Position Information:
Lee Health Heart Institute is currently seeking a Palliative Care Physician to join the Advanced Heart Failure Center at HealthPark Medical Center in Fort Myers, Florida. The Advanced Heart Failure-Specialized Palliative Care Physician will be working alongside the cardiac team of AHF cardiologists and cardiac surgeons and joining the Palliative Care team that consists of 4 physicians and 13 APRNs. Candidates for this position will be interested in doing coordinated care, medication & chronic symptom management, end of life management, as well as goals of care discussions in heart failure patients; this includes hospitalized patients in cardiogenic shock supported with ECMO, Impella and left-ventricular assist devices, as well as consultations of ambulatory patients in Advanced Heart Failure Center. This is a newly established Advanced Heart Failure program and a great opportunity for someone with an interest in development, program growth and out of the box thinking with a high focus on exceptional patient care to join the team and assist with this strategic redesign process.
