of the organization
- Providing timely and constructive feedback to providers so the medical group continues to improve performance for patients and colleagues
- Helping providers master the data-driven tools and practices required to keep patients well and out of the hospital, when avoidable
- Reviewing regular quality dashboards with providers, coaching on tools to improve quality outcomes
- Coach providers on appropriate coding and documentation
- Provide managed care experience/teaching to providers
Additionally, the Medical Director will be part of the At Home leadership team that will have strategic input into divisional projects supporting the enhancement of the care model, including:
- Medical management, care management, utilization management, and quality improvement tools and processes
- Development of clinical compliance policies and guidelines
- Representing Oak Street to external partners (e.g., vendors, payers, providers) and creating relationships and partnerships that support our mission and value-based model
- Understanding and communicating provider performance/compensation plans and being able to answer questions and deliver results to direct reports
- Serving as formal manager doing regular performance reviews and check-ins, approving paid-time-off requests, and managing miscellaneous human resource topics for providers in the market
- Driving culture: fostering innovation, open dialogue, working with urgency, and setting an example for others

@HOME delivers high-quality primary care to Medicare patients in their home. The @HOME program takes a team-based approach to providing outstanding patient care to the population attributed to Oak Street Healths Managed Service Organization (MSO). Our Care Teams - consisting of a Provider, Medical Assistant/Scribe (CIS/MA), Behavioral Health Specialist (BHS) and Mobile Operations Coordinator (MOC) - build individual relationships across a panel of about 75 patients, seen monthly. The Care Team is responsible for delivering excellent, high-touch primary care, and coordinating the care of our patients throughout the healthcare delivery system.
Role Description
The Market Expansion Medical Director is responsible for ensuring standardization and success of new at-home geographies. For the first 6-12 months of a new market launch, the Medical Director will travel 30-50% of time to be in-market, side-by-side with newly hired providers and leadership teams to teach, guide and coach through workflows to ensure on-the-ground teams are executing best practices in patient care and operations.
Candidate Responsibilities
- Help establish new market infrastructure
- Partner with operational leadership to establish offices and resources in new markets
- Participate in launch planning, including structure for care team zones and geographies
- Seek and establish relationships with local vendors for home health, hospice, wound care, physical and occupational therapy, and other support services as needed
- Drive development of new market care teams
- Participate in hiring, onboarding, and clinical education
- Establish a spirit of clinical excellence and camaraderie in the new market
- Supervise and coach providers (Nurse Practitioners and Behavioral Health Specialist) using standardized tools and best practices, including
- Shadowing visits, reviewing admissions and prevention strategies, and coaching end of life discussions
- Guiding data-driven practices to keep patients happy, healthy, and out of the hospital
- Sharing data and helping providers understand it
- Ensuring accurate coding and documentation
- Supporting scheduling and billing processes
- Owning first 90-days provider performance reviews (until local leadership is ready), including gathering performance feedback from others in the market
- Assist with supporting and coaching other team members (CIS/MAs and MOCs) as needed
- Train and transition new market leadership to operate independently
- Help cultivate dyad relationship between market Medical Director and Practice Manager
- Help market leadership establish team culture and relationships
- Ensure patients are always delighted with the level of care they receive

Were Looking For Motivated Leaders With

- Experience using a metrics-driven approach to the provision of medical care and/ or quality projects
- Minimum 2 years of experience providing in-home primary care
- 5+ years of experience in outpatient practice preferred
- Fellowship training in Geriatrics, other professional degrees (e.g., M.B.A., J.D., M.P.H.), and prior executive roles welcomed but not required
