Job Summary:
As an Integrated Care Plan Program Coordinator, you will provide administrative support and assistance to the health insurance team, Integrated Care Management team, and the Clinical Oversight team. You will manage completed Health Risk Assessment (HRA) forms among the ICM team members responsible for completing Care Plans.

The goal of this primary task is to clear the "ICM Pending Queue" Tab in CareStat Databased (CSDB) daily and assign a Care Manager to the case in MHK.

Requirements and Qualifications:

* Provides Bridge Appointments: review hospital discharge orders, shared appointment with provider, enter new orders into epic, enter hospital info into epic such as reason for hospitalization and other items as needed
* Assists patient with scheduling with PCP
* if provider is not available for shared appointment then assesses patient and if patient is stable on current meds, then sends refill request to provider
* Coordinates, implements, and administers IM injections
* Accesses OARRS report when patient is on controlled meds
* obtain urine for drug screen for any patient that is on a controlled med. (when provider orders this)
* Communicates and coordinates services with other medical providers
* Engages in health promotion, prevention, and education activities (i.e. assess for risky behaviors and attempt behavior change)
* Meets with patients individually to provide comprehensive medication education and to monitor for side effects and the effectiveness of medications. Assists patients in developing a system to assure medication compliance. Involves family and/or significant others as needed and requested by the patient
* Educates other team members to help them monitor psychiatric symptoms and medication side effects
* When patients are in agreement, develops strategies to maximize the taking of medications as prescribed (e.g. behavioral tailoring, development of individual cues and reminders)
* Works with the program psychiatrist to initiate and stabilize patients on medications
* Assists patients in obtaining medications through medication assistance programs, as needed
* Participates in clinical team meetings to assure appropriateness and coordination of care
* Provides consultation services regarding psychiatric and medical difficulties, as necessary
* Monitors and records patient/patient medication availability and usage, acts as liaison to pharmacies
* Maintains organization and safety regarding medications by storing and wasting them according to policy
* Performs shift management in coordination with other team members according to established policies and procedures
* Provides triage line coverage.
* Participates in daily staff organizational meeting and treatment planning review meetings.
