Position Summary: The Home-Based Crisis Intervention (HBCI) Interventionist provides intensive, short-term, in-home crisis services to children and adolescents ages 5–20 and 11 months who are experiencing acute psychiatric crises that place them at risk for psychiatric hospitalization. Under the supervision of a licensed clinical supervisor, the Interventionist works collaboratively with families to stabilize the crisis, assess needs, and build coping strategies to prevent future hospitalizations.

The Interventionist provides direct support services, assists with treatment planning, and implements behavioral strategies and interventions to promote emotional regulation, problem-solving, and safety within the family environment. The Interventionist also coordinates care with schools, community providers, and other support systems to ensure continuity of care. Services are typically delivered multiple times per week, with flexible scheduling (including evenings and weekends) to meet family needs.

Requirements

Education:

  • Bachelors in human services, psychology, social work or other related field required
  • Masters in Social Work, Psychology, Social-Welfare, Human Services or other related fields preferred
  • Licensed mental health provider or unlicensed Masters preferred

Experience:

  • 1-2 years experience in case management, community based mental health, and other related responsibilities required
  • Driver's License and ability to maintain insurability throughout employment required .

Summary/Objective:

To help maintain the child/youth in the home, school, and community, the Home Based Crisis Intervention (HBCI) team delivers intensive crisis intervention to children and families within Jefferson County. The HBCI Program aims to serve children/youth ages 5 to 20 years at imminent risk of psychiatric hospitalization or admission to a group treatment program, or at risk for a rapid readmit to such settings.

Under the supervision of the HBCI Clinical Supervisor, the HBCI Interventionist responds to referrals and provides treatment to address the clinical and complex needs of the child/youth and their family/caregivers. In addition to providing direct crisis intervention and stabilization, the Interventionist provides case management services linking the family with additional short and long-term available services.

Essential Functions:

The major responsibilities of this position include but are not limited to the following:

  1. Respond to each referral within 48-hours.
  2. Meet with each youth and family/guardian to discuss program options, service provision, and alternative service options.
  3. Conduct all admission paperwork and ensure all necessary consents/releases are signed.
  4. Collaborate with already established providers such as care managers, therapists, and psychiatric providers.
  5. Provide intensive crisis intervention and supportive counseling to the youth and family.
  6. Participate in team meetings.
  7. Work with community agencies, organizations, and systems for long-term planning and support of the youth and families served.
  8. Make referrals to mental health services and other programs as needed.
  9. Provide thorough documentation regarding all case related activities including but not limited to; face to face contacts, phone calls, case conferences, clinical interventions, noted incidents, etc.
  10. Represent CHJC as a liaison with various private as well as public service providers and resource agencies.
  11. Complete all CHJC and New York State mandated reports and forms in a timely fashion.
  12. Continuously promotes the program and provides community education.
  13. Participates in an after hours on-call rotation.
  14. Other duties as assigned or requested.

Special Requirements:

Work Environment: Office environment will require occasional travel to recruitment events, local schools, conferences and/or meetings and moderate travel within the community to include off-site/in-home visits. May have contact with service recipients that are agitated and/or are in crisis. Some risk involved working with service recipients with mental illness. Works in an office setting with a controlled temperature environment. Occasional exposure to inclement weather conditions may occur depending upon assignments.


Equipment: This position is required to maintain a working knowledge of related office equipment including personal computers and printers, audio-visual equipment, telephone systems, copiers, fax machines, etc.


Physical Demands: Must be able to sit, stand, walk, lift, carry, push/pull, climb, bend, and stoop. Must be able to perform fine motor skills, read, and type. Must be able to sit for long periods of time. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.


Position Type/Expected Hours of Work:

Full-time, traditional hours and non-traditional hours. This position is required to work on-site, and offsite/in home visits. This position will be in the on-call rotation.



Travel:

Travel is required throughout service area (Jefferson, Lewis, Oswego, and St. Lawerence County).

Required Education and Experience:

Bachelor’s Degree in a Human Services field, with at least two years relevant experience in community-based mental health or case management is required. As this program is seeking applicants possessing a Master’s Degree in a clinical field (i.e., Mental Health Counseling or Social Work), those possessing a Master’s Degree and/or those as licensed clinicians will receive preference.


Skills/Abilities/Knowledge:

Strong organizational abilities and office skills are essential for this position. This person must also have excellent people skills and be a team player. Ability to work with diverse populations. Must demonstrate effective and excellent oral and written communication skills and the ability to work cooperatively with others. Must be detail-oriented and possess excellent time-management skills. Requires self-direction, tact, diplomacy, and the ability to be clear, courteous, and professional. Able to react to change productively and handle other duties as assigned.

What would this role do?

The Behavior Interventionist plays a critical role in implementing behavioral health treatments for children with Autism Spectrum Disorder (ASD) and other developmental disabilities. Through strategic programs and initiatives, this position will assist families seeking support with Autism and other related developmental disabilities by providing behavioral health treatment in-home, at a support center or in a community. This position is responsible for making meaningful change in the Client and families’ life.

    • Pay Rate: $20.00 - $5.00 per hour
    • 300 for part-time employees after 150 clinical hours worked within 6 months

Role Responsibilities (Here’s what it takes)

  • Provide 1:1 care directly with Clients during scheduled sessions under the supervision of a Board- Certified Behavior Analyst (BCBA).
  • Implement established behavioral treatment plans based on the science of applied behavior analysis (ABA) across skill levels, moderate to severe problem behaviors, and service delivery settings.
  • Ensure that 100% of the scheduled direct services are provided without any scheduling interruptions.

  • Collect data related to Client’s progress during treatment sessions.
  • Incorporate activities and games that are developmentally appropriate and engaging for the client in alignment with the treatment plan.
  • Promote Client engagement to build their strengths in a natural and enjoyable manner.
  • Consulting with case Supervisor on treatment plans, Client updates, and intervention strategies.
  • Receiving and utilizing ongoing training, guidance, and support.
  • Follow program procedures, administrative tasks, direct therapy skills, and other duties that are in line with our compliance requirements.
  • Maintaining reliable communication to record scheduled appointments and return phone calls within 24 hours.
  • Ensuring that the company’s “zero tolerance policy” is understood, implemented, and reinforced in the region.

Behavior Interventionist's are responsible for assisting behavior analysts and case managers to implement treatment plans which consist of behavior reduction and skill acquisition programs. The team is composed of Board-Certified Behavior Analyst and Licensed Mental Health Professionals. The behavior interventionist will primarily be working under the supervision of the Board-Certified Behavioral Analyst while they work directly with clients in their home. Trainings are held in-person at our office.

General Accountabilities


  • Model and prompt activities to train self-care practices.
  • Converses with the individuals to promote social skills and social interaction.
  • Accompanies families in community outings.
  • Observes and documents behaviors in order to facilitate assessment and development of treatment goals.
  • Maintain strict confidentiality.
  • Be comfortable working in homes (studio apt., housing projects, etc.) of families with limited resources.
  • Attend training to acquire further knowledge.
  • Provide services based in the science of Applied Behavior Analysis, including behavior management, discrete trial training, social skills training, and caregiver education to clients with developmental disabilities and their families.


Job Qualifications


  • Bachelor's or Associates degree (in progress) in a behavioral health or related field.
  • Driver's License

  • Pass FBI/DOJ clearance through Live Scan
  • Must be available to work between the hours 2pm-8pm AND be available for a minimum of 10 hours per week - weekend coverage also available.

What would this role do?

The Behavior Interventionist plays a critical role in implementing behavioral health treatments for children with Autism Spectrum Disorder (ASD) and other developmental disabilities. Through strategic programs and initiatives, this position will assist families seeking support with Autism and other related developmental disabilities by providing behavioral health treatment in-home, at a support center or in a community. Role Responsibilities (Here’s what it takes)

  • Provide 1:1 care directly with Clients during scheduled sessions under the supervision of a Board- Certified Behavior Analyst (BCBA).
  • Implement established behavioral treatment plans based on the science of applied behavior analysis (ABA) across skill levels, moderate to severe problem behaviors, and service delivery settings.
  • Ensure that 100% of the scheduled direct services are provided without any scheduling interruptions.
  • Maintain accurate and timely documentation of client progress, interventions, and outcomes.
  • Collect data related to Client’s progress during treatment sessions.
  • Incorporate activities and games that are developmentally appropriate and engaging for the client in alignment with the treatment plan.
  • Promote Client engagement to build their strengths in a natural and enjoyable manner.
  • Consulting with case Supervisor on treatment plans, Client updates, and intervention strategies.
  • Receiving and utilizing ongoing training, guidance, and support.
  • Follow program procedures, administrative tasks, direct therapy skills, and other duties that are in line with our compliance requirements.
  • Maintaining reliable communication to record scheduled appointments and return phone calls within 24 hours.
  • Ensuring that the company’s “zero tolerance policy” is understood, implemented, and reinforced in the region.
  • Completing all duties, tasks, and/or special projects assigned by leaders and/or the business.
  • Participating in team meetings, trainings, and professional development opportunities.
  • Providing make up hours as an alternate care team member.
