Integrated Care Plan Program Coordinator

W2 Contract

Salary Range: $47,800 - $54,000 per year

Location: South San Francisco, CA - Hybrid Role

Job Summary:

As an Integrated Care Plan Program Coordinator, you will provide administrative support and assistance to the health insurance team, Integrated Care Management team, and the Clinical Oversight team. You will manage completed Health Risk Assessment (HRA) forms among the ICM team members responsible for completing Care Plans.

Duties and Responsibilities:

  • Support data entry by entering Health Risks Assessments (HRA) received by mail into GlobalForms and update in other systems and applications, including CareStat Database, PSICapture (an advanced document capture software), and MHK.
  • Work on "closed cases" in the "Pending ICM Queue" to the MedHOK (MHK) application. This task involves following the workflow outlined by ICM to get the case assigned to an ICM Model of Care (MOC) team member. The goal of this primary task is to clear the "ICM Pending Queue" Tab in CareStat Databased (CSDB) daily and assign a Care Manager to the case in MHK.
  • Support the Care Plan Program Manager in testing and validating system updates in CareStat Database, GlobalForms, PSICapture, and MHK.
  • Support the Member Assessment Unit (MAU) in special projects like HRA for members with significant changes in condition, etc.
  • Perform other duties as assigned.

Requirements and Qualifications:

  • High school diploma or GED required
  • College-level courses in health care are a plus.
  • One year of managed care experience, preferably in Care Coordination.
  • Experience working with the health needs of the population served.
  • Experience as a Medical assistant is a plus.
  • At least one year of direct Care Coordination
  • Microsoft Office Suite applications, including Outlook, Word, Excel, Access, and PowerPoint.
  • Care management, Medi-Cal, Medicare benefits, and the complexities of working with the elderly and disabled population.
  • Bilingual skills are highly preferred, particularly in Spanish, Tagalog, or Chinese.

Summary: The Integrated Care Nurse acts as a liaison between primary care and behavioral health providers, ensuring patient care integration and facilitating communication between providers and partner agency care teams (i.e. nurses, social workers). Essential Duties and Responsibilities:

* Provides phone and walk in triage, facilitates medication refills, and administers medications.
* Promotes health and well-being for patients and families by assessing health literacy, providing meaningful health information, and supporting self-management of chronic physical and mental illness.
* Supports Medical Assistants by providing information, ongoing development and training.
* Assures safe patient care environment. Oversees infection control practices and management of the supply and storage of medication.
* Performs clinical patient visits such as administering long-acting psychotropic injections, providing education and/or follow-up on diabetes care, asthma and/or other chronic diseases.
* Collaborates with care team members to support patients during transitions of care:
* Provides necessary support in the coordination of referrals, including the access of necessary tests or other values and reports as needed
* Provides information and support to patients and families and works with the care team to assure appropriate follow up.
* Fosters relationships with community agencies, recovery counselors as needed.
* Additional responsibilities may include serving as the electronic medical record superuser and/or providing group health education.
* Participates in required care team and other organizational meetings.

Responsibilities

As a nurse in this department, you will have varying roles and responsibilities. You may work in Obstetrics - triaging patients or perform screening exams on walk-ins or you may guide patients through a routine induction and delivery. You're busy monitoring the mother's dilation progress while also keeping close watch on the baby's fetal heart rate. You'll be coaching mom as the baby is delivered and providing updates and support to the patient's partner and loved ones.

To thrive in this role, you'll need to appreciate that routine is never routine. We have some wonderful days in this department and get to be with people through huge milestones in their lives. We also have some hard days and are with people as they grieve. You must be sensitive and understanding to different circumstances. Above all else, you must remain calm in an emergency. A patient's status can change quickly and it's never good for the patient or family to see the nurse in a panic. You're calm. You're in control. You're handling the situation.

Responsibilities

* Triage walk-ins to determine if a patient is in labor, water is broken, false alarm, etc
* Support pregnant patients in labor including induction, epidural assistance (if needed), general pain management and fetal monitoring
* Assist and circulate in the operating room for cesarean sections, tubal ligation, cerclage and emergency procedures
* Educate patients on various interventions or procedures - medications (name and purpose), exams, and labor stages
* Assess newborn and provide care.
* Communicate with the care management team, including anesthesiologist, physician or midwife, lactation nurse, charge nurse, postpartum unit, patient and family

One (1) year Labor and Delivery experience preferred

Education:

ADN, Diploma, or Foreign Equivalent

BSN preferred

Licensure:

AZ RN license or RN License from a Compact State.

Current BLS Card (American Heart Association Only), Current ACLS Card (or within 6 months of hire),

NRP certification within 6 months of hire.
