Lee Health Heart Institute is currently seeking a Palliative Care Physician to join the Advanced Heart Failure Center at HealthPark Medical Center in Fort Myers, Florida. The Advanced Heart Failure-Specialized Palliative Care Physician will be working alongside the cardiac team of AHF cardiologists and cardiac surgeons and joining the Palliative Care team that consists of 4 physicians and 13 APRNs. Candidates for this position will be interested in doing coordinated care, medication & chronic symptom management, end of life management, as well as goals of care discussions in heart failure patients; this includes hospitalized patients in cardiogenic shock supported with ECMO, Impella and left-ventricular assist devices, as well as consultations of ambulatory patients in Advanced Heart Failure Center. This group of Endocrinologists and Nurse Practitioners provides a full range of clinical services for conditions such as diabetes, growth disorders, puberty disorders, adrenal and thyroid gland disorders, and obesity. Call will be 1:5. Position is mostly outpatient. You will also serve as the primary liaison between the hospital and Credential Processing Center (CPC) on process. · Knowledge of State, Federal, and JCAHO regulations.
· Previous credentialing and management experience in a healthcare setting preferred.
· Working knowledge of The Joint Commission accreditation process and standards, health care and credentialing industry, regulatory and legal requirements and other national standards preferred.
