Cabell Huntington Hospital is seeking a part-time Patient Care Assistant to work with our team in the Float Pool Department. The Patient Care Assistant works in a supporting role to nursing by conducting patient care activities. Applicants provide direct and indirect non-professional nursing care under nursing or medical supervision. Primarily this includes but is not limited to performing duties such as cleaning, taking vital signs, obtaining lab specimens, charting, escorting patients, assisting with activities of daily living, managing clerical duties and ordering and receiving supplies. Candidates interviewed for this role will be asked to present proof of required degrees, certificates and/or certifications at the time of the interview. Candidates unwilling or unable to provide said documentation will not be considered. To be considered, candidates must provide documentation of an 80% or higher score on the Medical Terminology exam, and proof of satisfactory completion of a Medical Assistant degree or Certified Nurses Aide certification. The Clinical Leader demonstrates advanced nursing skills in their respective clinical specialty, as well as the necessary leadership skills to provide functional direction to the PACE nursing team, serve as a clinical resource for staff, and help direct performance initiatives. They will be an advocate for quality patient outcomes and support the mission, vision, and values of Cambridge Health Alliance. Key Responsibilities Conduct comprehensive geriatric assessments for participants with chronic illness, dementia, and mental health conditions. Provide in-home and clinic-based triage, offering coordinated nursing care tailored to the individual needs of each participant. Collaborate with the Nurse Manager to ensure unit organization, quality care, and staff development. Provide leadership to nursing staff, offering guidance on clinical practice, professional growth, and care coordination. Support performance improvement initiatives within the clinical area, ensuring evidence-based nursing practices. Serve as a clinical resource for PACE nursing staff, contributing to optimal patient outcomes. Ensure compliance with all relevant standards of care, policies, and procedures. The Patient Safety Observer role utilizes MedSitter remote patient safety audio and video monitoring technology, to remotely observe up to ten patients simultaneously. The objective of the remote patients' safety monitoring program is to enhance patient safety. Utilizing MedSitter technology, the PSO will observe, communicate and document behaviors that could be disruptive to the patient plan of care.
