**Responsibilities**
Mayo Clinic Health System in Western Wisconsin is seeking a Board Certified/Board Eligible Infectious Disease physician to join a collegial department. The primary location for this position is in La Crosse, Wisconsin.
This opportunity is 100% Infectious disease. The physician will provide patient care in the clinic and hospital settings and participate in the Infection Control and Prevention and Antimicrobial Stewardship activities in the primary site. MCHS in Western Wisconsin is a fully integrated care system and is run by a single governing structure thus helping to facilitate care coordination and patient follow-up. In this setting physicians will:
+ Community practice with academic segment working with family practice residents and occasional medical students.
+ Opportunity to work with most medical specialties in a setting where you see a wide variety of illnesses and disease states.
+ Provide physician support Infection Prevention and Control initiatives and antimicrobial stewardship activities along with infection preventionists and pharmacist respectively.
**Hospital Practice:** Consultations/Follow ups, no admissions. There is a large variety of infections: Endocarditis, orthopedic infections, pneumonias, zoonotic diseases, complicated soft tissue infections, endemic mycoses, tick-borne diseases. Minority of transplant-related infections. In addition, to a small HIV/AIDS practice.
**Clinic Practice:** Post-hospital follow-ups, travel clinic, and referrals from other physicians. We have IV antibiotic infusion center at our clinic with a skilled nurse supporting the practice in the infectious disease clinic.
**IPAC and Antimicrobial Stewardship:** Infection control with a group of 3 infection preventionists. Antimicrobial stewardship program with Pharmacy. No antimicrobial approval calls.
You will work as a part of the One Wisconsin Infectious Disease department and participate in a Mayo Clinic Enterprise committees.

The Department of Pediatrics has an active pediatric allergy/immunology program treating a range of allergenic and immune system disorders and provides services including:

  • Allergy skin testing
  • Allergy shots (immunotherapy)
  • Asthma management and collaboration with pediatric pulmonary specialists for asthmatic patients
  • Eczema management
  • Food allergy evaluation and management
  • Oral food challenges

Physical Requirements

Work consistently requires walking, standing, sitting, lifting, reaching, bending, stooping, pushing, and pulling. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. Possible exposure to communicable diseases. Must speak, read, and write Basic English fluently. Intact senses of sight, hearing, smell, and touch. Manual finger dexterity.

Education, Experience and Certifications

Licensed Practical Nurse with current license to practice in applicable state (dependent on the practice's physical location) required. Graduate of an Accredited School of practical nursing required. Previous medical office experience preferred. BLS required per policy guidelines.

UNC ID Outpatient Parenteral Antimicrobial Therapy (OPAT) program is seeking RN to join our team! As an OPAT Nurse Coordinator, you will play a critical role in ensuring our patients can safely continue IV antibiotic therapy at home while reducing hospital readmissions and promoting better outcomes. What You'll Do: + Involves clinical oversight, patient education, and coordination with providers, pharmacists, and home infusion companies to ensure safe, effective treatment and smooth transitions of care + Coordinate and oversee infusion care for patients receiving IV antimicrobial therapy outside of the hospital setting + We are committed to excellence in patient care and employee development Location: Worksite is in outpatient/clinic setting in the Infectious Diseases Clinic at Eastowne Job Duties: + Monitor clinical progress, lab results, and response to treatment through regular follow-up (Phone or in-person) + Collaborate with multidisciplinary teams including providers, case managers and home infusion companies + Provide education to patients and caregivers on IV therapy, catheter care and signs of complications + Serve as a liaison between patients and the healthcare team to address questions or concerns + Assess patients for OPAT eligibility and assist with transitions from inpatient to outpatient care + Responds to patient calls regarding symptoms, IV complications or therapy issues + Participate in OPAT program quality improvement initiatives + Meet with patients during clinic visits Experience Preferred: + Experience in infusion therapy, home health care or acute care + Strong clinical judgement and patient education skills + Experience with central lines/PICC line Incentive: + $10,000 + To learn more, visit: Summary: Provides clinical administrative nursing care coordination in support of a patient care area. Duties include one or more of the following care coordination, discharge planning, data analysis and performance metrics, and patient/staff education. Responsibilities: 1. Assesses patient status of a specialized population. Rounds with an interdisciplinary health care team to determine patient care needs and coordinate ongoing acute care and long-term needs. Uses expertise and experience in specialty practice to facilitate patient care. Acts as a liaison between the patient, family and health care team to communicate patient care needs including coordination of patient/family care conferences. 2. Collaborates with health care team to provide information and resources to facilitate patient care. Provides inservices and formal presentations to nursing, medical and ancillary staff on topics related to the specialized patient population. Assist staff in the development of patient education materials for the patient population. Perform patient teaching related to specific topics and patient needs. Documents patient teaching in the medical record in the appropriate sections 3. Collects data related to specialized patient population and analyzes for trends. Enters data or coordinates its entry into databases and registries for benchmarking purposes. Participates in performance improvement activities to promote quality patient care. Analyzes data for trends and makes suggestions for revisions to patient care protocols for the patient population. Monitors outcomes of care for patient population and collaborates with the interdisciplinary health care team on evaluation of outcomes. 4. Coordinates patient care for post-hospitalization care and discharge planning. Makes referrals to other care providers during acute care episode and for post-discharge care. Communicates discharge needs to interdisciplinary health care team, patient and family. Coordinates appointments with clinics, outside referral sources, home care, durable medical equipment providers and other providers to ensure continuity of care for the patient. Attends care conferences and interdisciplinary care rounds. Documents care planning and communication with health care providers in the medical record. 5. Promotes own professional development through attending inservices and presentations. Keeps current in practice through conferences, reading current literature and research.
