Qualifications:

  • Minimum of 5-7 years of progressive experience in clinical practice within social services or behavioral health.
  • At least 3 years of supervisory or management experience in a related setting.
  • Demonstrated expertise in program development, management, and evaluation.
  • In-depth knowledge of community resources, social service systems, and relevant legislation.
  • Strong understanding of mental health, substance abuse, and child welfare issues.
  • Excellent leadership, communication, and interpersonal skills.

    Job Overview

    The Patient Care Assistant provides supportive services to patient care units as directed by the Unit Coordinators, Staffing Coordinators and Registered Nurses. Must consistently demonstrate caring and appropriate behavior when interacting with patients and families. Assists with wound and skin care, keeps unit and work areas stocked, and provides delegated nursing care activities as appropriate.

    Responsibilities

    • Delivers patient care including personal hygiene, nutrition, elimination, mobility, ambulation, comfort and rest.
    • Orients the patient and family to the patient care areas and assists them to feel comfortable with their surroundings.
    • Provides delegated nursing care activities and assists the caregiver as appropriate in daily care, oral hygiene, weights, I&O's.

    Job Summary
    Summary
    Working under the direction of licensed personnel, assists patients with activities of daily living tasks such as bathing, dressing, and eating. Also assists with taking patients' temperature, blood pressure, pulse, and respiration.

    Essential Functions
    -Performs vital signs, weights/heights, intakes, and outputs with accuracy and within established time frame and/or as requested.

    -Completes all documentation in the medical record as required.

    -Interacts with patients and their families effectively.

    -Helps patients with a number of basic hygiene tasks. This may include bathing and dressing patients, helping them use the toilet if needed, and feeding them.

    -Assist with helping patients turn and reposition themselves in bed as needed and help them move between the bed and a wheelchair if they use one.

    -Listens to any health concerns patients may have and report those concerns to the nurses.
